STATEMENT OF WORK (SOW)

RUBB REPAIRS

SHAW AFB, SUMTER

19 April 2017

SECTION 1 - GENERAL INFORMATION

1.1. SCOPE OF WORK. Building 1942 (RUBB 1, #93079) 85’ wide x 200’ long Replace (2) 3068 Insulated steel personnel doors, with existing opening anchorage, stainless steel hinges, cylindrical locks with lever handles, heavy duty closers, bottom sweeps, thresholds, and weather strip. (See attached documents) Perform complete building inspection and maintenance repairs as outlined in the attachment from RUBB, Inc.

The maintenance repairs will include fabric patching at up to (20) locations on each structure.

Base seal materials will include up to 50 linear feet of aluminum battens and screws and up to 32 linear feet of base sealing channel with concrete anchors. Up to 500 linear feet of lacing will be replaced or repaired during the inspection. Work beyond this scope will be performed on an hourly rate basis, plus materials. An example of this type of work would be missing structural components.

1.2. POINTS OF CONTACT. The contractor will provide a minimum of two points of contact within the company to interface with the Contracting Officer and his/her designated representative, hereafter referred to as the Contracting Officer’s Representative (COR), on issues concerning the repairs of the RUBB building. Names, business address, phone numbers, and hours of operation shall be provided before performance begins.

13. BASE ENTRY. For base entry all vehicles shall be registered, licensed and insured for the duration of the contract as required by South Carolina law.

1.4. RECORDS. The contractor shall maintain records of transactions with the Government and, upon request, make such records available to properly designated contract representatives within 5 working days.

SECTION 2 – QUALITY CONTROL

2.1. QUALITY ASSURANCE. The Government intends to evaluate the contractor’s performance under this contract using direct surveillance observations. Government personnel will record all surveillance observations. When an observation indicates defective performance, the COR will notify the contract manager or on-site representative.

2.2. PERFORMANCE ASSESSMENT REPORT (PAR). The contractor’s performance will be evaluated by the COR on a on an as-needed basis, utilizing the Performance Assessment Report (PAR). Copies of this document are available upon request.
