PERFORMANCE WORK STATEMENT

(28 Feb 17)

PROVIDE AND INSTALL HVLS (High-Volume Low Speed) Fans

1. SCOPE: The contractor shall provide all labor, material, supervision, travel, initial assembly and administration necessary to deliver and install HVLS fans/supplemental cooling systems in buildings 278, 285, 291, 450 and 517.

2. PERIOD OF PERFORMANCE: Work must be performed within 12 calendar weeks of receipt and work will be accomplished in buildings 278, 285, 291, 450 and 517 Altus

AFB, OK 73523.

3. PRINCIPAL FEATURES: The work to be accomplished consists, in general, of the following principal items. This does not mean the work consists only of these items, but that it consists of these principal items along with all incidental minor work which becomes necessary to complete in order to accomplish the overall project in satisfactory and workman like manner meeting the approval of the customer.

a. Building 278:

Eight (8) vertical column mounted fans for AGE Maintenance Bays

Fans require at least 1/5 HP motor, 120v/400w power

Minimum 30 inch diameter, maximum 36 inch

Fans required to oscillate a minimum of 180 degrees

Fans required to be installed with wall mount/column mount oscillator/speed controller for all units

Fans shall be HI-VIS color for safety

Fans required to be adjustable +/- 90 degrees from horizontal

Fans required to have OSHA compliance cage/shroud

Shrouds and cages required to be easily removable to facilitate cleaning

Fans required to be variable velocity, 528 Feet Per Minute (fpm) to 113 (to 140’) (fpm)

Fans required to have a minimum 3-year parts warranty

b. Hangar 285 Hyd Back Shop

Two (2) vertical column mounted fans for Hydraulic Back Shop

Fans require at least 1/5 HP motor, 120v/400w power

Eleven (11) vertical column mounted fans for Wheel and Tire Shop

Fans require at least 1/5 HP motor, 120v/400w power

Minimum 30 inch diameter, maximum 36 inch

Fans required to oscillate a minimum of 180 degrees

Fans required to be installed with wall mount/column mount oscillator/speed controller for all units

Fans shall be HI-VIS color for safety

Fans required to be adjustable +/- 90 degrees from horizontal

Fans required to have OSHA compliance cage/shroud

Shrouds and cages required to be easily removable to facilitate cleaning

d. Building 450

Three (3) vertical column mounted fans for NDI

Fans require at least 1/5 HP motor, 120v/400w power

Minimum 30 inch diameter, maximum 36 inch

Fans required to oscillate a minimum of 180 degrees

Fans required to be installed with wall mount/column mount oscillator/speed controller for all units

Fans shall be HI-VIS color for safety

Fans required to be adjustable +/- 90 degrees from horizontal

e. Hangar 517

Eight (8) vertical column mounted fans for Wash Rack

Fans require at least 1/5 HP motor, 120v/400w power

Minimum 30 inch diameter, maximum 36 inch

Fans required to oscillate a minimum of 180 degrees

Fans required to be installed with wall mount/column mount oscillator/speed controller for all units

Fans shall be HI-VIS color for safety

Fans required to be adjustable +/- 90 degrees from horizontal

Fans required to have OSHA compliance cage/shroud

Shrouds and cages required to be easily removable to facilitate cleaning

f. Hangar 517

One (1) - 20 ft. diameter fan, mounted in ceiling beams for Wash Rack Outermost 10 inches of fan blades shall be HI-VIS color for safety Fans require 200-250 volt/3 phase power Fans required to be installed with appropriate wall mounted controller for all installed units Fans required to be installed with fire control shutdown relay Fans required to be mounted on 5 foot drop style mounting brackets Fans required to rotate at 75 Revolutions Per Minute (RPM) (minimum) Area coverage: Approx. 48,000 sq. ft.

Fans required to have 10-year or better non-prorated warranty on motor, drive and controller

4. GENERAL: All work shall be performed in accordance with the manufacturer's installation specifications and procedures for each type of work required. When work is not covered by specifications, said work shall be performed in accordance with the manufacturer’s "written" specifications, installation instructions, performance procedures and (NEC) National Electrical Code, (NFPA) National Fire Protection Association Fire Code, UFC 3 -550-03N and 3-55-03FA, (UFC) Unified Facility Criteria.

5. SUBMITTALS: Contractor is responsible to provide detailed records of all parts installed and any and all warranty information, electrical plans to the contracting officer.

6. SPECIAL REQUIREMENTS: There is no smoking in buildings. There are designated tobacco areas located at or near all buildings.

7. The contractor will assign employees who possess certification to handle all required material handling, transportation, loading, unloading and product installation tasks to meet OSHA and base safety requirements. The contractor will ensure product meets required safety standards at the time the project is finished and accepted by the customer.

8. WARRANTY: Contractor must provide the standard commercial product warranty certificate specifying warranty period.

9. CONTRACTOR MANPOWER REPORTING: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site.

The contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY) which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

10. SECURITY REQUIREMENTS: The contractors will comply at all times with base security requirements to include base pass requirements and personnel security clearance.

11. ANTITERRORISM CLAUSE: Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate, Altus Air Force Base, Oklahoma.

12. IP-SECURITY GUIDANCE

12.1. Pass and Identification Items. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements.

12.2. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, and last, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974.

A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.

12.3. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma.

Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.

12.4. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.

13. Environmental Statement: Contractors must comply with all environmental requirements to include completion of initial Environmental Management Systems (EMS) awareness level training within 10 days of contract award or with a new contract employee beginning work on the Air Force Installation and the appropriate documentation of training completion is to be maintained. 14. Contractor personnel shall present a neat and clean appearance and be easily recognized as contractor employees. The contractor shall provide contractor personnel working at Altus AFB with a method of identifying the company name, employee’s first and last name, and job title. This may be accomplished by a name badge or a company shirt. Employees must wear identification at all times.
