The suitability of concrete subfloor for receiving flooring products with regard to moisture content shall be determined by spreading a 6” x 6” patch of adhesive in each of 150 SF area to receive flooring and allowed to dry overnight. If latex underlayment is used, test patches shall include areas covered with underlayment. If the set adhesive can be scraped easily from the floor surface, the floor is not sufficiently dry. Repeat tests until adhesives adhere properly. When adhesive adheres tightly to the floor surface, proceed with the installation.

Local Anti-Terrorism Office Info 20 September
Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of
Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base.
In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the
Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

RESTRICTED AREA FREE ZONES
Procedures While Working Within Controlled or Restricted Area Free Zones
1. Welcome to Altus AFB. To ensure your employees are aware of requirements for performing work on Altus AFB, we ask you to take a few moments to read this memo. We look forward to working in concert with you to ensure work is completed efficiently, while meeting our force protection/antiterrorism requirements. Ifyou see anything you feel is suspicious, please call the Base Defense Operations Center (BDOC) at (580) 481-
7444. When using an on-base telephone line, you must dial the 481 prefix. You may also dial 911 from any government phone. Ifyou are on-base and dial 911 from a cell phone, the call may route to the City of Altus 911. To avoid delay, always request Altus AFB 911 if calling from on-base.
2. The following security requirements apply to all contractors working on Altus AFB.
a. The Force Protection Condition (FPCON) at Altus AFB reflects local, national and international security threats. FPCON Normal is the lowest security level, FPCON
Alpha is the next security level, FPCON Bravo is the next higher security level, FPCON Charlie the fourth highest, and FPCON Delta is the highest security level.
Normally, all contractor employees receiving a picture ID badge will be allowed entry onto the base during FPCONs Normal, Alpha, and Bravo. Ifthe official contractor requires access during higher FPCONs for mission accomplishment , it is the responsibility of the unit commander most directly related to that contract to develop an entry authority list (EAL) that includes the company name, full name of the individual(s), last six numbers of the individuals ' social security numbers, and times and days the individual(s) is authorized to enter the installation during increased
FPCONs.
b. The contract administrator must provide the 97th Security Forces Squadron Pass and
Registration office (bldg 428) a memo on company letterhead listing names and social security numbers of all employees who will work on Altus AFB. The memo will be revised when personnel requiring access to the installation are added or deleted. The
97th Security Forces Squadron's Installation Access Control will provide guidance for obtaining contractor badges. The 97th Security Forces Squadron Pass and Registration office will facilitate the issuance of the badges. To ensure installation access is not delayed, contract administrators/ sponsoring organizations must ensure the contractors have all required documentation to obtain contractor badges and vehicle passes prior to the scheduled appointment with Pass and Registration. Large groups require more processing time.
c. Security at Altus AFB, OK, requires a DBIDS pass or badge for all contractor employees entering the base. A contractor working less than 30 day will be issued a pass verses a badge.
The following is a summary of the DBIDS pass/badge process:
(1) Prior to starting work at Altus AFB, construction contractors and subcontractors must coordinate with the sponsoring base organization and complete the "Individual Application for Badge" form. Under most circumstances, contractor employees must qualify for unescorted entry to work on Altus AFB.
(2) The sponsoring base organization will schedule an appointment with the Security
Forces Squadron, Pass and ID office for personnel processing. The contractor personnel must have a valid driver's license or state ID present in addition to their completed individual application badge form before processing can begin. The sponsor will escort the contracting personnel to the Pass and Registration office and will remain present until the background checks are complete. A criminal background check and fingerprinting of the personnel will be conducted, and upon successful completion, a contractor badge/pass will be issued.
(3) Completion of the above steps normally requires from one to two hours. It is recommended that processing be completed prior to start of work but MUST be completed no-later-than close of business on the contractor's first work day. Thereafter, base access will be denied until processing is completed.
(4) Ifderogatory information is revealed during a background check, the Wing/CC may deny access and access credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline and morale of the installation. Personnel will be denied an unescorted entry credential if any of the following apply: the individual is known to be, or reasonably suspected of being a terrorist, or belongs to an organization with known terrorism links/support; unable to verify the individuals claimed identity; there is reasonable suspicion to believe the individual has submitted fraudulent information concerning his or her identity; there is reasonable suspicion to believe the individual will unlawfully or inappropriately use an access credential outside the workplace; there is a reasonable basis to believe, based on an individual's criminal or dishonest, history, that issuance of an access credential poses an unacceptable risk to the installation/mission; a repetitive history of criminal activity defined as at least one arrest and/or conviction during the three previous years (i.e. at least one in
2010, one in 2011, and one in 2012); the individual has/had been barred from entry/access to a Federal installation or facility; actively wanted by federal or civil law enforcement authorities (regardless of offense or violation); incarcerated for 12 months or longer within the past ten years (regardless of offense/violation unless released on proof of innocence);
any conviction for espionage, sabotage, terrorism or murder; the individual's name appears on any federal agency's "watch list" or "hit list" for criminal behavior or terrorist activity;
the individual has been convicted of any firearms or explosive violation within the past ten years; the individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution; there is a reasonable suspicion to believe the individual will attempt to gain unauthorized access to classified documents, information protected by the Privacy
Act, information that is proprietary in nature, or other sensitive or protected information;
there is a reasonable suspicion to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk to the installation/mission;
there is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission (defined on Altus AFB as
3 alcohol related incidents); there is a reasonable basis to believe, based on the nature or duration of the individual's illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission (defined on Altus AFB as 3 drug related incidents); a statutory or regulatory bar prevents the individual's contract employment or would prevent Federal employment under circumstances that furnish a reasonable basis to believe that issuance of a access credential poses an unacceptable risk to the installation/mission; the individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force; any other misconduct the commander determines may have a negative effect on good order and discipline or installation security.
(5) Contractors will enter Altus AFB through the South Gate, located on highway 62 east of Veterans Drive, from 6:00 AM. through 6:00 P.M. Monday through Friday, except on holidays. Entry through the Main Gate is authorized when the South Gate is closed.
(6) Proof of vehicle insurance, the vehicle registration certificate with the expiration date, and a driver's license are required to operate a privately owned vehicle (POV) on the installation. POVs shall only access the base through the Main Gate.
(7) Speed Limits:
Base Cantonment Area: 25 MPH unless otherwise posted
Base Housing Area: 20 MPH unless otherwise posted
Parking Lots: 5 MPH unless otherwise posted
(8) Emergency traffic lights are installed adjacent to the Main Gate and South Gate and are activated when the gates are being closed. All vehicle traffic will stop and hold their position until the gates re-open.
d. Free zones may be established within restricted/controlled areas when construction projects and similar activities make it inappropriate or impractical to apply normal circulation controls.
In such cases, a free zone corridor is established from some point on the restricted/controlled area boundary to the work project. The USAF agency most closely associated with the project will be responsible for the physical establishment of the free zone. That same agency will provide Free Zone escorts with immediate visual assessment (IVA) and control entry/exit into the Free Zone. When a free zone corridor is impractical due to its location, owner and/or user personnel will escort workers through the restricted area Entry Control Point (ECP) to the free zone ECP. In addition, the owner/user agency is responsible for maintaining and posting an EAL with the free zone owner and/or user entry controller and the BDOC.
(1) Free zone boundaries are marked with elevated red ropes, barriers, fencing or other suitable materials to include restricted/controlled area warning signs every 100 feet to clearly delineate it from the restricted area.
(2) Contractors must be aware of the free zone boundaries and ensure they do not cross the boundary without specific approval from their escort or from security forces.
(3) Notify Security Forces at (580) 481-7444 prior to going to the free zone and secure the free zone by roping it off with red rope at the end of the normal working hours and notifying Security Forces when closed.

(4) Contractors working in free zones will search all vehicles prior to entering the installation and will report any discrepancies to the gate guard upon entering the installation or Security Forces Desk Sergeant at 481-7444.
e. Only those personnel specifically authorized by the installation commander may enter restricted areas. Restricted areas are readily identifiable by an elevated red rope barrier, red pavement lines and restricted area signs. Ifyou are unclear on restricted area boundaries or procedures, contract the Installation Security Section (ISS) at 481-7242 or the BDOC at 481- 7444 before proceeding. Individuals who violate restricted area procedures will be detained and either escorted off the installation or turned over to proper supervisor/civilian authorities.
Personnel working in restricted areas will:
(1) Be escorted by an individual authorized unescorted entry authority and in possession of a valid restricted area badge.
(2) Remain within your escort's physical control at all times and follow any instructions given to you. Should you become separated from your escort, you must contact the nearest person or proceed to the nearest entry control point and exit the restricted area.
(3) Immediately adhere to all instruction provided to you by the Security Forces or other personnel within the restricted area.

(a) The offeror certifies that --
(1) The prices in this offer have been arrived at independently, without, for the purpose of restricting competition, any consultation, communication, or agreement with any other offeror or competitor relating to –
(i) Those prices,
(ii) The intention to submit an offer, or
(iii) The methods of factors used to calculate the prices offered:
(2) The prices in this offer have not been and will not be knowingly disclosed by the offeror, directly or indirectly, to any other offeror or competitor before bid opening (in the case of a sealed bid solicitation) or contract award (in the case of a negotiated solicitation) unless otherwise required by law; and

52.209-5 CERTIFICATION REGARDING RESPONSIBILITY MATTERS (APR 2010)
(a)(1) The Offeror certifies, to the best of its knowledge and belief, that-
(i) The Offeror and/or any of its Principals-
(A) Are ( ) are not ( ) presently debarred, suspended, proposed for debarment, or declared ineligible for the award of contracts by any Federal agency;
(B) Have ( ) have not ( ), within a three-year period preceding this offer, been convicted of or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public (Federal, State, or local) contract or subcontract; violation of Federal or State antitrust statutes relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, violating Federal criminal tax laws, or receiving stolen property (if offeror checks “have”, the offeror shall also see 52.209-7, if included in this solicitation); and

(1) Federal taxes are considered delinquent if both of the following criteria apply:
(i) The tax liability is finally determined. The liability is finally determined if it has been assessed. A liability is not finally determined if there is a pending administrative or judicial challenge. In the case of a judicial challenge to the liability, the liability is not finally determined until all judicial appeal rights have been exhausted.
(ii) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.
(2) Examples. (i) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(ii) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(iii) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.
(iv) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).
(ii) The Offeror has ( ) has not ( ), within a three-year period preceding this offer, had one or more contracts terminated for default by any Federal agency.
(2) Principal, for the purposes of this certification, means an officer, director, owner, partner, or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager;
head of a division or business segment; and similar positions).
(b) The Offeror shall provide immediate written notice to the Contracting Officer if, at any time prior to contract award, the Offeror learns that its certification was erroneous when submitted or has become erroneous by reason of changed circumstances.
(c) A certification that any of the items in paragraph (a) of this provision exists will not necessarily result in withholding of an award under this solicitation. However, the certification will be considered in connection with a determination of the Offeror's responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.
(d) Nothing contained in the foregoing shall be construed to require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a) of this provision. The knowledge and information of an Offeror is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.
(e) The certification in paragraph (a) of this provision is a material representation of fact upon which reliance was placed when making award. If it is later determined that the Offeror knowingly rendered an erroneous certification, in addition to other remedies available to the Government, the Contracting Officer may terminate the contract resulting from this solicitation for default.

52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (JULY 2013)
(a) Definitions. As used in this provision--
Administrative proceeding means a non-judicial process that is adjudicatory in nature in order to make a determination of fault or liability (e.g., Securities and Exchange Commission Administrative Proceedings, Civilian
Board of Contract Appeals Proceedings, and Armed Services Board of Contract Appeals Proceedings). This includes administrative proceedings at the Federal and State level but only in connection with performance of a Federal contract or grant. It does not include agency actions such as contract audits, site visits, corrective plans, or inspection of deliverables.
Federal contracts and grants with total value greater than $10,000,000 means--
(1) The total value of all current, active contracts and grants, including all priced options; and
(2) The total value of all current, active orders including all priced options under indefinite-delivery, indefinite-quantity, 8(a), or requirements contracts (including task and delivery and multiple-award Schedules).
Principal means an officer, director, owner, partner, or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager; head of a division or business segment; and similar positions).
(b) The offeror ( ) has ( ) does not have current active Federal contracts and grants with total value greater than
$10,000,000.
(c) If the offeror checked “has” in paragraph (b) of this provision, the offeror represents, by submission of this offer, that the information it has entered in the Federal Awardee Performance and Integrity Information System (FAPIIS) is current, accurate, and complete as of the date of submission of this offer with regard to the following information:
(1) Whether the offeror, and/or any of its principals, has or has not, within the last five years, in connection with the award to or performance by the offeror of a Federal contract or grant, been the subject of a proceeding, at the Federal or State level that resulted in any of the following dispositions:
(i) In a criminal proceeding, a conviction.
(ii) In a civil proceeding, a finding of fault and liability that results in the payment of a monetary fine, penalty, reimbursement, restitution, or damages of $5,000 or more.
(iii) In an administrative proceeding, a finding of fault and liability that results in--

(d) The offeror shall post the information in paragraphs (c)(1)(i) through (c)(1)(iv) of this provision in FAPIIS as required through maintaining an active registration in the System for Award Management database via https://www.acquisition.gov (see 52.204-7).

THIS IS A COMPETITIVE BEST VALUE ACQUISITION USING SIMPLIFIED PROCEDURES AUTHORIZED
BY FAR 13.5 “TEST PROGRAM FOR CERTAIN COMMERCIAL ITEMS”
(a) The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes:
1. Price
2. Technical Acceptability
3. Past Performance Acceptability
(b) Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
