Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

PAST PERFORMANCE EVALUATION RATINGS

Rating Description

Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. (See note below.)

Unacceptable Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”

2 The offeror demonstrated experience in executing ITW/AA scenarios through the Test Development Facility (TDF) and capture outputs when necessary.

6 The offeror demonstrated experience in their ability to generate ground based sensor response and support simulations for requested scenarios and requested sensor locations.

The 55th Contracting Squadron of the Air Force Installation Contracting Agency (AFICA) is in the process of selecting a contractor for the Integrated Tactical Warning/Attack Assessment (ITW/AA) Simulation Support contract at United States Strategic Command (USSTRATCOM), Offutt AFB NE and NORAD and United States Northern Command (USSNORTHCOM) at Peterson AFB, CO.

One of the considerations in proposal evaluation is the verification of your past performance on contracts that are relevant to the proposed effort. The 55th Contracting Squadron and (insert name of your corporation and division) are depending on information received from agencies such as yours, which have had first-hand experience with our efforts.

You have been identified as a point of contact for a past and/or present performance evaluation of the firm listed on the attached questionnaire. This firm is currently being considered for a Noncommercial Firm- Fixed Price (FFP) contract at Offutt AFB, NE.

SUBCONTRACTOR/TEAMING PARTNER CONSENT LETTER

FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION

TO THE PRIME CONTRACTOR

Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime contractor during exchanges. In an effort to assist the Government’s Performance Confidence Assessment Group (PCAG) in assessing your past performance relevancy and confidence, we request that the following consent letter be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent letters should be submitted as part of your Present/Past Performance Volume.

CLIENT AUTHORIZATION LETTER

Past performance information concerning private sector contractors, subcontractors and joint venture partners cannot be disclosed to the Government without their consent. Client authorization letters are required for each identified effort. This letter will authorize release to the Government of requested information on the offeror's performance.

The Government will need that consent before contacting customers to assess the offeror’s past performance. In an effort to assist the Government's Performance Confidence Assessment Group (PCAG) in assessing your past performance relevancy and confidence, we request that the following client authorization letter be completed by any customers identified in your proposal. The completed client authorization letters should be submitted as part of your Present/Past Performance Volume.

Offerors should send with their list of references a letter similar to the following authorizing the reference to provide past performance information to the Government.
