MEMORANDUM FOR XXX/CC (NOTE 2)
FROM: Visiting/Requesting Agency (NOTE 3)
SUBJECT: Entry Authority List (EAL)
1. (Name of the Organization/Team/Company/Individual(s)) is/are visiting Minot AFB from DD MMM YYYY through DD MMM YYYY (inclusive dates of the visit) and requires access to controlled, restricted, limited and exclusion areas, as noted below. The purpose of this visit (describe purpose – to perform Mx, to conduct inspection, etc.). This EAL consists of a cover letter and a list of XX names of visitors/inspectors in alphabetical sequence. Each individual is verified to be current for Standard Form (SF) 312, Classified Information Nondisclosure Agreement. All listed individuals have a valid need-to-know and are authorized access to classified information and materials to the level of their identified clearance. All clearances have been verified through the Joint Personnel Adjudication System (JPAS). (Notes 4, 5)
2. CAMERAS AND PICTURE TAKING: All individuals listed are authorized to carry cameras and to take pictures in all controlled, restricted areas, limited areas and exclusion areas for which they have entry access. All other visitors must have specific authorization, in writing, from the Wing Commander or designated representative. (Note 6)
3. CONTROLLED AREAS: All individuals are granted escorted/unescorted entry to all controlled areas based upon this EAL (see attachment) and either a valid AF Restricted Area Badge; DD Form 2/2 AF/Common Access Card; or a government issued photo ID.
4. RESTRICTED AREAS: All individuals are granted escorted/unescorted entry to (add areas I.e., WSA, APA, MPA, 5 BW/CP) restricted areas based upon this EAL (see attachment) and either a valid AF Restricted Area Badge, DD Form 2/2 AF/Common Access Card or a government issued photo ID.
5. LIMITED AREAS: All individuals are granted escorted/unescorted entry to (add areas I.e., WSA, APA, MPA - as applicable) limited areas based upon this EAL (see attachment) and either a valid AF Restricted Area Badge, DD Form 2/2 AF/Common Access Card or a government issued photo ID.
6. EXCLUSION AREAS: Visitors are granted escorted/unescorted entry into (add areas I.e., WSA, APA, MPA – as applicable) exclusion areas, as indicated by the EAL area identifiers (see attachment) and either a valid AF Restricted Area Badge; DD Form 2/2 AF/Common Access Card; or a government issued photo ID. Inspectors/visitors will never make up part of a two-person team.

Notes:
1. Use DD MMM YYYY format for all dates.
2. The Memorandum is for the Approval Authority (in most cases, this will be the Wing Commander).
3. The Visiting/Requesting Agency is the organization requesting entry.
4. Include ONLY the area(s) requested; remove area(s) not required.
5. If those requesting entry do not have a clearance, remove the last three lines of paragraph 1.
6. If photography is not requested, input “N/A”.
7. Pages must be numbered “1 of XX”, “2 of XX”, etc, to account for all pages of the respective EAL.
8. IG/NSSAV Teams will use the following: This EAL expires upon departure of the visiting individual(s) or team.
9. Signature should be the requesting CC, Vice/Deputy CC or requesting unit Team Chief.
10. 1st Ind is for the APPROVAL AUTHORITY. Administratively, if the 1st Ind is included on the first page, or on a page with at least one line of supporting text from the memorandum, use the following format:

11. The name listed will match that on the ID media being used for entry; e.g., Use "First, Last Middle Initial" if that is how the name is listed on the ID card. List names alphabetically; Last, First, Middle name or initial, as applicable.
12. A government issued photo ID with a controlled number (Driver's License Number, DoD Control Number, etc.) is acceptable. Only enter the last 6 digits of the DoD Control Number; all other media requires the full number. If a RAB is used, the controlled number will be the number listed in the SSAN block of the RAB.
13. Badge numbers are only required/used for IG/NSSAV team members; indicate N/A for all others, unless they have a home station RAB, in which case their RAB information should be placed on the EAL.
