STATEMENT OF WORK (SOW)
FLOORING SERVICES IDIQ
1. DESCRIPTION OF COMMODITY – NON PERSONAL. The Contractor shall provide all personnel, equipment, tools, materials, supervision, administration and services necessary to remove, dispose of, purchase and install floor coverings and accessories as specified in each delivery order. The work occurs in various facilities and commercial applications in indefinite quantities. All work to be performed shall be IAW recommended manufacturer’s commercial specifications and industry standards. The Contractor may be required to perform floor covering installation on multiple delivery orders at one time. The guaranteed minimum contract amount for the life of the contract is $9,000.
1.1. LOCATIONS.
1.1.1. Altus Air Force Base (AFB) is located in the city of Altus in Jackson County, Oklahoma.
1.1.2. Sheppard Air Force Base is located in the city of Wichita Falls in Wichita County, Texas.
1.2. COORDINATION OF WORK. The Contractor shall coordinate all work with the Quality Assurance Offices (QAO) listed below:

1.2.1. The Contractor shall be notified by telephone or e-mail when a proposed new flooring requirement is received. The QAO shall have available for the Contractor a copy of the building drawing(s) depicting the areas of the flooring request.
1.2.2. The Contractor shall do a site visit to verify all existing flooring conditions and obtain field measurements. The Contractor will provide the QAO a written estimate for the flooring request that includes exact area measurements, flooring quantities, required accessories and any additional floor treatment.
1.2.3. After the flooring requirement has been approved, the QAO will contact the using section to obtain color choices. The Contractor shall provide flooring samples or swatches to each
QAO.
1.2.4. Once the contract Delivery Order (DO) has been issued by base contracting, the Contractor shall have 90 days to order and receive materials.

1.2.4.1. If floor covering materials require more than 90 calendar days for delivery from the manufacturer, the Contractor shall provide a letter to the QAO from the manufacturer stating the estimated date of shipment to Contractor. The QAO shall forward this information to the Contract Officer (CO) or Contract Administrator (CA) so that the DO can be modified appropriately.
1.2.4.2. The Contractor shall notify the QAO immediately of any condition(s) that will prevent the suitable completion of the DO, such as sub-floor conditions that prevent a proper flooring installation.
1.2.5. Written work schedules detailing the installation dates and estimated date of completion shall be submitted to the QAO prior to DO performance start. A copy of the building drawings depicting the areas receiving services will be supplied to the Contractor.
1.3. MOVING FURNITURE. The Contractor shall be responsible for the removal and reinstallation of the following furniture type items: desks, bookcases, cabinets, credenzas, tables, boxes, chairs, trashcans and dormitory furnishings. Contractor will not be responsible for the moving of computer equipment, vending machines, personal items, safes, pool tables or wall hangings. Furniture moved by the Contractor shall be returned to its original location. The Contractor shall be responsible to move, lift and reset System and Modular Furniture to the extent necessary for tucking carpet tile under walls/legs.
1.4. REMOVAL OF FLOOR MATERIALS. The Contractor shall remove all flooring materials to accommodate the installation of new materials. The Contractor shall clean and prepare existing substrates to receive new materials.
1.5. FLOOR PREPARATION. Before any work begins, the Contractor shall remove top layer of existing flooring and correct all defects such as rough or spalling concrete, low spots, high spots and uneven surfaces to ensure a smooth, true and level plane except where indicated as a sloped area. Concrete curing compounds, paint, varnish, oils, release agents, sealers, waxes and adhesives shall be completely removed. After cleaning and removal of loose particles, prime chalky or dusty surfaces with primer.
1.6. CLEAN-UP AND DISPOSAL. Vacuum newly installed carpet using a machine with a face-beater bar element. Remove spots or stains and replace flooring materials where spots cannot be removed and clean base cove of all foreign materials. Clean all other flooring materials according to the manufacturer recommendations and specifications. The Contractor is responsible for removing all waste materials and debris from the installation site and discarding at a state permitted disposal site. The Contractor must report the quantity in tons of debris removed from the base and the quantity/type of materials recycled to the QAO each month.
1.7. FLOOR LEVELING/FLOATING. If Contractor determines that additional floor leveling or floating is required, the contractor shall immediately contact the QAO prior to commencement of work. A contract modification must be issued prior to leveling or floating of the floor. Price for floor leveling/floating shall be included in CLINS X011.
1.7.1. Contract shall provide a latex underlayment to fill depressions and for smoothing, leveling or to create a feathered edge. Latex filler or underlayment shall have a minimum compressive strength of 2500 PSI.
1.8. MOISTURE TESTS. Contractor may be required to perform moisture tests as specified in the delivery order. Testing will be coordinated with the QAO. Concrete floors must be allowed to cure prior to installation of flooring materials. The suitability of concrete subfloor for receiving flooring products with regard to moisture content shall be determined by spreading a 6” x 6” patch of adhesive in each of 150 SF area to receive flooring and allowed to dry over-night. If latex underlayment is used, test patches shall include areas covered with underlayment.
If the set adhesive can be scraped easily from the floor surface, the floor is not sufficiently dry.
Repeat tests until adhesives adhere properly. When adhesive adheres tightly to the floor surface, proceed with the installation. Moisture tests shall be separately priced in CLIN X012.
1.9. EMERGENCY WATER EXTRACTION/REMOVAL. The QAO is responsible for determining the square footage area for emergency water extraction/removal prior to notifying the Contractor of the requirement (to ensure funds are available). Upon notification from the QAO, the Contractor shall respond within 4 hours to the site with equipment and personnel to preform water extraction/removal services. Emergency water extraction/removal shall be separately priced in CLIN X013.
1.10. FLOORING MATERIALS. The Contractor shall furnish adhesives, seam sealer, seam sticks, welding rods, seam tape, tack strips, etc. for installation of floor covering materials.
Contract Line Item Number (CLIN) pricing shall include the cost of flooring removal, clean-up and disposal debris, flooring preparation (excluding leveling floors and floating floors), flooring materials and installation costs (including labor). Once the materials are received, the Contractor shall contact the QAO to coordinate the installation schedule with the work section having the flooring installed.
1.10.1. Commercial Carpet, Broadloom and Tile (CLINs X001 – X002). Carpet and carpet tile for commercial applications shall be installed using the standard for commercial carpet, Carpet and Rug Institute – CRI-104.
1.10.2. Vinyl Composition Tile (CLIN X003). Vinyl composition tile shall be asbestos free and conform to the American Society for Testing and Materials (ASTM) F1066-04, Standard Specification for Vinyl Composition Floor Tile. Materials shall be from the same lot and have the same shade and pattern.
1.10.3. Luxury Vinyl Flooring (CLIN X004). Flooring shall conform to requirements of ASTM F1700 Class III Type B. Materials shall be from the same lot and have the same shade and pattern.
1.10.4. Ceramic, Tile (CLINs X005). All flooring shall be Class IV-Heavy Traffic or Class IV Plus-Extra Heavy Traffic as rated by the manufacturer when tested in accordance with ASTM C 1027-09, Standard Test Method for Determining Visible Abrasion Resistance of Glazed Ceramic Tile.
1.11. FLOORING ACCESSORIES. The Contractor shall furnish adhesives, seam sealer, seam sticks, welding rods, seam tape, tack strips, etc. for installation of flooring accessories.
Contract Line Item Pricing for accessories shall include the cost of the accessory material and installation costs (including labor).
1.11.1. Rubber/Vinyl Cove Base (CLIN X006). Surfaces on which new cove base is to be applied shall be patched and be free of all foreign materials. Rubber tile and products shall conform to ASTM F 1344-10, Standard Specification for Rubber Floor Tile. Cove base material color shall be identified in the delivery order. Manufacturer’s preformed inside and outside corners shall match the straight length of cove base. The Contractor shall not use straight lengths of cove base that are bent, folded, tucked or creased around corners.
1.11.2. Vinyl Transition (CLIN X007). The Contractor shall install vinyl transitions as needed. These will be included in the Contractors initial estimate of materials.
1.11.3. Metal Transition (CLIN X008). The Contractor shall install metal transitions and door thresholds as needed. These will be included in the Contractors initial estimate of materials.
1.11.4. Door Threshold (CLIN X009). The Contractor shall install door thresholds as needed. These will be included in the Contractors initial estimate of materials.
1.11.5. Stair Treads/Nosings (CLINs X010). Vinyl/Rubber stair treads/nosings shall be one piece. Surfaces of treads shall be raised, non-slip, studs, diamond, rectangle or ribbed pattern.
1.12. WARRANTY WORK. Installation shall have a warranty period of 90 days. If there is a problem with the installation (such as tiles coming loose), the Contractor shall be required to remedy the problem within 10 business days of notification.
2. SERVICE SUMMARY (SS): The Contractor service requirements are summarized into performance thresholds that relate directly to mission essential items. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. All aspects of the Contractor performance are subject to QAO surveillance, and absence of any contract requirement from the SS shall not detract from its enforceability nor limit the rights or remedies of the Government under contract clauses.
2.1. SS TABLE:
2.2. QUALITY ASSURANCE: The QAO will assess the Contractor’s performance and adherence to standards using the SS. If inspection reveals unacceptable performance, the QAO shall give the Contractor a punch list to complete prior to acceptance of the work. If the work is still unacceptable after the punch list has been addressed, the QAO shall notify the CO. If a second inspection reveals unacceptable performance, the CO shall notify the Contractor and require re-performance of the service (referred to as “call back”). The Contractor shall be allotted time as agreed to by all parties, but no longer than 5 business days, to return to Altus AFB and accomplish corrective call back work.
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES:
3.1. GOVERNMENT FURNISHED PROPERTY: The Government will not provide any facilities, equipment, supplies or materials. The Government will furnish without cost to the Contractor a reasonable amount of electricity and potable water in connection with the contract performance. The Contractor shall practice energy conservation in the Government provided facilities.
3.2. GOVERNMENT FURNISHED SERVICES: The Government shall provide police and fire protection. In the event of a medical emergency, call “911” for transportation of an injured employee by civilian ambulance to a local hospital. The Government shall provide emergency medical treatment for life-threatening emergencies only until civilian ambulance arrives.
3.3. DAMAGE TO GOVERNMENT FACILITIES AND PROPERTY: The Contractor shall reimburse the Government if details of formal investigation of fire or other related or unrelated structural damage result in pecuniary liability assessed to the Contractor. The Contractor shall replace all damaged or lost Government Furnished Facilities, Government Furnished Equipment and Government Furnished Materials at no cost to the Government.

4. GENERAL INFORMATION:
4.1. CONTRACTOR PERSONNEL: Contractor shall provide a foreman who is currently certified by the International Certified Floor Covering Installers Association, Commercial II or higher. The foreman shall be on base at all times while work is being performed. Contractor shall provide copies of certification to the CO prior to performance start date and upon any personnel changes.
4.1.1. Contract personnel shall present a neat appearance. Contract personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
4.2. SECURITY REQUIREMENTS. The Contractor shall comply with Altus AFB and Sheppard AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.
4.2.1. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB and Sheppard AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus AFB and Sheppard AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.
4.2.2. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the QAO within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use.
Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.
4.2.3. Anti-Terrorism. Altus AFB and Sheppard AFB have an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. All Contract employees and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages or items you see while conducting work on Altus AFB and Sheppard AFB. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base.
4.2.4. Vehicles. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause FAR 5352.242- 9000, entitled “Contractor access to Air Force Installations.” All vehicles shall be registered, insured, and operated by a licensed operator at all times.
4.3. SAFETY. The Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.
4.4. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)
REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB and Sheppard AFB until that material is authorized for use by the Civil Engineer Environmental Element. The Contractor shall submit a list of chemicals with attached Material Safety Data Sheets (MSDS) and other supporting documentation for each HM prior to their use to the Environmental Element and QAO. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies. Point of Contact for each base are listed below:

4.4.1. Spill Control. The Contractor shall maintain spill control material on hand at all times sufficient to contain a worst-case spill, both volume and hazard level. Contractor is responsible for disposal of all material used during spill clean-up. Materials must be disposed of IAW local, state and/or federal regulations. If the Contractor spills or releases any hazardous substances/hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department and the QAO/CO. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.
4.4.2. Hazardous Materials Management. The Contractor shall remove all hazardous materials by the end of each workday, unless permission is granted by CE Environmental Element. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base. Contractor is responsible for proper storage, labeling and security of each cabinet.
4.4.3. Altus AFB Contractor Environmental Awareness Training. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental
Management System (EMS) Awareness training IAW AFI 32-7001, paragraph 5.6, Environmental Management within 30 days of starting work. The EMS Contractor Awareness training can be accessed at http://www.altus.af.mil/altusafbenvironmentalnews.asp. The Contractor shall submit a list of the employee names that have completed this training to the QAO within 30 days of employee starts date. This is a one-time requirement for all current and new Contractor/contract employees working on Altus AFB.
4.5. HOURS OF OPERATION: The Contractor shall perform services during normal duty hours which are 0700 to1700, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the QAO.
4.5.1. Recognized Holidays: The Contractor is not required to provide service on federal holidays, except in emergency situations. The holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.
4.5.2. Base Closures: Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
4.5.3. Performance of Services During Crisis or Heightened Security: Services at Altus AFB and Sheppard AFB are not considered mission essential and service will not continue in the event of a crisis or heightened security caused by a national emergency, natural disaster or other cause, as necessary, in support of the Air Force mission. Contractor employees with approved background checks will be allowed access to the base for performance of work, except during “Charlie” conditions. If the “Charlie” condition exists for an extended duration, some services may be required. The CO may direct that services continue on a case-by-case basis.
