STATEMENT OF WORK
CARPET AND COVE BASE INSTALLATION
Building 201; Rooms 1, 2, 3, 4, 5, 6, 22, 43, 105, 106, 107, 133, 210 and 214 7 July 2017
1. DESCRIPTION OF SERVICES. The contractor shall furnish all management, labor, tools, supplies, equipment, transportation, and all necessary items, provide and install new 6.0 inches vinyl cove base, and new SHAW Contract 24.0 X 24.0 inches carpet tiles; Collection – View; Tile Style – Shape;
Style Number - 5T070; Color Name - Zone; Color Number – 69505, in Bldg. 201; Rooms 1, 2, 3, 4, 5, 6, 22, 43, 105, 106, 107, 133, 210 and 214; Nellis Air Force Base, NV 89191. The contractor shall install approximately 9,770 Square Feet of carpet tiles and approximately 1,636 Linear Feet of cove base;
transitions, thresholds, and other materiel as required. The work shall be accomplished in the following order, from first to last: Room 22; Room 133; Rooms 1-6; Room 105-107; Room 43; Room 214; and
Room 210. The work for each area must be completed prior to starting another location. The contractor shall provide the customer samples of vinyl cove base and thresholds of different color, pattern, and type, and the selection will be determined during the post award site visit or at an agreed upon date by both parties. The work shall be performed in accordance with this Statement of Work, Air Force Carpet
Standard Engineering Technical Letter (ETL) 07-4 dated 28 March 2007, Federal, State, and local laws and regulations, and industry standards that may apply. Specific requirements are identified below.
2. SPECIFIC REQUIREMENTS.
2.1. PRE-INSTALLATION REQUIREMENTS. The contractor shall be responsible for verifying all of the requirements of work to be accomplished such as quantities, dimensions, measurements, and any information needed for the performance of this contract. The contractor shall notify the Contracting
Officer Representative (COR) of any conditions such as, but not limited to; depressions, cracks, crevices, holes, ridges between concrete slabs, uneven area, incorrect floor plan, or other floor conditions preventing a proper installation of the carpet and cove base. The contractor must receive approval from the COR before proceeding with performance after finding such conditions. If the contractor discovers such and unforeseen problems associated with the work after starting, immediately notify the COR.
2.2. FLOOR PREPARATION. The contractor shall remove if any; all existing carpet, carpet padding, cove base, transitions, and thresholds before installation of new materials. The contractor shall perform all necessary sub-floor preparation to ensure the carpet will adhere to the floor’s surface and have a smooth appearance. This includes, but is not limited to removing old adhesives, loose particles, and grease. The surface to be covered shall be completely dry and free from moisture prior to installation of the carpet.
2.3. CARPET, COVE BASE, AND THRESHOLD INSTALLATION. The carpet shall be installed
1/4 (quarter) turn throughout, fit securely to the floor and shall have no bulges, wrinkles, bubbles, open seams, or other flaws affecting the carpet’s appearance or serviceability. Cove base shall lie flat against the wall and shall have no bulges, wrinkles, bubbles, wide gaps between connection points, or other flaws affecting the cove base’s appearance or serviceability. Install thresholds properly wherever the carpet meets non-carpeted floors. Inspection and acceptance of the work will not be conducted until three (3) working days after installation in order to allow the carpeting to “settle in.” Excess carpet tiles shall be left with the customer for spare use, and will be provided at the time of the final inspection.
2.4. DOORS. The contractor shall remove, trim and reinstall metal/or wood doors as necessary to ensure it does not rub against the newly installed carpet. Once trimmed, the doors shall swing freely and have a smooth bottom edge to prevent the carpet from damage. If necessary, the contractor shall remove and reinstall kick plate guards to trim doors.
2.5. FURNITURE MOVING. The contractor is required to move office furnishings such as filing cabinets, modular desk, tables, sofa, safes, chairs, partitions, and other related furniture and office equipment; dismantle furniture and wall partitions as necessary to accomplish the carpet installation. The contractor shall reset furniture and equipment to its original location. NOTE: Some safes will be moved by the user.
2.6. THEATER SEATING. For rooms with theater seating, the contractor shall take out the seats to properly accomplish the carpet installation and reset to its original location. NOTE: Do not remove or displace the existing bolts anchored to the ground when detaching theater seating to prevent incorrect placement and misalignment during reinstallation.
2.7. ACCESS FLOOR SYSTEMS. For rooms with raised and detachable flooring, the contractor shall install the carpet tile; only if the same size, directly on top of the raised flooring and match the existing layout. Otherwise, the carpet tiles will be installed the best means possible;
2.8. COMPUTERS AND PHONES. The contractor is not required to move and reset computer equipment and phones; user will be responsible.
2.9. ELECTRICAL APPLIANCES. The contractor is not required to move and reset electrical appliances; user will be responsible.
2.10. ASBESTOS REPORT/ABATEMENT. Macrotec Consulting, LLC; Asbestos and Lead Based
Paint Inspection Report 16634 dated 23 Aug 2016, indicates asbestos present in Rooms 1-6, 22, and 43.
Asbestos abatement for the rooms having asbestos present will be accomplished through a separate contract. Carpet work shall only be performed after the abatement work.
3. CONTRACTOR PERSONNEL. The contractor shall provide a designated work site manager who shall be responsible for the performance of the work and act for the contractor on all matters relating to daily operations of this contract, and shall be available during normal duty hours to meet on the job site with government personnel to discuss issues or concerns. The work site manager must be able to read, write, speak and understand English. The contractor shall ensure that employees are properly and trained to remove and install carpet and cove base.
3.1. IDENTIFICATION OF CONTRACT EMPLOYEES. Contractor personnel shall present a neat and professional appearance, and shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wearing of distinctive clothing or badges.
4. SECURITY REQUIREMENTS. The contractor shall requests a visitor and vehicle pass through the base contracting office at the time of the contract award date. The contractor shall comply with base access REAL ID Act requirements IAW Public Law 109-13, 49 U.S.C., section 30301 and the Code of
Federal Regulations (CFR) Title 6, Part 37. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed, limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.
5. QUALITY CONTROL. The contractor is responsible for quality of workmanship. If the work is not completed in accordance with commonly accepted commercial standards or those standards defined in this contract, the contractor shall re-accomplish the work at no cost to the Government.
6. QUALITY SURVEILLANCE. The COR will assess the contractor’s performance through inspections, and through complaints from customers.
7. GOVERNMENT FURNISHED PROPERTY & SERVICES.
7.1. FACILITIES. None.
7.2. UTILITIES. The Government will furnish electricity, water, and sewage as necessary for accomplishment of work in accordance with this contract. Not all facilities receive the same utility services. The contractor shall not change or modify any utility system or component; or connect any contractor property, equipment, or system without prior CO review and approval.
7.3. MATERIALS. None.
7.4. SECURITY, FIRE, & MEDICAL SERVICES. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available, on a cost reimbursement basis.
7.5. TELEPHONE. None.
7.6. SOLID WASTE COLLECTION & DISPOSAL. None.
8. CONTRACTOR-FURNISHED ITEMS & SERVICES. Except for those items or services specified as Government-Furnished, the contractor shall furnish everything needed to perform this contract according to all its terms and conditions
9. GENERAL INFORMATION.
9.1. HOURS OF PERFORMANCE. The contractor shall work between the hours of 7:00 AM and
4:00 PM, Monday through Friday, excluding federal holidays. Work on weekends or after 4:00 PM is allowed, as required and must be coordinated with the COR and must be approved by the CO.
9.2. RECOGNIZED HOLIDAYS. The contractor is not required to provide service on Federal holidays, except in emergency situations; the holidays are:
New Year's Day - l January
Martin Luther King Day - 3rd Monday in January
Presidents Day - 3rd Monday in February
Memorial Day - last Monday in May
Independence Day - 4 July
Labor Day - 1st Monday in September
Columbus Day - 2nd Monday in October
Veterans Day - 11 November
Thanksgiving Day - 4th Thursday in November
Christmas Day - 25 December
If these holidays fall on Saturday, the proceeding Friday will be observed. If these holidays fall on
Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the contractor will be responsible for rescheduling services for the first day post the holiday observance.
9.3. INTERFACES. Do not unduly interfere with regularly scheduled government operational activities in the performance of contract requirements. In the event a government employee requests temporarily cease work in the area, report the instructions given and the name of the government employee involved to the CO immediately; by the most expedient means. Notify the CO verbally of disputes with customers or other base contractors and follow-up in writing.
9.4. HAZARDOUS MATERIAL MANAGEMENT. The contractor shall not use asbestos or lead containing building materials. Also, the contractor should consider alternative carpet installation systems that use dry adhesives or mechanical bonding to eliminate emissions of odor and volatile organic compounds (VOC), or use low-emitting wet adhesives without a solvent base. The contractor shall maintain a copy of the Safety Data Sheet (SDS) for each hazardous material line item used, and shall maintain spill control material on hand at all times sufficient to contain a worse case spill, both volume and hazard level.
9.5. PERMITS, LICENSES & CERTIFICATIONS. Unless otherwise specified in this contract, the contractor shall obtain necessary permits, licenses, and certifications; give all required notices; and shall comply with applicable Federal, State, County, and local laws, codes, and regulations in performance of the requirements of this contract. The contractor shall maintain records of such requirements and make the documentation available to the Government for review.
9.6. SAFETY REQUIREMENTS & REPORTS. The contractor shall perform work in a safe manner as required by the Occupational Safety and Health Administration (OSHA) safety and health requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to
Government property, or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five (5) calendar days of each occurrence.
9.7. SIGNS. The contractor shall post notifications and/or cordon entrances and affected areas with
“Caution Carpet Work in Progress” signs, during the duration of the work.
9.8. VENTILATION. The contractor shall coordinate with the COR or the facility manager if necessary, to open windows and doors, use exhaust fans, air scrubbers or other means to receive fresh or filtered air to mitigate chemical odor during application and curing of carpet adhesive; and the contractor must supply all electrical appliance/equipment.
9.9. CLEAN-UP. The contractor shall get rid of construction debris and clean-up the work area at the end of each work day. The contractor shall vacuum all carpet at the completion of the installation to remove clippings, fuzz, dust and other remnants. All construction debris resulting from the contractor's operations shall be removed and the entire work area left clean and acceptable.
9.10. WASTE MATERIALS. The contractor shall dispose of all waste material/debris including but not limited to old carpet, old pad, containers, wrappings, and trimmings generated by this contract at an off-base Clark County approved waste disposal site. All waste materials generated by any work under this contract performed on a government installation shall be handled, transported, stored and disposed of by the contractor and by his subcontractors at any time, shall be in accordance with all applicable Federal, State or local laws, ordinance, regulations or other types of rulings.
9.11. DAMAGES TO GOVERNMENT PROPERTY. The contractor assumes full responsibility for any and all government property damaged in whole or part from negligent acts or omissions by the contractor, subcontractor or any employee, agent or representative of the contractor. The contractor shall repair or reimburse the Government at no cost for any and all damage of whatever kind to any and all government property associated with the performance of this contract.
9.12. WARRANTY. The contractor shall install carpet, cove base, padding, and other associated materials in accordance with manufacturer’s instruction to ensure the warranty remains in effect. The contractor shall warranty the carpet against defects due to workmanship for a period of one year.
9.13. PERIOD OF PERFORMANCE. Due to the work center’s high operational schedule and limited availability, period of performance for the entire carpet work mentioned above shall be extended up to a year from date of award. Work for each room shall start; scheduled at an agreed upon date by both parties after the contract has been awarded and completion date is to be determined. The contractor shall comply with paragraph 9.1 of this Statement of Work, if weekend work or work after 4pm is required to meet the performance schedule
