This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and

A WRITTEN SOLICITATION WILL NOT BE ISSUED.

AMENDMENT 0001 (27 Mar 2016) – The following changes have been incorporated.

1. Included detailed instructions on which solicitation attachments must be submitted with the quotation.

2. Included a statement in PWS Section 4.5.1 clarifying that the two (2) athletic trainer positions will be full-time positions, requiring a minimum of 40 hours per week.

3. Added Attachment 7, Responses to contractor questions.

4. To avoid confusion, questions previously submitted must be resubmitted if they have not been addressed in solicitation amendment.

AVAILABILITY OF FUNDS

The Government intends to award for this service, HOWEVER FUNDS ARE NOT PRESENTLY AVAILABLE FOR THIS CONTRACT. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.

The solicitation number for this procurement is FA4830-18-Q-S005 and is issued as a Request for Quotation (RFQ).

All references to offer/offeror are deemed to have the meaning of quote/quoter and any quote received in response to this RFQ is not binding until contractor signs the purchase order issued at time of award or starts performance.

This RFQ and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-96 (Correction), Defense Acquisition Circular (DAC) 20171228, and Air Force Acquisition Circular (AFAC) 2017-1003. The North American Industry Classification System (NAICS) is 621340. The small business size standard is $7.5 million. This acquisition will be set aside 100% for Small Business in accordance with Federal Acquisition Regulation (FAR) 19.502-2(a).

Note: FAR 52.222-17 Nondisplacement of Qualified Workers applies. Quoters may contact the primary or additional points of contacts listed for this acquisition for contract information of current employees. Quoters will have access to employee information of only those employees who have consented to release this information. All current employees of the incumbent contractor may be considered.

Description: Certified Athletic Trainers shall assist in the training of tactical athletes to enhance and optimize human performance by focusing on the full integration of human performance capabilities in the delivery of athletic training/physical therapy. The Athletic Trainer under the direction and supervision of the 347th Rescue Group (RQG) Flight Surgeon Officer in Charge (OIC) shall collaborate with the Rescue Group team to treat, screen, optimize human performance, prevent injuries and rehabilitate injuries. This requirement is a personal service.

Base Year: 01 August 2018 – 31 July 2019 Option Year: 01 August 2019 – 31 July 2020

Response Time: Quotations will be accepted electronically no later than 2:00 PM Eastern on Wednesday, 9 April 2018. All quotations shall be marked with the RFQ number and title. Email quotations shall be sent to both the primary and all alternate points of contacts identified below.

The provision at FAR 52.212-1, Instruction to Offerors – Commercial Items, applies to this acquisition.

Quotations shall contain the following information: RFQ Number; Time Specified for Receipt of Quotations; Name, Address, and Telephone Number of Quoter; Name and Email address of Representative authorized to discuss quote; Any Discount Terms; and Acknowledgement of all Solicitation Amendments (if applicable). Attachment 3, Contractor Insurance and Attachment 4, Base Pass Request are not required to be submitted with the quote. They are provided for informational purposes only and will be requested from the awarded contractor prior to contract performance. The quoter is required to appropriately mark and sign Attachment 5, the SFLLL and return with their quote. Instructions on how to fill out this form is provided on page 2 of the attachment. Information requested in Attachment 6, Contractor Financial Information also need to be submitted with the contractors quote. Quotations shall also include the following required documentation:

1. Price: Quoted prices must be submitted using the Pricing Schedule at Attachment 2.

Quoters shall insert unit prices in the attached pricing schedule, rounded to the nearest cent (for example $0.02 not $0.0231). Quoters rejecting the terms and conditions may be excluded from consideration.

2. Technical: The quoter must submit a resumes for two (2) Athletic Trainers demonstrating that the Athletic Trainers have the following minimum qualifications:

a) A Bachelor’s or a Master’s degree in Athletic Training from an accredited institution.

b) Certified by the Board of Certification for the Athletic Trainer and state licensed or eligible for state license.

c) Have at least three (3) years of professional experience working with college level or higher athletes and/or tactical military personnel of which one (1) year of full-time experience must be within the last three (3) years. Candidates with Master’s degrees require only a minimum of two (2) years professional experience working with college level or higher athletes and/or tactical military personnel.

d) Certified in Instrument Assisted Soft Tissue Mobilization. Some examples of certification could be Graston, ASTM, SASTM

NOTE: The purpose of obtaining resumes is to demonstrate the quoter can identify a pool of fully qualified Athletic Trainers that meet of the PWS. It is not to demonstrate compliance with “Nondisplacement of Qualified Workers” clause. Resumes must be submitted; resumes can be but do not have to be resumes of incumbent employees. The submission of any resume will not imply or require employment under any future contract. Statements such as potential employees are qualified, or that qualified employees will be hired are not resumes and as such will not meet this requirement.

3. Past Performance: The contractor may submit no more than three (3) past performance references. References should be for services similar in scope to this requirement that have occurred in the last five years prior to the closing date of the solicitation. References for past performance shall include a point of contact, their phone number, email address, name of the company, contract number (if applicable), initial contract cost and period of performance.

Evaluation: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a contract to the quoter whose quotation is judged to represent the best value to the Government based on Price, Technical Factors, and Past Performance.

Price: Evaluation of price will consider the total of the annual prices for all contract years (base plus all options, including a possible six-month extension), with each year’s price evaluated as the sum of the prices of all contract line items (CLINs) for that year. Each CLIN price will be evaluated as the quantity multiplied by the unit price, rounded to the nearest cent. Pricing will not be provided by the contractor for CLIN 0003 and CLIN 1003. These CLINs will be estimated at the price provided by the Government. Overtime CLINs – Contractor shall sum up all overtime costs utilizing the estimated hours provided by the Government (estimated 25 hours) to provide a total amount. The Overtime CLINs are as follows: CLINs 0004 and 1004. A potential six-month extension IAW the clause FAR 52.217-8 will be evaluated at the pricing of the final option year. In the event of an extension under this clause, it will be exercised at the prices of the prior contract year.

Technical Factors: Technical will be evaluated to determine if the resumes of the Athletic Trainers meet the minimum qualifications as outlined in items a-d above.

Past Performance: Evaluation of past performance will consider past performance information obtained from any suitable source, such as Air Force automated past performance data bases (i.e.

CPARS, PPIRS), direct local business experience with quoter, or similar.

Site Visit: A site visit will not be held.
