PERFORMANCE WORK STATEMENT

FOR

WATER WELLS 2, 3 AND 5 REPAIR

Cannon Air Force Base, New Mexico

1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, personnel, equipment, tools, materials, vehicles, supervision, qualifications, certifications and other items and services necessary to restore wells 2, 3 and 5 to an operational condition at Cannon Air

Force Base (CAFB), New Mexico. The contractor shall perform the requirements of this PWS in compliance with all federal, state, and local laws and regulations.

1.1 General Requirements. The contractor shall bail each well and run a camera for inspection of each wells’ casings and screens.

1.1.1 The Government believes Well 2 was hit by lightning. There is a direct short to the ground in the submersible pump within the well casing. The well casing is approximately 400 feet in depth. The contractor shall install a new 50 horsepower (HP) submersible well pump and piping to predetermined optimal depth. Once the new well pump is installed, the contractor shall camera and swab both the well casing and screen to remove debris and then conduct a well test in accordance with industry well testing and developmental standards. Existing well specifications: Pump Model - Peerless Submersible 4 Stage 8lb 50 HP 6" Franklin Motor - GPM:

500; Well Depth: 400 Feet; Diameter: 6" X 387'; Column Pipe: 16" X 360"; Head: 380 Feet.

1.1.2 The Government believes Well 3 is running out of water due to the well pumping 400 gallons per minute (GPM) initially but then tappers off to 0 GPM within minutes. The well casing is approximately 400 feet in depth. The contractor shall camera and swab both the well casing and screen to remove debris and then conduct a well test in accordance with industry well testing and developmental standards. The contractor shall determine by the test if the pump is properly working at its current depth. If the contractor deems the pump to be inoperable supported by test results, a new pump will be considered to be installed by a modification to the contract. Existing well specifications: Pump Model - Peerless Submersible 4 Stage 8lb 50 HP 6"

Franklin Motor - GPM: 500; Well Depth: 402 Feet; Diameter: 6" X 387'; Column Pipe: 16" X

360"; Head: 380 Feet

1.1.3 The Government believes Well 5 has the same problems as Well 3 as stated in paragraph

1.1.2 of this PWS to which the well pumps water for a few minutes and then drops to 0 GPM.

Well 5’s pump, power cable, and piping have been pulled out of the well and is on site beside the well. The submersible, well pump and motor, was determined bad and no longer on site. The contractor shall install a new 50 HP submersible well pump and piping to predetermined optimal depth. Once the new well pump is installed, the contractor shall camera and swab both the well casing and screen to remove debris and then conduct a well test in accordance with industry well testing and developmental standards

1.2. Disinfection. The contractor shall disinfect each well with Chlorine in accordance with industry standards. After 24 hours, 27th Special Operations Medical Group Bioenvironmental

Engineering (BEE) flight will conduct bacteriological tests before wells become serviceable.

The results of this test will be provided to the Contracting Officer Representative (COR) and the contractor shall be notified if all three wells are servicable.

1.2.1 The contractor shall ensure that each well head has a proper sanitary seal when all testing has been completed prior to the final bacterological tests conducted by 27th Special Operations

Medical Group BEE flight.

1.3. Documentation. The contractor shall be responsible for creating and maintaining reports that are specifically required by The Office of the State Engineer. This is to include but not limited to all well data, depths, and GPM. All documentation, records, schedules, etc., are the responsibility of the contractor and are the property of the Government and shall remain so even upon termination of the contract. The contractor shall keep these items current at all times.

Documentation and records shall be turned over to the Government upon completion of the contract or when requested by the CO or COR.

1.4. Utilities. The Government will allow the use of electricity (as necessary) and potable water for accomplishment of service IAW this PWS. External electricity on CAFB may be used by the contractor (as necessary) if available.

2. GENERAL INFORMATION.

2.1. Personnel. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. Employees must be clearly able to read, write, speak and understand

English.

2.2. CAFB Access. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. The Pass and Identification Section of

27th Special Operations Security Forces Squadron (27 SOSFS) will provide base entry authorizations/passes to each authorized contractor employee. In addition, contractor personnel wishing to operate a privately-owned vehicle on CAFB must comply with all applicable state and base requirements. Entry authorizations must be renewed as required by the 27 SOSFS Pass and

Identification Section. All contractor base pass requests must be sent to the Contracting Officer

(CO) on the contract from 27th Special Operations Contracting Squadron (27 SOCONS) within

3 business days of the time needed for the pass.

2.3. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes, from employees who depart for any reason before the contract expires

(e.g., terminated for cause, retirement, etc.).

2.4. Vehicles. The contractor shall provide and maintain all vehicles necessary to perform the requirements of this contract. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicles. Vehicles shall be clean and always have a neat, professional appearance. All vehicles shall be in operable condition. Vehicle repairs, other than emergency repairs, shall not be accomplished on CAFB. Equipment failure shall not alleviate the contractor from performing any requirement contained in this contract.

2.4.1. Only qualified operators shall be permitted to operate the vehicles or equipment required by this PWS. Vehicle/equipment operators shall possess a valid driver’s license.

2.4.2. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned vehicles or privately-owned vehicle while on CAFB.

2.4.3. Traffic Laws. Contractor employees shall comply with all base traffic regulations. The contractor is not permitted to use cell phones on CAFB while operating a moving vehicle, unless the device is hands-free.

2.5. Work Hours. The work hours and work days for the period of performance 0700 - 1600, Monday through Friday, with the exception of U.S. Federal holidays (listed below).

Additionally, if the contractor needs to work outside of the normal work hours, a request must be submitted to the CO seven (7) calendar days before requested time.

2.6. Security Forces and Fire Protection Services.

2.6.1. On CAFB: Contact the appropriate agency in case of emergency by dialing 911 from a base telephone. If dialing 911 from a cell phone, the call may be automatically routed to an off-base agency, such as the city of Clovis, NM. Be clear about whom you are communicating with and what your situation requires.

2.7. Environmental Management System (EMS). All contractor personnel shall complete

EMS Awareness training prior to commencement of any activities. Training is available through the learning management system called The Environmental Awareness Course Hub (TEACH) at https://usaf.learningbuilder.com/account/login/?ReturnUrl=%2f. The contractor shall submit the

Awareness Training Certificates of Completion to the COR prior to any on-base performance under this contract. Use of Microsoft Edge, Google Chrome or Mozilla Firefox is required to print certificates.

2.8. Green Procurement Program (GPP). In performance of this contract, the contractor shall use Environmental Protection Agency (EPA) designated recycled content products, Information https://usaf.learningbuilder.com/account/login/?ReturnUrl=%2f

Technology (IT) Energy Star products/appliances, Federal Energy Management Program

(FEMP) Designated Energy Efficient Low Stand By Power products/appliances, U.S.

Department of Agriculture (USDA) Bio based/Bio preferred products, Environmentally preferable products, Electronic Product Environmental Assessment Tool (EPEAT) registered products, Water Sense or other water efficient products, non- or Low Ozone depleting substances under the Significant New Alternatives Policy (SNAP), non or Low toxic or hazardous constituents (e.g. non-VOC paint) and any other environmentally sustainable product/method, to the greatest extent possible.

2.9. Hazardous Materials Handling. The contractor shall comply with all federal, state and local regulations (including Cannon Air Force Base & Melrose Air Force Range Civilian

Contractor Environmental Guide) concerning the use, storage, and reporting of Hazardous

Materials (HM). In accordance with AFI 32-7086, Hazardous Materials Management, the contractor shall provide the following information to the CO for all HM to be used:

(1) Product identification or label

(2) Safety Data Sheet (SDS)

(3) Estimate of quantity to be used

(4) Maximum quantity to be stored

(5) Storage location

2.9.1. The above information must be submitted to the CO on a Cannon AFB Contractor Hazmat

Worksheet and Cannon AFB Contractor HM Usage Worksheet provided upon award. The contractor may not bring a HM onto Cannon AFB, nor use a HM, until the contractor receives all required authorizations. This HM information shall be submitted 30 calendar days prior to the

HM being brought onto CAFB. The contractor shall submit monthly reports thereafter, by the fifth business day after the completed month, to the CO that contains actual usage information.

The Government may determine after review of the initial report that follow-on reports are not necessary. For definition of HM, the contractor shall refer to FED-STD-313D. HM includes pesticides and cleaning supplies. The contractor shall not use EPA-prohibited chemicals and shall be encouraged to not use products which result in the generation of hazardous waste, air or water pollution. Class I Ozone Depleting Substances shall not be used for any requirement of this contract. The contractor shall maintain a copy of current SDSs readily available to employees for HM in use or being stored. The contractor shall be OSHA compliant.

2.9.2. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws/regulations come into effect. The contractor shall notify the CO at earliest knowledge of the law/regulation change.

2.10. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302, Designation, Reportable Quantities, and Notification into the environment, the contractor or its agent shall immediately report the incident to Cannon AFB

Fire Dept. at 911. The liability for the spill or release of such substances rests solely with the

Contractor and its agent.

2.11. Freedom of Information Act (FOIA). The contractor shall comply with DoD Directive

5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding For Official Use Only (FOUO) material.

2.12. The Force Protection Condition (FPCON) level may change for real-world events or for exercise scenarios which may prevent contractor employees from entering or leaving CAFB until the situation has lessened. Depending upon the situation, contractor employees shall react to

FPCON requirements and comply based on 27 SOW guidance provided through the CO or COR.

Continuation of Essential DoD Contractor Services. This service is determined to be non-essential for performance during a crisis. In the event of a crisis, the contractor will be notified by the Contracting Officer of the need to discontinue services due to contingency, base closure or other causes. If notification to discontinue services is not received, the contractor will be expected to continue performance in accordance with this PWS.

2.13. Contractor Manpower Reporting Requirements. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs 1 October through 30

September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the help desk at:
