STATEMENT OF WORK
FOR
CARPET INSTALLATION
WO76404 7 Aug 2017
1. Scope: Contractor shall furnish all labor, materials, permits, and equipment necessary to accomplish the installation of 1779 sq. yds. of carpet and 720 linear feet of cove base in building 1029. The Contractor is responsible for field measuring of carpet to insure exact amount of carpet and cove base required to complete this installation. The measurements given are an estimate. The extra carpet tile shall be given to the Facility Manager after the project is complete.
The Contractor shall have the carpet installed as soon as possible, after the abatement is finished in each area, so as to keep the down time for the facility to a minimum.
2. Contractor requirements for carpet replacement: Carpet to be purchased and installed shall be solution dyed, 24” X 24” tile, tufted weight 20 oz. density 7347 per cu yard, Collection:
Virtual Spaces, Style: Augment tile, Color: Energize 64585, multi-lever pattern loop, 100% solution dyed, primary backing synthetic, inherent stain resistance treatment, lifetime commercial limited warranty, as manufactured by Shaw Contract group, or approved equal. The tiles shall be turned ¼ turn.
a. Contractor is responsible for purchasing and installing transitions to be replaced in the all doorways where carpet tile is installed. The transitions shall be black rubber as manufactured by Roppe or approved equal. The contractor shall purchase and install cove base in rooms that have existing cove base. The cove base shall be black rubber as manufactured by Roppe or approved equal.
b. The existing floor surface shall be suitable for installation after the asbestos abatement but if the floor needs filling of holes or cracks the contractor shall be responsible for making the floor carpet ready. Contractor shall furnish all cement, tape, thresholds and cap down moldings.
c. Carpet to be installed in accordance with the manufacturer’s instructions. The contractor shall furnish all adhesive as recommended by carpet manufacture. Contractor shall be responsible for preparation of floor surfaces as necessary to receive carpet. All surfaces to be free of voids and protrusions. Installed carpet shall be smooth, uniform, secure, and installed in a professional manner. Patterned goods shall be accurately matched. Fit cutouts such as doors, jambs, columns, steps, landings, and ducts neatly and securely.
d. Extra attention is to be observed while butting seams together. Contractor shall follow manufactures instructions on in matching the seams to prevent fraying and raveling and for unity in pattern. Contractor is to provide at least a five year warranty against fraying, raveling, separating, buckling and on his workmanship.
e. The Contractor shall install carpet as soon as the abatement is complete in each room/area.
3. Schedule Tasks:
a. The Contractor shall perform the services specified herein Monday through Friday with the exception of observed, national holidays. However the contactor may work on second shift and weekend with approval of Facility Manager. The Contractor shall not be required to provide service on observed, national holidays, nor shall the Contractor be paid for these holidays. If a holiday falls on Sunday, the following Monday shall be observed as the national holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a national holiday by U.S. Government agencies. Also included would be any day specifically declared as a national holiday by the President of the United States.
b. The Contractor shall start the carpet installation after the abatement and shall take no longer than 3 weeks from start to finish.
c. The Contractor shall notify the Facility Manager (Bldg. 1029 MSgt Wood @ 895- 6034 and SMSgt Patterson @ 895-6042) and POC (Charles Whittington 803 895-9584) at least one week before start of work and upon completion.
4. GOVERNMENT FURNISHED PROPERTY AND SERVICES. Utilities (water and electricity
5. Hazardous Material Identification. Safety Data Sheets (SDS) are required as specified in the latest version of Federal Standard No.29 CFR 1910.1200 (g) (8) (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. SDSs must be submitted by the Contractor at the pre-performance conference. Failure to provide SDSs or certificates when requested could result in unsatisfactory performance ratings and possible termination of the contract.
6. Spill Response. The Contractor shall be briefed on Shaw AFB spill response procedures at the pre-performance conference. The Contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations, in the event that it is necessary to utilize government material, equipment or personnel to clean up a Contractor caused spill, the Contractor shall be required to reimburse the government for all associated costs.
7. SECURITY REQUIREMENTS: This is the template for contracts without access to classified information. Replace with the other version if contractor will have access to classified information. Language is contained in embedded word document.
a. Listing Of Employees: The Contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager/COR and sponsoring agency's Security Manager. An updated listing shall be provided when an employee's status or information changes.
b. Security Training: The Contractor ensures all Contractor employees receive initial and recurring security education training from the sponsoring agency's security manager.
The security manager shall provide employees with training required by DODM 5200.01, DoD Information Security Program, Volume 3, and AFI 16-1404, "Air Force Information Security Program". The Contractor will also provide initial and follow-on training to contractor personnel who work in Air Force controlled/restricted areas. Air Force restricted areas and controlled areas are explained in AFI 31-101, Integrated Defense.
c. Pass And Identification Items: The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees. All contractor employees shall undergo a background check conducted by 20th Security Forces Squadron prior to issuance of base access credentials. Only identification cards or driver's licenses from REAL ID compliant states or territories will be accepted. Contact 20th Security Forces Squadron for alternative forms of acceptable identification to conduct background checks.
d. Background Checks: All Contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Each Contractor personnel must submit their Social Security Number, Date of Birth, and Driver's License state and number (if applicable).
Failure to submit required information can result in denial or delay of issuance of credentials. Background checks are only good for the company that requested them.
Processing background checks takes, at a minimum, 7-10 days from time of submission.
e. Retrieving Identification Media: The Contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc.
f. Traffic Laws: The Contractor and its employees shall comply with base traffic regulations.
g. Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw AFB SC.
h. For Official Use Only (FOUO): The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
i. Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
j. Physical Security: The Contractor shall be responsible for safeguarding all Government property and controlled forms provided for Contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
k. Photography Restrictions: No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, photographs taken from personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities.
l. Any Contractor requiring pictures or videotape of Shaw's airfield to support their contract must request and possess written authorization prior entering the airfield area. Written authorization letters to photograph or video of the flightline must be signed by one of the following: 20 Maintenance Group Commander or Deputy; Aircraft Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; 20 Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations; or 20 FW/CVN.
