ADDENDA TO 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (JULY 2013)
To assure timely and equitable evaluation of the proposal(s), the offeror must follow the instructions contained herein. The proposal(s) must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of five (5) separate parts: Part 1 – Price Proposal (Pricing Schedule), Part 2 –
Technical Capability, Part 3 – Signed Amendments (if any), Part 4 – Contingency Plan; and Part 5 – Representations and Certifications.

Part 5 - Return of the FAR 52.212-3 “Offeror Representations and Certifications – Commercial Items”.
Note that current registration in SAM meets this requirement.
Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price reasonableness exists, no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.
PART 1 – PRICE PROPOSAL – PRICING SCHEDULE
Instructions for completing the pricing schedule:
A. Complete all required portions of the solicitation. In doing so, the offeror accedes to the terms and conditions as written in the solicitation, with attachments.
B. Insert proposed unit prices, extended prices and totals as applicable in each contract line item on the SF1449
PART 2 – TECHNICAL PROPOSAL
Offerors shall electronically submit hard copy documentation outlining the details of their proposed maintenance plan, and confirming that this plan meets the requirements of the PWS.
PART 3 - SIGNED AMENDMENTS
This solicitation may be formally amended to incorporate changes to items such as the requirement or proposal due date. Offerors shall sign and return all published amendments with their proposal.
PART 4 – CONTINGENCY PLAN
As outlined in DFARS 252.237-7023 & 252.237-7024 and to not be used as a basis for award, but reviewed for completeness and compliance with DFARS 252.237-7024. The contractor may be determined removed from consideration for award for failure to submit the contingency plan with its proposal.
PART 5 - REPRESENTATIONS and CERTIFICATIONS
Offerors shall complete FAR 52.212-3 (located in this solicitation) and return it with the company’s offer, unless the offeror is currently registered in good standing in SAM.
Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:
1. Requirements of the solicitation (Item Numbers) and Performance Work Statement (PWS), and Government standards and regulations pertaining to the PWS.
2. Evaluation Factors for Award
Proposal Submissions: Proposals are due 2:00pm, Pacific Daylight Time, on Monday, 17 August 2015.

52.212-2 AND ADDENDA
EVALUATION AND AWARD CRITERIA
EVALUATION PROCESS:
(a)The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotations:
TECHNICAL CAPABILITY
PRICE
PAST PERFORMANCE
Technical capability combined with past performance are approximately equal to price.
The technical evaluation will consist of a review of the maintenance and training plans proposed for this requirement, and the conformance of the proposal with the requirements set forth in the PWS. The past performance evaluation will consist of a review of government sources of past performance information, such as
PPIRS.
(b)Options: The Government will evaluate the quotation for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that the offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options(s). Additionally, the price for a 6-month extension of services will be evaluated, in accordance with FAR
52.217-8 for price reasonableness. The price for this extension will be defined as 6 months times the monthly unit price proposed for the final option period. Although evaluated, the price for this extension will not be included in the overall contract price.
(c)A written notice of award or acceptance of the offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.

(d)Offerors are cautioned to submit sufficient information and in the format specified in the proposal preparation instructions to permit a meaningful assessment of the proposal. Offerors may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.
(e)The Government intends to award a contract without discussions with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in its best interest.

52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days.

(a) Definitions. As used in this clause--
Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.
Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow
(WAWF).
Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

252.237-7024 NOTICE OF CONTINUATION OF ESSENTIAL CONTRACTOR SERVICES (OCT 2010)
(a) Definitions. Essential contractor service and mission-essential functions have the meanings given in the clause at
252.237-7023, Continuation of Essential Contractor Services, in this solicitation.
(b) The offeror shall provide with its offer a written plan describing how it will continue to perform the essential contractor services as described in the PWS during periods of crisis. The offeror shall, during such periods, continue to comply with the Tier 1 and Tier 2 maintenance requirements as outlined.
(1) Identify provisions made for the acquisition of essential personnel and resources, if necessary, for continuity of operations for up to 30 days or until normal operations can be resumed;
(2) Address in the plan, at a minimum, the plan they will utilize to continue performance and provide staffing for services described in the PWS as “Tier 1 and Tier 2.”
(i) Challenges associated with maintaining essential contractor services during an extended event, such as a pandemic that occurs in repeated waves;

5352.201-9101 OMBUDSMAN (APRIL 2014)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. (a) Unless the requiring activity has obtained prior Senior Acquisition Official (SAO) approval, contractors may not:
(1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or

5352.223-9001 HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (JUN 1997)
(a) In performing work under this contract on a Government installation, the contractor shall:
(1) Comply with the specific health and safety requirements established by this contract;
(2) Comply with the health and safety rules of the Government installation that concern related activities not directly addressed in this contract;
(3) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and
Government personnel performing or in any way coming in contact with the performance of this contract; and
(4) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health Standards (AFOSH) and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.
(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
5352.223-9002 REQUIREMENTS AFFECTING CONTRACTOR PERSONNEL PERFORMING MISSION
ESSENTIAL SERVICES (APR 2003)
(a) The Contracting Officer has identified all or a portion of the services performed under this contract as “Essential
DoD Contractor Services” as defined and described in DoD Instruction (DoDI) 3020.37, “Continuation of Essential
DoD Contractor Services During Crises.” Hereafter, the personnel identified by the contractor to perform these services shall be referred to as “Mission Essential Contractor Personnel.”
(b) Within 30days after contract award or incorporation of this clause into a contract by modification, the Contractor shall provide a written list of all “Mission Essential Contractor Personnel” to the Contracting Officer or designee.
The list shall identify names and country(ies) where each employee will perform work under this contract.
(c) As required to comply with or perform pursuant to DoD or Air Force requirements, the contracting officer shall direct the contractor to comply with requirements intended to safeguard the safety and health of Mission Essential
Contractor Personnel. The Contracting Officer may communicate the requirements through a letter of notification or other means, and subsequently modify the contract to incorporate the requirements via full text or by reference. The
Contractor may file a proposal for cost or other impacts under the Changes clause or a Request for Equitable
Adjustment.
(d) This clause shall be inserted in all subcontracts meeting the criteria in paragraph (a) of this clause.
5352.242-9000 - CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOVEMBER 2012)
(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) In accordance with Department of Defense (DoD) policy guidance on Physical Access Control to all military bases, all individuals entering DoD installations must have a valid purpose to enter, have their identity proofed (providing acceptable documentation of proof of identity) and validated through the process of vetting (evaluation of an applicant’s character and conduct for approval or denial of access) through law enforcement data bases to be issued or be in possession of a Fairchild AFB access credential.
DoD ID card holders and those designated on local access credentials may vouch for and escort daily deliveries to/from the job site. Those with locally produced access credentials may vouch for and escort visitors only during normal business/operating hours or as approved by the contracting officer for individual contract performance. Up to three (3) on-site contract superintendents may be designated, in writing by the contracting officer for this purpose.
(c) Contractor identification badges will be issued for the performance period of the contract only, or for
