REPAIR CDC PRETODDLER PLAYGROUND FALL PROTECTION
31 MARCH 2015
1.1 SCOPE: The contractor shall provide labor, materials, supervision, transportation and equipment to able to remove a DuraPlay fall protection in two pre-toddler playgrounds and install a new Duraplay fall protection or Equal Item.
1.2 LOCATION: Two Existing Play grounds on the North side of building 337, McConnell Air Force Base, Kansas 67221.
1.3 PRINCIPAL FEATURES:
1.3.1 The work to be accomplished is the removal and disposal of existing fall protection while protecting playground equipment in place.
1.3.2 The work is to be phased to allow for one playground being complete and usable at all times.
1.3.3 Adjacent playgrounds are to be protected from harmful chemicals and construction debris.
1.3.4 All haul routes and working times shall be coordinated with CDC staff.
1.3.5 Installation of new DuraPlay fall protection or Equal Item to meet 26 inch fall height criteria.
1.3.6 The surface is concrete. There is no curbing and the edges of the fall protection will have to be tapered down to meet the concrete.
1.3.7 Color shall be 70% grey 15% green and 15% tan.
1.3.8 Contractor shall provide cleaning specs in regards to what chemicals may or may not be used to clean the surface.
1.3.9 Contractor is to provide a patch kit with demonstration on how to repair damaged surface.
1.3.10 Contractor is to provide a 5 year warranty.
1.3.11 The playgrounds are approximately 800 square feet each and are approximately 2 inches deep. Contractor is responsible to field verify SF and depth.
Specifics shall be discussed at the site visit.
1.4 WASTE DISPOSAL: All waste, rubble and trash shall be disposed of off base.
Contractor's trucks shall be equipped with covers to prevent spills of waste materials.
1.5 PERMITS: The Contractor shall obtain a McConnell Form 199 from the Civil Engineer Squadron during normal duty hours, 0700 to 1600 hours.
1.6 WARRANTY: The Contractor shall pass on all commercial warranties to the government. A copy of each warranty shall be furnished to the Contracting Officer not later than the date of the final inspection. The warranty shall:
A. Provide a written warranty stating that work executed under this Section will be free from defects of materials and workmanship for a period of two years from date of Substantial Completion, and that material breakdown and unraveling will be remedied on written notice at no additional cost to the Owner.
B. The Warranty shall be supplied in writing, and honored by Contractor. Contractor shall Warranty removal and replacement of materials as required repairing or replacing PIP surfacing.
1.7 SUBMITTALS: Within 21 days after issuance of the award and before any material or equipment is purchased, the contractor shall submit to the Contracting Officer for approval, three copies of the manufacturer's data, catalog cuts, samples, or other information as required.
1.8 RECORD DRAWINGS: The contractor shall maintain a separate set of drawings at the work site marked to fully indicate As-Built conditions. These drawings shall be available for review by government personnel. Record drawings will be submitted according to Section 01000 General Conditions. The contractor shall provide As- Built drawings in digital format and shall not be revised without written approval from the Contracting Officer. These drawings shall be furnished to the Contracting Officer at the final inspection.
1.9 UTILITIES: A reasonable amount of utilities will be made available to the contractor by the government from government owned and operated utility systems without cost to the contractor. All temporary lines will be furnished, installed, connected and maintained by the contractor in a manner satisfactory to the Contracting Officer and shall be removed by the contractor at his expense prior to final acceptance of the construction.
1.10 ACCESS TO WORK AREA:
A. The contractor shall submit in advance C o n t r a c t o r / V e n d o r Ac c e s s Request/Authorization form to the Contracting Officer for all personnel requiring access to the work area during construction. A form must be completed in full for each person. The form and badging is to be completed for each delivery order. A badge must be worn on the outside clothing and above the waist of the person while they are on the base.
B. Contractor vehicles are subject to search when entering and leaving the base.
Also, periodic Air Force exercises may require the contractor to vacate the work area.
C. No work, deliveries or any construction related activities are permitted prior to 7:00 A.M. (0700).
1.11 OZONE DEPLETING SUBSTANCES/CHEMICALS POLICY: As per SAF/AQC Ozone Depleting Substance/Chemicals Policy dated 11 Jun 93 and AFFARS 5310.002-71(90) (c) the Air Force must not require the Contractor to deliver or use Class I Ozone Depleting Chemicals (ODCs) in performance of the contract. The Contractor will furnish a written statement certifying that this project does not require or install any equipment or fitting with the use of ODCs. Contractor will submit the letter with their proposal.
1.12 DRAWING NUMBER FOR RECORD DRAWINGS: 40-12
1.13 USE OF RECYCLED MATERIALS: The contractor shall reclaim or recycle a minimum of 60% of the construction debris. Written documentation shall be provided detailing the quantity (volume or weight) of material that was removed from the project site and delivered to a recycle center, reclaimed for use off site, re-used onsite, delivered to a landfill, transfer station or hazardous dump site. In the event 60% of the debris is unable to be recycled, reclaimed or reused the contractor shall provide written documentation detailing why the goal was not attained.
1.14 RECORDS MANAGEMENT: In the event the Contractor, creates, handles and maintains records for the Air Force, either electronic or paper, the Contractor shall, must, and will meet the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Management Program, AFI 33-364, Records Disposition Procedures and Responsibilities, and AFM 33-363, Management of Records. In order to meet the requirement established in accordance with (IAW) the above mentioned regulations please contact the Base Records Management Office (22d CS/SCOK, Bldg 522) at (316)759- 4881 for guidance and training. The
Contractor, at or before the final project acceptance, shall return any/all borrowed records, and deliver complete project records, any needed or created software, and technical documentation and “Record Drawings” sufficient to allow Government access to all project information throughout the life-cycle of the project and related records.
1.15 Security Requirements:
The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations included herein. All contractor employees shall carry contractor photo identification at all times.
Contractor employees shall comply with any base security measures implemented at all times. This includes, but is not limited to, vehicle and goods searches and identification checks during times of increased security.
1.15.1 Employee Base Pass and Identification, Special Clearances and Vehicle Passes.
The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference. Strict security and personnel access control is enforced at MAFB. All employees desiring access must be identity proofed and vetted . All Contractors and sub-contractors must pass a Criminal History Report Inquiry [CHRI] through National Crime Information Center [NCIC] , National Law Enforcement Telecommunication System [NLETS], state and local law enforcement databases. The primary contractor will ensure all employees possess proper credentials allowing them to work in the United States.
IAW E.O 12989, the use of E- Verify is mandatory. Identification required for identity proofing includes but is not limited to:
a. United States Passport
b. Permanent Registration Card/Alien Registration Receipt Card [Fm I-551]
c. Foreign Passport with a temporary [I-551] stamp or temporary [Fm I-551] printed notation on machine readable immigrant visa
d. Employment authorization document that contains a photograph [Fm I-766]
e. Current /Valid Driver’s License
f. Identification card issued by Federal, State or Local Government
g. Social Security Card
h. I-9
Employees will be identity proofed and vetted each time a pass/credential is issued.
Security Forces may conduct random screenings at any time; if disqualifying information as prescribed in DoD, Air Force, or local directives is found, employees may denied access. The COR will be notified of the denial. Due to mandates of 5 USC 552a(b)(3) as related to the Privacy Act of 1974 as amended, the information can only be discussed with the employee and will not be expressed outside of law enforcement. Employees requesting to contest denial of installation access, or requesting a waiver/exception to policy must submit a written rebuttal /request to Commander, 22nd SFS/CC, through 22nd SFS/S3, Attention: 22ndSFS/S3PC, McConnell AFB KS 67221.
Foreign passports with a current arrival-departure record (Form I-94) bearing the same name as the passport and containing an endorsement of the alien’s nonimmigrant status, if that status authorizes the alien to work for the employer. In the case of a nonimmigrant alien authorized to work for a specific employer incident to status, a foreign passport with Form I-94 or Form I-94A bearing the same name as the passport and containing an endorsement of the alien’s nonimmigrant status, as long as the endorsement has not yet expired and the proposed employment is not in conflict with any restrictions or limitations identified on the form.
The contractor shall not be entitled to any additional compensation for delays or expenses associated with complying with these requirements. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required.
The application for a pass/credential if driving must be accompanied by a valid driver’s license, state vehicle registration and proof of insurance. Further, the vehicles and equipment may be escorted during exercise operations and may be rendered immobile when not in operation by the removal of ignition keys or by other readily reversible means. Contractor vehicles are subject to search prior to entering and exiting the base and while on the base.
1.15.3. Base Access Credentials. The Contractor shall submit a request to the Contracting Officer/Contract Administrator (CO/CA) for all mandatory information on each employee as indicated in sub-paragraph 3a. et seq. below to 22 SFS/S-3PV via encrypted e-mail on each employee or sub-contract employee requiring access to the base. The information submitted will be reviewed and signed by personnel authorized to submit and sign as sponsors. Contractors who are not U.S. citizens must also submit an INS Form I-9 accompanied with all original supporting documents. A formatted Contractor/Vendor Access Request/Authorization template will be provided upon request for ease of information submission. We highly recommend you utilize this formatted request template. For the protection of the personal information, Pass and Registration officials provide a secure website for contractors to use, (https://safe.amrdec.army.mil/safe/Welcome.aspx). This website will allow you to create an encrypted email that can be forwarded to the Government sponsor. The following information must be submitted with each request for access (sub-paragraphs a-v)
a. Complete name as it appears on identification (last, first, middle initial)
b. Complete address; (Street, Apt number, City, state, Zip)
c. Social Security Number
d. Date of Birth
e. Driver’s license or identification number & issuing authority
f. Company or vendor name

t. Is the individual going to be designated to sponsor passes?

[FMC, inside the fence line] ?
v. Does the individual require Mission essential base access? [Must be listed in the 22 ARW Mission Assurance Plan]
The Government sponsor will verify the information and forward the required employee information via encrypted email to the Issuance Office, 22sfs.s5pv@us.af.mil.
Incomplete information will result in requests not being processed and will be returned to the CA. Processing the requests will take from three to five business days. Upon completion of processing, the Government sponsor will be notified by a Pass and Registration issuing official via confirmation email with instructions for the employee(s) to proceed to the Contracting Identification Office. Approved individuals will report to the Contractor I.D. Office (Bldg 61) at the CVI. Hours of operation for badge issuing are 0700-1545, M-F. This must be accomplished within 45 days of approval, and a government-issued photo ID is required. The request must be resubmitted for approval if the credential is not issued within 45 days. The applicant will be issued a DBIDS Access Credential (DAC) for the duration of the contract period, not to exceed one year. DACs will be renewed annually after a current criminal history check has been conducted. In the event of loss or the credential is stolen the employee must report this to the Contract Administrator. The Contract Administrator will notify the Contractor I.D. office, (316) 759-1781 during duty hours, or the 22nd Security Forces Base Defense Operations Center, (316) 759- 3976/3978 during non-duty hours. A new request and a Lost Badge letter must be submitted for a replacement credential by the Contract Administrator.
Credentials and/or passes will not be issued to personnel who are waiting for the processing. Also, a pass does not replace the requirement for a contractor I.D. for temporary employees working longer than five days, even though they are under the escort of a person holding a contractor credential with sponsorship privileges. All personnel issued DACs must display their credential(s) on the front of their mailto:22sfs.s5pv@us.af.mil outermost garment, on the upper torso, while working on MAFB. The ID allows movement for the issued employee to and from work sites only. Use by any persons other than the issued employee is strictly prohibited is punishable under multiple federal, state and local laws.
The ID does not authorize entry/access to restricted areas. Contractors/vendors who need access into a restricted area must be escorted IAW AFI 31-101, Integrated Defense.
Credentials and associated access cards are property of the U.S. Government. The contractor and the CA are jointly responsible for ensuring that all credentials and other forms issued by MAFB are returned after completion of the contract or should an employee no longer work for the contactor/subcontractor. Failure to return all credentials may result in final payment being withheld. Items may be returned in person at either office or by mail to 22 SFS/S3PV, 53403 Kansas St, McConnell, AFB, KS 67221-3716 ATTN: Pass & Registration.
