STATEMENT OF WORK
FOR
Constructing Drop Arms, Sumter Gate
PROJECT: VLSB 19-0056 11 April 2019
The contractor shall provide all management, tools, supplies, equipment, labor, and applicable licenses and permits necessary to ensure that any demolition, utility cut and capping, and debris removal service is performed at Shaw AFB, South Carolina in a manner that will restore a neat and professional appearance of base areas. All work shall be done in strict accordance with this Statement of Work and subject to the terms and conditions of the contract and in accordance with (UFC) Unified Facilities Criteria, Unified Facilities Guide Specifications (UFGS), the International Building Code (IBC), International plumbing code (IPC), South Carolina Department of Health and Environmental Control (SCDHEC), National Electrical Code (NEC), National Electrical Safety Code (NESC), National Fire Protection Association (NFPA), EPA 402-K-01-001 (2008), South Carolina Department of Transportation (SCDOT), South Carolina Department of Health and Environmental Control (SCDHEC) Shaw Air Force Base Design and Construction Standards, Change 2, 21 Aug 2018 and all other applicable codes.
PERMITS
The contractor shall pay for and obtain any required permits. The contractor shall act in accordance with this Statement of Work, SCDHEC, Federal, and Shaw AFB guidelines.
WORKING CONDITIONS
The Contractor shall take all precautions to insure that his operations are conducted in a manner as to minimally interfere with the normal operations of the base and the safety and convenience of the base personnel. The contractor shall coordinate utility outages with the Civil Engineering Squadron (CE) and the user. The contractor shall provide written notice a minimum of 10 days prior to outages to CE and the user. Normal working hours shall be 0730-1630 Monday through Friday excluding Federal Holidays.
GENERAL PROJECT NOTES
Contractor is responsible for verifying all dimensions, quantities and square footages.
All dimensions, quantities and square footages included are approximations.
Construction and demolition shall be in strict accordance with the Shaw Air Force Base Design and Construction Standards. The information provided in this SOW, the provided

drawings, and the requirements of the Shaw Air Force Base Design and Construction Standards shall be determined to be the minimum standards required in this project.
UFC’s take precedence over all standards, codes and Shaw AFB Design Standards and requirements that pertain to the work within this SOW.
During construction, the contractor shall take care not to damage existing structures, etc. that will not be replaced as part of this contract. Contractor shall be responsible for the temporary support of all devices as required to perform work. Any demolition work that needs to be accomplished to complete the contract is the responsibility of the contractor. All material shall be installed per manufacturers’ recommendations. All items listed in the SOW are to be supplied and installed by the contractor unless otherwise noted.
A pre-construction meeting will be held at Shaw AFB to include the Contractor and Government representatives. The Contractor shall set up this meeting, to be held on Shaw AFB, with the Contractor and Government representatives, to discuss all remodel requirements, as well as any other construction issues the Contractor may have. All finishes shall be presented to the project inspector as one submittal for selection and approval prior to installation. All material that could be discontinued shall be on site, i.e. carpet and tile prior to starting project so there will be no difference in color.
Where specific part numbers or manufactures are referenced, it shall be understood that the contractor may use an approved equal.
ASBESTOS/LEAD BASED PAINT
Selective Building Material System Demolition: Prior to any demolition, the Contractor shall perform a destructive asbestos and lead survey in accordance with SCDHEC regulations and requirements and prepare a bound report as well as an electronic version form submitted to the Government. The electronic version shall be in Adobe pdf format. The report shall be submitted to the Government for approval prior to requesting abatement permit. The survey shall be used as estimated quantities for abatement.
WORK TO BE COMPLETED:

• One drop arm for lane #2 (right lane as approaching) at the Sumter gate.
• The drop arm shall be K-4 rated (or a drop arm with equivalent specification) that is manually operated, counterbalance, and lockable. The road opening is approximately 15 feet.

• The drop arms shall be installed in a manner that does not impede traffic when upright. All parts of the drop arm shall be behind the curb when upright.
• All areas disturbed shall be dressed up and seeded.
GENERAL DESIGN AND CONSTRUCTION NOTES
1. Any references to State Government, Federal Government, or Industry standards or specifications made herein shall form a part of this specification to the extent referenced thereto and all materials and workmanship under this contract shall comply with or exceed these standards and references. All referenced standards shall be the latest edition.
2. All manufactured equipment and products shall be new materials in good condition and installed as per manufacturer's latest printed instructions, unless specified otherwise herein.
3. The Contractor is responsible for protection of existing facilities equipment from damage during installation and testing operations.
4. The contractor is responsible for submitting shop drawings for approval. The drawings must be approved by the government prior to the start of construction.
5. The Contractor shall show all calculations on the drawings in lieu of a separate document, except where specifically allowed by the Shaw AFB Design Engineer.
6. Contractor shall take into account the special treatment of hazardous waste, as well as the environmental implications related to building demolition management (i.e. fluorescent lighting tubes, t-stats, etc.). Every effort shall be made to recycle eligible materials, and those not suitable, shall be disposed of off base in accordance with all SCDHEC regulations.
7. Any refrigerant amounts removed or installed shall be documented and the information shall be provided to Shaw AFB.
8. The contractor shall include make, model number and inherent operational characteristics on the drawings for all of major equipment supplied for the facility. The Contractor shall supply manufacture’s cut sheets with the submittal where the component is identified.
9. All Operations and Maintenance (O&M) manuals shall be provided to the government upon completion.

10. If any submittal doesn’t, in the opinion of the Contracting Officer, meet or exceed the requirements contained within this SOW, in the Shaw AFB Design Standards or in any referenced design guidance or nationally recognized code then it shall be considered to be unacceptable and returned to the Contract for correction and resubmission. The Contracting Officer has the option, in lieu of disapproval, to provide acceptance of a submittal with required corrections identified if it is determined the submittal is substantially complete and correct.
11. The facility/ area shall be cleaned in its entirety, including exterior areas, before being returned the government.
12. All Contractors shall be responsible for submitting a completed DD1354 (AUG 2013 edition, PDF file) with each progress payment request (beginning at 50% construction completion). This form shall be marked "Draft" until the Contractor has reached approximately 90% completion, or when it has been determined that funding will not be adjusted due to modifications. The Contractor shall have a reviewed and accepted DD1354 submitted to the Government Inspector two (2) weeks prior to the scheduled final inspection date (i.e. Placed In Service Date). This reviewed and accepted DD1354 shall be marked "Interim" until the final inspection occurs.
WARRANTY
Contractor shall furnish a minimum one year warranty against defects in material and workmanship.
DEBRIS HAULING AND DISPOSAL
Contractor shall use an approved hauling route to prevent interference with base operations. In order to prevent problems with traffic, noise, wear and tear on roads, or other problems having the potential to adversely affect health, safety, or the environment, the government reserves the right to regulate routes, intervals, delivery points, and times for hauling.
All debris hauled by the contractor shall be enclosed to prevent leaking, spilling, or blowing. The Contractor shall ensure all debris or dirt is promptly removed from any transportation route if such debris is spilled, blown, or leaked from hauling equipment and vehicles. The contractor shall remove all dirt and mud caused by tracks of hauling equipment within one hour of notification by the Government. All demolition debris collected by the contractor shall be housed dumpsters and covered at all times and disposed of in an authorized landfill or transfer station. No disposal or burning is allowed on base. The contractor shall provide the government inspector with an

electronic copy of each dump receipt on the following work day by 0800 with the daily report. (Electronically)
DAILY REPORTS
The contractor shall provide the government with an electronic copy of each dump receipt along with the daily report by 0800 the following work day. All daily reports will be submitted in RAKEN format and shall include pictures of work performed for each day.
BID OPTION #1
The Government will at a minimum award the base bid. The Government is requesting the contractor provide pricing to install a second drop arm at the reject lane at the Sumter gate, this pricing will be known as bid option #1. If the base bid and bid option #1 are within the Governments funding limits, the base bid and bid option #1 will be awarded together.
