1. DESCRIPTION OF SERVICES.
1.1. The Contractor shall provide one (1) Local Area Network (LAN) Administrator to perform on-site services for various information management/information technology (IM/IT) support activities for the 23 Medical Group (MDG), also referred to as the Medical Information Services Support (MISS). The LAN Administrator shall perform to or meet standards and provide support consistent with the policies, procedures, and standards of the Department of Defense (DoD), Air Force (AF), 23d Wing, 23 MDG, and the 23d Communications Squadron.
2. REQUIREMENTS.
2.1. Life Support Certification. At a minimum, the employee shall maintain current certification in either the American Heart Association Basic Life Support (BLS) Course C or the American Red Cross CPR/BLS and be maintained in the personnel training folder.
2.2. Listing Of Employees. The Contractor shall maintain and provide to the CO/COR a current listing of employees. The list shall include the employee's name, social security number, and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the Installation Commander. An updated listing shall be provided when an employee's status or information changes.
2.3. Security Clearance. At a minimum, the employee must have a Secret Security Clearance.
2.4. Microsoft Certified System Administrator (MCSA). The employee must have an MCSA with at least 2 years of experience with LAN Administrator duties. Documentation must be provided.
2.5. CompTIA Security+, CompTIA Network+, and CompTIA A+. The employee must have all three certifications. Documentation must be provided.
2.6. Optional but highly recommended.
2.6.1. CISSP – Certified Information Systems Security Professional
2.6.2. BSCS – Bachelor of Computer Science
2.7. Key Personnel. At least 30days prior to removal of any Contractor personnel from this contract, the Contractor must identify potential replacements and describe qualifications to assume duties. The Contractor shall replace personnel without interruption of services required by this contract.
2.8. English Language Requirement. All contract personnel providing services under this contract shall read, write, understand, and have effective verbal and written communication skills.
3. RESPONSIBILITIES.
3.1. Service Requirements for 23 MDG.
3.2. Systems Administration. The Contractor shall provide support for the installation, maintenance, configuration, and administration of the information systems and network servers.
Currently, the network consists of over 10 servers, including, but not limited to, Remote Access Servers (RAS), Fax Servers, Print Servers, Compact Disc (CD) Towers, File, Print and Application Servers. Also, new servers that are introduced to 23 MDG as needed.
3.2.1. The Contractor shall support the functionality, operation, administration, and end-user requirements for the Integrated Clinical Database (ICBD) Servers, Coding Compliance Editor, Dental Imaging Collaborative Environment (DICE/MiPACS), AGFA digital radiology, Air Force Medical Service (AFMS) systems, as well the Military Health System (MHS) systems as needed. This includes, but is not limited to, system administrator duties, application development support, end-user support, and report development and processing support, as required.
3.2.2. As determined by the 23 MDG the Contractor shall serve as the 23 MDG’s point of contract (POC) for applications developed/implemented within the organization and by other organizations (e.g., Health Affairs, TRICARE Management Activity, etc).
3.2.3 Detailed activities, essential tasks, and knowledge responsibilities shall include, but are not limited to, performance of the following:
3.2.4. Manage, administer, and support the 23 MDG’s servers, applications, hardware operation, operating system software, and system/user support.
3.2.5. Assist in the installation of data center equipment, network devices, and peripheral devices; test and certify new revisions of the operating system, and layered product software.
3.2.6. Experience with more than one (1) server operating system and internetworking between operating systems; e.g., Microsoft’s Disk Operating System (MS-DOS), and its Windows XP, Vista, Windows 7, and 2003 server family of operating systems, its Exchange family of electronic mail systems, and its SharePoint system; UNIX; and Virtual Memory System (VMS).
3.2.7. Ability to write scripts in some administrative language, e.g., Practical Extraction and Report Language (PERL), Win Script, or other scripting applications.
3.2.8 Experience with complex transport control protocol/internet protocol (TCP/IP) networking protocols, e.g., Windows Internet Naming Service (WINS), Dynamic Host Configuration Protocol (DHCP), VPN, UNIX, Domain Name Service (DNS), Point to Point Protocol (PPP), Serial Line Internet Protocol (SLIP), and VMS.
3.2.9. Experience with and ability to appropriately configure Redundant Arrays of Inexpensive Disks (RAID).
3.2.10. Thorough understanding of and experience in the Open System Interconnection (OSI) model.
3.2.11. Install, configure, modify, update, upgrade, and troubleshoot various internetworking hardware and software including, but not limited to, production and test hubs, routers, switches, bridges, firewalls and VPN devices in a LAN/WLAN/VPN environment.
4.0. TASKS.
4.1. Systems Administrator
4.1.1. Eliminate excess connections, clean-up current cables, and label essential drops in the communications rooms. Remove hubs and other network equipment to longer being used in the communication rooms.
4.1.2. Maintain library of current scripts, software media, licenses, and documentation.
4.1.3. Develop and maintain documentation on network administration processes, e.g., current network diagrams, copies of device configurations, and standard operating procedures for tasks required in this PWS. To include a hard copy.
4.1.4. Troubleshoot and resolve LAN/WAN connectivity problems.
4.1.5. Perform, maintain, and document Disaster (Contingency) Recovery Procedures, e.g., maintain system disc integrity, perform backups and fail-over tests, monitor system performance, and conduct capacity planning and general troubleshooting.
4.1.6. Create and maintain all Medical Group user accounts in Active Directory.
4.1.7. Install all Microsoft and other software vendors critical security patches on all servers as applicable.
4.1.8. Support cell phones (iOS devices, etc.) management and establish, disable, and transfer accounts, etc.
4.2. Network Administrator. The LAN Administrator shall provide support in the administration, configuration and maintenance of Local and Wide Area Network (LAN/WAN) and Virtual Private Network (VPN) equipment and software. This includes, but is not limited to, hubs, routers, switches, fiber and copper wiring, serial communication lines, and other communications systems. The LAN Administrator shall ensure security procedures are implemented and observed and assist the 23 MDG in establishing and implementing LAN/WAN/VPN policies, procedures, and standards.
4.2.1. Implement and maintain the 23 MDG’s LAN/WLAN/VPN-related security policies.
4.2.2. Manage and participate in network upgrade and expansion planning, as needed.
Knowledge and experience in MIS Project Management Concepts and associated project tracking software.
5. INFORMATION ASSURANCE AWARENESS TRAINING AND EDUCATION. The Contractor shall complete initial information assurance training before gaining access to any Government information system. The Contractor shall also complete annual information assurance refresher training to ensure functional competency related to threats and system vulnerabilities, and protective measures taken to counter such disruptions. The 23 MDG shall arrange access to the training which shall be completed at the place of performance on the first (1st) day of the performance period and annually thereafter.
6. HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA)
OF 1996. All individuals performing services at an Air Force MTF are required to complete and initial an annual refresher HIPAA Privacy and Security Rule training as provided by the MTF for its personnel and will be held accountable for complying with health information privacy and security policies and procedures, reference Section H for detailed HIPAA language.
7. SERVICES SUMMARY (SS). The LAN Administrator services summary requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum standards of service required for each objective. These thresholds and objectives are critical to mission success. The LAN Administrator will be inspected/assessed on the items listed in this Table, as well as other requirements of the contract.

Technical Support Service.

LAN/WLAN is operating 100% of standard work schedule hours.
All computer stations are connected to the LAN/WLAN.
System uses the most recent technology. All users have e-mail accounts. All users have access to their applicable network drives. All data is sufficiently protected from system failure or disaster. The contractor shall notify management of all network outages anticipated to last more than 30 minutes.
Respond to Government request for assistance within 8 hours 90% of the time during standard duty hours. Valid 23 MDG complaints concerning execution of all work orders shall not exceed 1 per month.
Inform of network outages within 30 minutes 95% of the time.

Database Maintenance & Management.
System shall be available 95% of the time, during standard duty hours.

Clean-up networking wiring in the communication rooms.

Not later than the last business day of the month.

Remove excess equipment from computer rooms.

Network Device Configuration File.
Maintain a binder containing hard copies of the configuration of each network device on the 23 MDG’s network, updating as changes are made in a device configuration.
(1) Not later than the last business day of the quarter; (2) and updated within ten (10) business days of a change in any device configuration.
SS#9.
Document Disaster(Contingency) Recovery Procedures.
Document all Disaster Recovery Procedures for all backed-up servers.
8. GOVERNMENT- FURNISHED EQUIPMENT, FACILITY, MATERIALS,
PROPERTY, RESOURCES, SERVICES.
8.1. Documentation/Publications. The 23 MDG shall provide the LAN Administrator copies of, or access to, all required directives, publications and documents for this task. All documentation provided to the LAN Administrator shall remain the property of the 23 MDG and shall be returned upon completion of this contract.
9. RECORDS.
9.1. Records Management.
9.1.1. All records, files, and documents, regardless of media (e.g., paper, electronic, etc), as described in this PWS, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.
9.1.2. The Contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, paragraph 10, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule which may be accessed from the following web address:

9.1.3. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.
The Moody AFB Records Management Office (23 CS/SCXKR), 257-3500, will provide assistance and training to the Contractor to ensure compliance with federal record keeping requirements.
10. CONTRACTOR MANPOWER REPORTING. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided uder this contract for Customer Support Specialist at Moody AFB via a secure data collection site. The Contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year beginning with 2015.

11. FACILITIES, MATIRIALS,SUPPLIES AND SERVICES. The 23 MDG will provide office supplies, work areas, forms, desks, reproduction services, access to PCs and other IT equipment, and telephones and telephone service (including commercial long distance) for this task only for official business, all at no cost to the Contractor.
11.1. Obtaining Replacement of Government-Furnished Equipment. The Contractor shall direct request(s) for replacement of Government-furnished equipment to the COR for processing.
Such requests shall specify the reason for the replacement request.
11.2. Equipment Accountability. The Contractor shall be liable for loss or damage to Government- furnished property and equipment beyond fair wear and tear in accordance with the clause of the contract. The Contractor shall report all lost, damaged or destroyed Government-furnished equipment to the CO within seventy-two (72) hours of discovery. In the case of loss or damage beyond economical repair to property and equipment, the CO shall determine the amount of the Contractor’s liability.
11.3. Equipment Turn-In. The Contractor shall coordinate any equipment to be submitted to the Defense Reutilization Marketing Organization (DRMO) with the COR and CO.
11.4. Telephone Services. Use of Government-furnished telephone(s) is provided solely for official Government business and shall not be used to transact personal business by the Contractor or his employees. The Contractor shall contact the COR for repair service. The Government will not be responsible for charges for long distance telephone calls made or accepted by Contractor personnel. Telephone services will be subject to the standard monitoring requirements of the Government telephone network.
11.5. Refuse Collection. The Base refuse and recycling contractor is responsible for the collection and removal of all trash and refuse. However, when the Contractor finds it necessary, at times, to dispose of refuse and trash generated as a result of this contract requirement, the Contractor may use the base dumpsters.
11.6. Recycling Program. The Contractor will be required to participate in the Base Recycling Program.
12. EMERGENCY MEDICAL SERVICE. The Moody Air Force Base Medical Treatment Facility (MTF) will provide emergency medical services to Contractor personnel, if required, on a reimbursable basis. The Contractor shall be held responsible to reimburse the Government.
13. GENERAL INFORMATION.
13.1. Quality Assurance. A systematic plan of all actions necessary to provide confidence that services will conform to established requirements and satisfactory performance is achieved.
Quality assurance refers to those actions taken by the Government. The Government Contracting Officer Representative (COR) shall inspect the Contractor’s performance.
Inspections shall be conducted in accordance with the Quality Assurance Surveillance Plan (QASP) with the intent of verifying compliance with contract requirements. Contract requirements not listed on the SS will be inspected also during the performance period of the contract. The Government will record all surveillance observations. When observation indicates defective performance, the COR will require the contract manager or representative at the site to initial the observation. This initialing of the observations does not necessarily constitute concurrence with the observation only acknowledgment that he or she has been made aware of the defective performance. If performance defects are identified and validated by the COR, the Contractor shall be provided the opportunity to re-perform. Disputes between the COR and Contractor personnel over performance standards shall be referred to the CO for final determination. The Government reserves the right to inspect tasks not shown on the SS.
13.2. Quality Control Program. The Contractor shall develop and maintain a quality program to ensure all services are performed in accordance with all applicable standards and codes. The
Contractor shall develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in paragraph 2--Services Summary (SS).
13.3. Quality Control Plan. The Contractor shall provide a .Quality Control Plan (QCP) for all work required in this PWS. The written QCP shall be submitted to the CO for validation no later than the pre-performance conference date. The CO will notify the Contractor of validation or if any required modification(s) is needed. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in paragraph 2--Services Summary (SS).
13.3.1. The plan shall include, but is not limited to, the following:
13.3.2. A description of the inspection system to cover all services listed on the service summary (SS). Description shall include specifics as to the areas to be inspected on both a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspectors. Additionally, control procedures for any Government-provided keys or lock combinations shall be included.
13.3.3. A description of the methods to be used for identifying and preventing defects in the quality of service performed.
13.3.4. A description of the records to be kept to document inspections and corrective or preventive actions taken.
13.3.5. The records of inspections shall be kept and made available to the Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract. Refer to Records Management paragraph 9.1.
13.4. Contractor Employees. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer (CO) as a potential threat to the health, safety, security, well-being or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily identified as contractor employees. The contractor shall furnish an identifying badge, which shall include as a minimum, the person’s name, the name of the contractor, and the function. Each employee shall wear the badge on the outer garments at all times.
13.4.1. The Contractor understands that, while the Military Treatment Facility (MTF) Commander may allow the contract employee to temporarily occupy non-critical sensitive positions pending the National Agency Check with Inquiries (NACI), the contract employee will be immediately removed from the position if at any time the NACI receives unfavorable adjudication, or if other unfavorable information that would affect the NACI becomes known.
14. GENERAL SECURITY REQUIREMENTS.
14.1. Pass and Identification. The Contractor shall ensure each contract employee(s) obtain the required pass and identification items as applicable for Contractor personnel. The Security Police Pass and Identification Office located at Moody's South Gate, Building 1030, shall issue these forms. The Contractor shall retrieve all identification media from employees who leave contract employment for any reason before the contract expires; e.g., termination, retirement, etc.
These items will be returned to the COR. The Contractor shall maintain a current listing of all their employees. The list shall include employee names and social security numbers. This list shall be provided to the CO and the 23d Security Forces Squadron at the contract start date. An updated listing shall be provided upon any change of status of any employee.
14.2. Keys. The Contractor shall prohibit the use of keys issued by the Government by any persons other than the Contractor's employees and the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in performance of contract work requirements in those areas.
14.3. Traffic Laws. The Contractor and its employees shall comply with base speed limits and other traffic laws.
14.4. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their Contractor-owned or privately-owned vehicle while on Moody AFB, GA.
14.5. On-base Driving Requirements. Contractor shall ensure employees have current and valid professional certifications before starting work under this contract, including: Vehicle operators shall have a valid driver's license. All vehicles shall be operated in accordance with base traffic regulations.
15. SAFETY REQUIREMENTS.
15.1. Federal Occupational Safety and Health Act (OSHA).
15.1.1. The Contractor shall comply with all Federal Occupational Safety and Health Act (OSHA) requirements and standards as published in the 29 Code of Federal Regulations (CFR).
The CO, COR, Base Civil Engineer, Safety, Bioenvironmental, or Security Police representatives may stop employees from performance of any work that poses an immediate threat to employees or Base personnel, and shall immediately contact the COR or CO. The CO will make a determination of whether the Contractor shall be permitted to resume work or take immediate corrective action. Other safety or health violations that do not cause an imminent threat shall be reported to the CO. Any delays related to such violations are not excusable. Contractor will not be permitted to resume work until unsafe conditions are satisfactorily corrected at the direction of the CO. Contractor shall also be liable for any costs associated with delays or re-performance of work.
15.2. Cell Phone Usage. The use of a cell phone while driving on base is prohibited, unless a completely “hands-free” device is utilized.
15.3. Smoking Policy. All Contractor employees shall comply with Air Force smoking policy (AFI 40-102, Section 2), which states smoking is not permitted in any DoD facility.
16. HOURS OF OPERATIONS AND PLACE OF PERFORMANCE.
16.1. Standard Hours of Operation. The Contractor shall perform the services required under this contract during the hours of 7:30 a.m. through 4:30 p.m., Monday through Friday (excluding holidays and emergencies).
16.1.1. Place of performance : Medical Treatment Facility Bldg 905, 3278 Mitchell Blvd, Moody Air Force Base, GA, 31699
16.1.2. Contractor personnel shall initially respond telephonically to the Government’s request for after-duty-hour support within two (2) hours.
16.1.3. Emergencies may occur at any time of the day or night, and on any day of the week.
SPECIAL EVENTS require services during the events which usually occur during night hours and weekends.
16.1.4. The Government reserves the right to establish specific days and times for performance of any work under this contract. Presently all work is performed during dayshift hours, with the following EXCEPTIONS:
16.2. Holidays. The Contractor is not required to provide service on the following days:
New Year’s Day 1 January

16.2.1. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a
Sunday, it is observed on Monday.
16.3. Holiday Services. Services scheduled for a certain day of the week, but cannot be performed due to a holiday, shall be performed either the day before or the day after the holiday.
If a service is scheduled for Monday and that Monday is a holiday, then the service shall be performed on the day after the holiday (Tuesday). If services are scheduled for Friday and that holiday is on Friday, then the services shall be performed on the Thursday before the holiday.
16.4. Family or Down Days. The Contractor shall coordinate with the COR and CO regarding these days. If the COR or another Government representative will not be on duty, arrangements shall be made in advance to make-up any hours not worked on a missed day or the Contractor employee shall take leave (paid or unpaid) due to the base being minimally manned on these days.
The Contractor may substitute the “Good Friday” holiday time-off for the day after Thanksgiving, which is usually considered a “family day” for the general base population. If the Contractor decides to take advantage of this option, this arrangement must be finalized between the CO, Contractor, and COR two (2) weeks prior to the holiday. If substitution is used, the Contractor shall perform scheduled services on Good Friday at no additional cost to the government
17. CONTINUATION OF ESSENTIAL CONTRACTOR SERVICES DURING CRISES.
17.1. Services.
17.1.1. All services under this PWS shall be performed during any wartime operations.
Wartime operations are those actions, including contingency planning, which would be required to support current or future United States Air Force wartime requirements. The CO or the COR shall notify the Contractor of a crisis by any means of communication. There shall be no costs paid by the Government to the Contractor during a crisis except those incurred for work performed under this contract. Notwithstanding any other provision of this contract, the contractor agrees that, in the event of the occurrence of any wartime-related contingency, the Government shall have the unilateral right to extend performance of this contract as long as the wartime contingencies exist. Contract payment will be in accordance with the existing contract rate.
17.1.2. The Government will notify the Contractor within three (3) hours of any crisis declared by the national combatant commander by telephone, email, or in person.
17.1.3. The Contractor shall prepare a list of all essential positions and personnel required to support any crisis and submit to the CO at the pre-performance conference.
17.1.4. The Contractor providing services designated as “essential” by Moody AFB is expected to use all means at their disposal to continue to provide such service in accordance with the terms and conditions of the contract during periods of crisis until appropriately released or evacuated by the CO.
17.1.5. The Contractor is responsible for providing the requested information as required in the DFARS Clauses 252.237-7023 and 252.237-7024 of this contract.
18. CONTRACT CHANGEOVER. The Government reserves the right to conduct site visits in all Contractor-operated facilities in conjunction with the solicitation of offers for any follow-on-contract. In the event the follow-on-contract is awarded to other than the incumbent, the successor or Contractor is responsible for obtaining all information from the incumbent Contractor, necessary to permit a smooth transition without an interruption in service during the transition period. Additionally, the incumbent Contractor shall cooperate to the extent required to permit an orderly changeover to the successor Contractor. With regard to the successor Contractor's access to incumbent employees, a recruitment notice may be placed in each facility.
19. REMEDIES FOR NONCONFORMING SERVICE(S). Procedures for performance that does not meet contract standards shall be as cited in accordance with FAR 52.212-4, Contract Terms and Conditions—Commercial Items (a) Inspection/Acceptance. The Contractor shall only tender for acceptance those items that conform to the requirements of the contract. The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or re-performance of nonconforming services at no increase in contract price. If repair/replacement or re-performance will not correct the defect(s) or is not possible, the Government may seek an equitable price reduction or an adequate consideration for acceptance of nonconforming supplies or services.
