A. ACCREDITATION/LICENSE

B. PHYSICAL FACILITY

1. Is the facility overall clean and sanitary (i.e., restrooms supplied with toilet paper, hand soap, running water, paper towels or dryer, working toilets, clean floors)? ref: OSHA 1910.1030

E. PRE-WASH PROCESS

1. Is equipment arranged to prevent cross-traffic of clean and soiled linen?

2. Are there separate areas for the processing of soiled and clean linen?

3. Does the facility conduct maintenance on equipment as recommended?

F. WASH PROCESS

3. Are designated clean carts used in this area?

G. POST WASH PROCESS

1. Are clean items transported in containers exclusively for clean linen, clean hampers lined with plastic or reusable liners, placed in a clean cart with a secure plastic or reusable cover? Ref: AFI 44-108, para 3.11.3.1.

2. Are clean and soiled textiles transported separately and/or are there physical barriers separating clean from soiled linen? Ref: AFI 44-108, para 3.11.3.1.

3. Clean linen will be stored in clean storage areas (dedicated storage rooms with closing door or covered carts or closed drawers or cabinets) Ref: AFI 44-108, para 3.11.3.1.

4. Clean linen remains protected until point of use. Ref: AFI 44-108, para

3.11.3.2.

5. Is the lint vent cleaned with each load?

6. Is the area for sorting and folding clean and free of food and beverage items?

7. Are presses dusted each day?

8. Are sprayers washed, dried and refilled when emptied?

H. WRAPPING AND TRANSPORTING

1. Does a moisture repellant wrap cover the entire bundle per contact requirements?

2. Are items maintained in a clean environment at all times?

3. Are the clean linen carts cleaned on a regular basis?

4. Are the transport vehicles cleaned on a regular basis?
