Statement of Work – Ballistic Windows Minot Gate
Minot Air Force Base
North Dakota Ballistic Resistant Window Installation Entry Control Facilities
Statement of Work (SoW)
1.0. DESCRIPTION OF SERVICES:
The contractor/manufacturer shall install ballistic resistant windows and doors. Four (4) ballistic resistant windows (2) two doors with ballistic resistant windows and other ballistic rated material at the main guard shack (building 195) located at the Magic City Gate (main gate) and two (2) doors with ballistic resistant windows and other ballistic rated material at (building 1010) located at the Minot Gate (south gate) on Minot AFB. The windows and other ballistic material at a minimum will be UL 752 Level V rated. The contractor/manufacturer will install a system as described below.
2.0. WORK QUALITY:
After any installation, maintenance, adjustments or repairs are accomplished, the window will meet minimum standards outlined in Unified Facilities Criteria (UFC) 4-022-01, Security Engineering:
Entry Control Facilities/Access Control Points and manufacturer's recommendations. All work will be certified by representatives from the 5 CE and 5 SFS.
3.0. WINDOW REQUIREMNENTS:
The contractor will install at a minimum UL 752 Level V rated ballistic resistant windows
4.0 INSTALLATION/PREPARATION:
The contractor will conduct a sight survey of the facility to understand the work, materials and preparations needing to complete the removal of the old windows and old doors system and the requirements for the new ballistic windows and doors. If significant framing and support id needed for the install of the new windows the contractor will alert the government of possible cost over runs before the start of the project.
5.0. SECURITY REQUIREMENTS
5.1. The contract holder will complete a listing of all personnel needing regular access to the base in association with this contract within 15 calendar days following the start date of the contract. The following items required are as follows:

5.1.1. This is required of all personnel and must be kept current as changes occur. After review and approval, the Pass and ID office will contact the company informing them that their employee can come to the Pass and ID office to be issued an identification card.
5.1.2. Visitor Pass. Minot Air Force base is a closed base. All personnel entering must have a specific permission for entry. This permission is granted when the contractor and employees are issued an installation base pass.
5.1.3. Any employees with pervious felony arrests will be denied access to the base, any employees with multiple misdemeanor arrest may also be denied base access.
5.2. Contractor Access to Worksite. The contractor will enter the installation through the Minot Gate.
5.3. Reporting Requirements. Contractor personnel shall report to security forces any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor's employees shall be briefed by their immediate supervisor upon initial on-base assignment.
5.4. Physical Security. The contractor shall be responsible for safe guarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment and material shall be secured.
5.5. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable federal, state and requirements regarding environmental protection to include AFI 32-7080, pollution Prevention Program, AFI 32-7042, Solid and Hazardous 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event environmental laws/regulation/requirements change during the term of this contract, the contractor shall comply with such regulations as changed. The contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from the spill.
5.6. Security Requirements. All contractor employees shall be required to have in their possession their base pass at all times while on the installation. The contractor shall obtain passes by an application at the Base Contracting Office.
5.7. The contractor and his employees shall be subject to all base regulations.
5.8. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
5.9. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base.
5.10. Damage to Government Property. The Contractor shall notify the government when damage to government property is discovered. Any damage to government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report, the damage, the cause of damage (if known) and a proposal for repair.
5.11. Fire Extinguisher. The Contractor is responsible for providing sufficient-fully charged and operable fire extinguisher at each job site.
5.12. Fire and Emergency Reporting. Dial 911 from a standard phone. Dial 723-2461 from a cellular phone.
6.0 GOVERNMENT FURNISHED PROPERTY:
6.1 The Contractor shall not use property provided by the Government for any purpose other than in the performance of this contract.
6.2. Availabilities of Utilities. All reasonable quantities of utilities shall be made available to the contractor without cost. Any temporary connection of lines that may be used required or removed by the contractor will be at the contractor's expense. Temporary connection shall follow existing lines of the building and shall be installed safely IAW standard commercial practices.
7.0. STORAGE OF MATERIAL.
7.1 The Contractor shall not store material and equipment in the work area without the approval and coordination with 5 SFS. Material and equipment approved for storage shall be placed to provide continued facility use, neat in appearance and shall assure safety.
8.0. GENERAL INFORMATION
8.1. Hours of Operation. Work shall be performed in a manner to create minimum disturbance and in convenience to the use of the facility. Contactor personnel will be limited to work areas.
8.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises or actual alert, will be accomplished as soon as possible after the reopening the base.
