by Rite-Hite OR EQUAL. The contractor shall provide any mounting stations, equipment and material necessary to completely install the new truck leveler, controls, and Dok-Lok
OR EQUAL. Existing dock locks will be removed and kept by the Government.
1.1.2. Equivalent products must meet the requirements detailed below:
Truck Leveler (QTY: 1):
1.1.2.1. Truck leveler standard structural lifting capacity of 60,000 lbs.
1.1.2.2. 1-1/8” collapsed height of the traffic surface.
1.1.2.3. 12” debris guards close off sides.
1.1.2.4. Integral lifting lugs.
1.1.2.5.Able to support loading as low as 33 inches.
1.1.2.6.Able to support loading as high as 53 inches.
1.1.2.7. Automatic tire sizing.
1.1.2.8. A minimum of two 4” thick x 10” wide x 36” high steel-faced bumpers.
1.1.2.9.The loading dock is 53 inches tall and the trailers that are receive vary in height with the shortest being as low as 33 inches tall. The dock leveler will need to be adjustable to zero out the height difference between the dock and the various trailers that arrive.
1.1.2.10. Electrical requirements: 3 phase; 480 Volts; 14 Amps.
1.1.3. The contractor shall provide professional installation and electrical services to permanently install and connect truck leveler and control box. All electrical work shall comply with
NFPA 70, National Electric Code.
Dock Lock (QTY: 2):
1.1.4 Automatic tire sizing
1.1.5 A minimum of two 4” thick by 10” wide by 36” high steel-faced bumpers
1.1.6 All electrical work shall comply with NFPA 70, National Electric Code
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. There will be no
Government Furnished Property or Services provided for this effort.
3. HOURS OF OPERATION.

Truck Leveler
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide and install control box, full truck lift, and dock lock at Hurlburt Field, FL.
1.1. BASIC SERVICES.
1.1.1. Provide and install ThinMan Truck Leveler OR EQUAL, controls, and SHR5000 Dok-Lok
3.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: MON-FRI 0700 to 1600, except for federal holidays. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government. Dates and times of installation must be coordinated and approved in advance by 1SOLRS personnel.
3.2. Recognized Holidays. The contractor is not required to provide service on the following days:
New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on
Monday.
3.3 The installation shall not last more than 7 calendar days from start of the project. The contractor shall uncrate/unpack all new items and perform required assembly in accordance with the manufacturer’s instructions. Contractor shall remove and dispose of packing material at an off base location.
4. BASE ACCESS
4.1 Contractor, Employee Base Pass and Identification, Special Clearances, and Vehicle
Passes. The contractor shall obtain base identification and vehicle passes, if required, for all
Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation. The contractor shall submit the employee listing the Contracting Officer no less than 3 days prior to the performance start date and at least one day prior as changes occur. Employees with active warrants or felonies will not be granted access to the base.
4.2 The Contractor Employee List. This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word
“escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in
EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the
Employee List to the Visitor Control Center.
4.3 Upon notification of the employee listing being forwarded to the Visitor Control Center
(VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification. If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of
Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.
4.4 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
4.5 WEAPONS, FIREARMS AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.
4.6 DRIVING LICENSE/ON-BASE REQUIREMENTS. The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.
