STATEMENT OF WORK

Maintain Flooring, Base Wide

XLWU 15-2053D

Tyndall Air Force Base, Florida

21 July 2017

1.0. SCOPE: Contractor shall furnish all labor, materials, transportation, tools, equipment, and supervision needed to replace flooring at Buildings listed below on Tyndall Air Force Base, Florida.

2.0. DESCRIPTION OF WORK:

2.1. Building 431

2.1.1. Remove existing carpet and transition strips. See attached drawing.

2.1.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.1.3. Install new Interface, Cubic, Horizontal carpet.

2.1.4. Install new Burke Flooring, #607, Cranberry transition strips at storage room, both ends of hallway and projection room door.

2.1.5. Large amount of equipment in room 104 and bolted down seating in room 101 to be moved as needed to install new carpet.

2.2. Building 505

2.2.1. Remove existing carpet, VCT in Comm. closet, cove base and transition strips. See attached drawing.

2.2.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new LVT.

2.2.3. Install new Armstrong, Bravo Putty 12 x 24, LVT.

2.2.4. Install new Burke Flooring, #701, Black, cove base.

2.3. Building 1255

2.3.1. Remove 2 layers of existing carpet, cove base and transition strips.

See attached drawing.

2.3.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.3.3. Install new Interface, Cubic, Horizontal carpet.

2.3.4. Install new Burke Flooring, #597, Mocha cove base and transition strips at restrooms and double entrance doors.

2.3.5. Personal lockers in room 1 to be moved as needed to install new carpet.

2.4. Building 421

2.4.1. Remove carpet in all areas, cove base in room 403B and transition strips at rooms 208, 212 and 214. See attached drawing.

2.4.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.4.3. Install new Interface, Cubic, 6392 dimension carpet.

2.4.4. Install new Johnsonite, 92-Blue Lagoon, cove base and transition strips.

2.5. Building 1470

2.5.1. Remove existing carpet to bottom of riser at start of repair area on pulpit and at the balcony. See attached drawings.

2.5.1.1. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.5.1.2. Install new Milliken, Colorweave, Colored Crystal, 200

Ruby Gem to match existing.

2.5.1.3. Move large and small organ, piano, table, chairs, pews at balcony etc. as needed to replace carpet.

2.5.1.4. All risers and step shall have carpet.

2.5.2. Repair carpet in Sanctuary. See attached drawing.

2.5.2.1. Replace 12 stained, torn carpet tiles with new tiles.

2.5.2.2. Repair loose tiles and joints which are buckling at

approximately 30% of the area.

2.6. Building 703, Chapel 1 – Main Level, Balcony, stairs and steps

2.6.1. Remove existing carpet and transition strips. See attached drawing.

2.6.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.6.3. Install new patcraft, Scholastic II, Tuition 00471 carpet.

2.6.4. Install new Burke Flooring, #323, Bluebonnet transition strips at main area entrance and storage rooms’ doors.

2.6.5. Move pews in main area as need to install new carpet.

2.7. Building 2893

2.7.1. Remove existing carpet, cove base and transition strips. See attached drawing.

2.7.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.7.3. Install new Milliken, Ghost Artist, Facade, Conceal carpet.

2.7.4. Install new Johnsonite, #20, Charcoal, cove base and transition

2.8. Building 432

2.8.1. Remove existing carpet in rooms 128, 129 and 130, cove base in all areas and transition strip room 128 entrance. See attached drawing.

2.8.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.8.3. Install new Interface, Cubic, Dimension carpet in all areas.

2.8.4. Install new Burke Flooring, #701, Black, cove base and transition strip.

2.8.5. Room 128 has lockers and room 129 has large work benches to be moved as needed to install new carpet.

2.9. Building 9400

2.9.1. Remove existing carpet and transition strips. See attached drawing.

2.9.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.9.3. Install new Milliken, Ghost Artist, Facade, Conceal carpet.

2.9.4. Install new Johnsonite, #20, Charcoal, cove base and transition

2.10. Building 649

2.10.1. Remove existing carpet, cove base and transition strips. See attached drawing.

2.10.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.10.3. Install new Interface, Cubic, Dimension carpet.

2.10.4. Install new Burke Flooring, #660, Rocky, cove base in all areas and transition strips at all doors.

2.10.5. Move equipment, cabinets, etc. in rooms 101, 119,121 and 123 as needed to install new carpet.

2.11. Building 164

2.11.1. Remove existing carpet, vinyl cove base and transition strips. See attached drawing.

2.11.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.11.3. Install new Interface, Cubic, Balance carpet.

2.11.4. Install new Burke Flooring, #323, Bluebonnet, cove base in all areas except rooms 140, 141, 142 and 143 and transition strips at all doors from corridor excluding Vault area.

2.11.5. Lockers in room 131 and work benches in room 128 to be moved as needed to install new carpet.

2.11.6. Work in Vault area shall be done on weekends only.

2.11.7. Building is located in the Airfield Controlled area.

2.12. Building 7025

2.12.1. Remove existing carpet, vinyl cove base and transition strips. See attached drawing.

2.12.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.12.3. Install new Interface, Cubic, Horizontal carpet.

2.12.4. Install new Burke Flooring, #597, Mocha, cove base in all areas and transition strips at room entrance doors.

2.12.5. Building is located in a Secure Area.

2.13. Building 290

2.13.1. Carpet replacement – See attached drawing

2.13.1.1. Remove existing carpet, vinyl cove base and transition strips. See attached drawing.

2.13.1.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.13.1.3. Install new Interface, Cubic, Dimension, carpet.

2.13.1.4. Install new Burke Flooring, #660, Rocky, cove base in all rooms and transition strips at all rooms excluding Vault area.

2.13.1.5. Lockers in room 170 and work benches in room 174

shall be moved as needed to install new carpet.

2.13.1.6. Work in Vault area shall be done on weekends only.

2.13.2. Tile replacement – See attached drawing

2.13.2.1. Remove carpet in Duty Desk area and demo existing tile in all other areas.

2.13.2.2. Remove existing vinyl cove base at Duty Desk area and tile cove base in all other areas.

2.13.2.3. Clean, repair, patch floor as needed to provide smooth / level surface for the new tile.

2.13.2.4. Install new 18 x 18 daltile, Heathland, White Rock tile in all areas with Custom Building Products #370 Dove Gray grout and Ashland accent tile.

2.13.2.5. Install new Burke Flooring, #660, Rocky, vinyl cove base at Duty Desk counter and display cabinet. Install White Rock tile cove base everywhere else. No cove base to be installed on blue wall at Duty Desk area.

2.13.2.6. Tile lay out shall be in a diamond pattern. Contractor shall verify tile layout prior to start of work.

2.13.2.7. Tile shall end at center of closed doors at all doors without a threshold.

2.13.2.8. Repair any wall damage at the cove base which occurs due to the replacement.

2.13.3. Building is located in the Airfield Controlled area.

2.14. Building 1138

2.14.1. Remove existing carpet and vinyl cove base. See attached drawing.

2.14.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.14.3. Install new Interface, Cubic, Construction, carpet.

2.14.4. Install new Burke Flooring, #663, Sky Gray, cove base in all areas.

Metal transition strip at doors and open office area border shall remain.

2.14.5. Open office area has system furniture.

2.15. Building 647

2.15.1. Remove existing carpet and vinyl cove base. See attached drawing.

2.15.2. Clean, repair, patch floor as needed to provide smooth / level surface for the new carpet.

2.15.3. Install new Interface, Cubic, Construction, carpet.

2.15.4. Install new Burke Flooring, #662, Rocky, cove base in all areas.

2.15.5. Replace transition strips to match new cove base at both double door entrances, Comm. room in Command area, janitor closet, 2 at break room and Comm. Room south of break room.

2.15.6. Large open areas have system furniture.

2.16. Building 6011

2.16.1. Remove 4”x4” ceramic tile area at entrance door.

2.16.2. Remove carpet in large and small rooms.

2.16.3. Remove VCT in restroom.

2.16.4. Remove vinyl cove base in all areas.

2.16.5. Clean, repair, patch floor as needed to provide smooth / level surface for the new LVT.

2.16.6. Install new Armstrong, Raw Crete, Paradiso, 18 x 18, LVT in all areas.

2.16.7. Install new Burke Flooring, #597, Mocha, cove base in all areas.

2.16.8. Remove toilet and partition and reinstall on new flooring and make any adjustments needed for change in floor height.

2.17. Carpet and tile shall be put under removable thresholds.

2.18. Buildings will remain occupied during work.

2.19. Move furniture as needed to accomplish the work.

2.20.Contractor shall coordinate phasing of work with Facility Manager.

2.21. Provide Facility Manager with extra carpet tiles for replacement later.

Provide at least 4 tiles for the smaller buildings and at least 9 for the larger ones.

3.0. HAZARDOUS MATERIAL INVENTORY AND TRACKING: The contractor shall submit TAFB Form 81 (Contractor’s Hazardous Material Questionnaire – Attachment 3) and TAFB Form 82 (Short-Term Contractor Hazardous Material Inventory – Attachment 4), prior to starting work. The Contractor should note that Tyndall AFB is required to report chemicals used such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, oils, cleaners, degreasers, pesticides. Copies of manufacturer-specific Material Safety Data Sheets (MSDS) must be attached to TAFB Form 82. After submission, 325 CES/CEI will notify the Project Manager and CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit TAFB Form 83 (Contractor Environmental Reporting Entry Form – Attachment 6) at the completion of the project. The CO must be notified of any changes from the original submittal (i.e.

new chemical is added, size of container or unit of issue changes or if the manufacturer changes). These MSDSs shall also be readily accessible at the location of each hazardous material. MSDS shall be provided to CE Environmental Engineering and to the Quality Assurance Personnel, and approved by the government, prior to commencement of work.

4.0. HOURS: The normal duty hours of work on Tyndall AFB are from 6:30 AM to

5:00 PM, Monday thru Friday excluding Federal Holidays. All work shall be performed during normal duty hours. All other hours shall be requested through the Contracting Officer for approval. This request shall be submitted no less than three

(3) workdays prior to the time requested.

5.0. MATERIALS: All material shall be high quality from a well-known Manufacturer and used per the manufacturer’s recommendations.

6.0. DEBRIS AND MATERIALS: The contractor shall dispose of all debris and waste materials generated by any work under this contract on a daily basis off-base at no additional cost to the government.

7.0. BASE ACCESS: The contractor shall notify Civil Engineering Quality Assurance

(QA) Personnel at 850-283-1380 at least two week prior to start of work to obtain Base Access badges.

8.0. COORDINATION: The contractor shall notify Civil Engineering Quality

Assurance (QA) Personnel at 850-283-1380 one week prior to start of work. The QAP shall inspect all work.

9.0. DRAWINGS: Drawings are provided for reference and are for “Official Use

Only”. All dimensions, measurements and quantities etc. are approximate and shall be verified by the Contractor.

10.0.SAFETY: The contractor shall comply with all Federal, State and Local

Regulations.
