SECTION 1
DESCRIPTION OF SERVICES
1.1 SCOPE OF WORK: The contractor shall provide all personnel, supervision, supplies, units, tools, materials, and vehicles for the installation, removal, maintenance/repairs and dryer vent cleaning services for the washers and dryers at Hurlburt Field, Florida. The contractor shall perform the work requirements in accordance with the Performance Work Statement (PWS).
1.2 INITIAL INSTALLATION: The contractor shall install commercial grade heavy duty washers and dryers that are brand new. The units shall have no defects, free of debris and shall be clean upon installation. All units shall be IAW the standards defined in section 1.7.
1.2.1 The initial duct vent cleanings shall be completed within 30 days after the initial installation of the new units. All duct vent cleaning shall be conducted in accordance with paragraph
1.3.6, Dryer Vent Cleaning in this PWS.
1.2.2 The installation of units shall be coordinated within 1 week from the date of award, and the entire installation shall be accomplished within 30 days from the date of award. Installation shall take place be between the hours of 0700 and 1600 CST, Monday through Friday.
1.2.3 The contractor shall coordinate with the Contracting Officer Representative (COR) prior to installation in order to schedule dates and times for installation of units at each location.
1.2.4 Units shall be numbered at the time of installation in accordance with paragraph 1.5 of this
PWS. Section 1.7 shall instruct the contractor on the style and type of units that shall be placed at each location.
1.3 MAINTENANCE. The contractor shall ensure that proper maintenance/repair is performed on all units in accordance with the PWS during the period of performance and that the manufacturer’s warranty is utilized.
1.3.1 Manufacturer Warranty or Express Warranties. IAW FAR 12.404—Warranties, “the contractor shall offer the Government at least the same warranty terms, including offers of extended warranties, ordered to the general public in customary commercial practice”. The contractor shall provide details on the manufacturer warranty for each washer, dryer, and stacked units.
1.3.2 Inoperative Units. The COR shall notify the contractor when units are inoperative or defected. The contractor shall repair the units within the time frame of 48 hours unless replacement parts are required. Please refer to section 1.3.4 for Replacement Parts.
1.3.3 Work Order Form. The contractor shall provide their own work order form. A work order form shall be completed by the contractor each time the unit’s needs maintenance, repair, or replacement. The work order form shall provide the following information date issued, date serviced, building number, unit’s number, parts used, repair comments, manufacturer warranty part or non-manufacturer warranty part and printed name and signature of the contractor. The contractor shall give a copy of the work order form to the COR for their records. The contractor shall provide a completed work order form after every visit for repair or maintenance.
1.3.4 Replacement Parts. The contractor shall coordinate with the COR within 48 hours of the normal hours of operation, if the contractor must order replacement part(s) to fix the inoperative or defected units. The contractor shall have a maximum of 7 work days within the normal hours of operation from the date of coordination with the COR to order, receive and replace the part(s), and the units shall be in working condition upon repair. The contractor shall return immediately if the COR states the replaced part(s) is not in working conditions. Repair(s) shall be applied to CLIN X006 for invoicing.
1.3.4.1. The contractor shall provide documentation for all replacement parts not included within the manufacturer warranty and that will be invoiced. The documentation shall include: manufacturer brand, cost of parts and labor hours required.
1.3.5 Replacement of Units. If the units is not repairable, the contractor shall replace the units with a new unit of units at no additional charge to the Government and utilize the manufacturer warranty if applicable. The contractor shall inform the COR immediately.
The units shall be brand new or comparable to the unit that will be replaced, and operable upon replacement. The contractor shall have a maximum of seven (7) work days within the normal hours of operation to replace the units. A work order form must be completed and submitted to the COR for replacement of units.
1.3.6 Dryer Vents Cleaning/Units Inspections. The contractor shall have a maximum of 10 work days to complete all dryer vent cleanings and units inspections per first day of each quarter. The contractor shall perform quarterly dryer vent cleanings on the items listed in
Appendix 2, which include the following types of dryer vents; exterior wall duct vents and eave duct vents.
1.3.7 The contractor shall also conduct an inspection on the washer, dryer and stacked units at the time of the dryer vent cleanings to ensure all units is working properly and has no defects; complete Appendix 3 for inspections of units. Appendix 2 lists the building number for each location and the type of vent associated with the building number.
1.3.8 The contractor shall complete the Dryer Vent Cleaning/Maintenance log attached as
Appendix 3, and submit the Dryer Vent Cleaning/Maintenance log every first business day following the end of the quarter to the COR. All dryer vents listed in Appendix 2 shall be maintained by the contractor. The dryer vents shall be free of any debris, which can cause blockage in the dryer vents. This is to prevent unit’s failure, fire and impairment to mission essential personnel. The contractor shall clean the lint traps, exhaust duct on the dryers, central ducts from the dryers to the exterior of the facilities and the multi-story facilities.
Appendix 2 lists all the duct vents that need to be cleaned quarterly. Building 90232, Commando Fitness Center must be cleaned up to 10 feet.
1.4 ADDING TO AND/OR REMOVING FROM EXISTING UNITS
1.4.1. Adding to Existing Units. The Government at any time during the contract period of performance may request adding additional units to the existing number of units at one or all locations on Hurlburt Field, FL. The COR will notify the contracting office of the request for additional units. The contractor shall have 7 working days from the date of award modification, to add the units, and to coordinate with the COR on location of the new units. The contractor shall install commercial grade heavy duty washers and dryers that in accordance with paragraph 1.2 and
1.7 of this PWS. Units shall be added during normal hours of operation.
1.4.2. Removing from Existing Units. The Government at any time during the contract period of performance may request removing existing units from the existing number of units units at one or all locations on Hurlburt Field, FL. The COR will submit a request to the contracting officer.
The contractor shall have 7 working days from the date of award modification, to remove the units, and to coordinate with the COR on location of existing units. Units shall be removed during normal hours of operation.
1.4.3. Upon approval, the contracting officer will contact the contractor and issue a contract modification reflecting the requested changes. No additions or removals shall take place without the receipt of a contract modification.
1.5. NUMBERING. The contractor shall number each units in sequential order starting at number one by each location. For example, 1 through 20 at location one and 1 through 30 at location two.
Numbers shall be placed on the front, upper right-hand corner of each units. Numbers shall be waterproof and replaced when they become faded, soiled or defaced.
1.5.1. Numbering on Work Order Form. The contractor shall list the assigned units number and location on each work order form.
1.5.2. Inventory List. The contractor shall provide the COR with an inventory list of all units prior to completion of initial installation. The contractor will submit an updated inventory list after initial installation from date of award, replacement of units, removal of units, and/or adding to existing units. This includes the assigned unit’s number, location, model number and the type of units such as washer, dryer, or stacked units. 1.6 END OF CONTRACT.
1.6.1 Removal of all Units. The contractor shall remove all units within 1 week after expiration or termination of this contract between the hours of 0700 and 1600 CST, Monday through Friday.
1.7 CONTRACTOR FURNISHED ITEMS:
1.7.1 Single Electric Washer Units. The single electric washer units shall be front load commercial grade heavy duty and shall be in accordance with energy star requirements stated in FAR 23.203—
Energy Efficient Products located at http://farsite.hill.af.mil. The washer units shall have a minimum tub capacity of 3.1 cu. Ft. The washer units shall have standard hot and cold water hoses and be capable of operating on a 20-amp circuit without overloading the electrical circuit during normal operation. The washer units shall have standard commercial settings in relation to water temperature and wash cycles for various fabric materials. The Government shall provide electrical at 120 volts AC, 60 Hz. The contractor shall provide the Government with the manufacturer warranty details, and utilize the warranty for covered parts during the duration of the contract.
1.7.2 Single Electric Dryer Units. The single electric dryer units shall be front load commercial grade heavy duty and shall be in accordance with energy star requirements stated in FAR
23.203—Energy Efficient Products located at http://farsite.hill.af.mil. The dryer units shall have an exhaust fan and shall have exhaust flex hose, and shall have standard commercial settings in relation to drying times and fabric settings. The dryer units shall be capable of operating on a 30-amp circuit without overloading the electrical circuit during normal operation. The Government shall provide electrical power at 208 volts AC, 60 Hz. The contractor shall provide the
Government with the manufacturer warranty details, and utilize the warranty for covered parts during the duration of the contract.
1.7.3 Stacked Electric Washer and Dryer Units. The stacked washer and dryer units shall be commercial grade heavy duty and shall be in accordance with energy star requirements stated in
FAR 23.203—Energy Efficient Products located at http://farsite.hill.af.mil. The stacked washer units shall have a minimum tub capacity of 2.8 cu. ft., 30-amp circuit and have standard hot and cold water hoses. The stacked dryer units shall have an exhaust fan and shall have exhaust flex hose, and shall have automatic dry cycles, minimum tub capacity of 7 cu. ft., 30-amp circuit. The
Government shall provide electrical power at 208 volts AC, 60 Hz. The contractor shall provide the Government with the manufacturer warranty details, and utilize the warranty for covered parts during the duration of the contract.
1.7.4 Stacked Electric Dryer and Dryer Units. The stacked dryer and dryer units shall be commercial grade heavy duty and shall be in accordance with energy star requirements stated in
FAR 23.203—Energy Efficient Products located at http://farsite.hill.af.mil. The stacked dryer units shall have an exhaust fan and shall have exhaust flex hose, and shall have automatic dry cycles, minimum tub capacity of 7 cu. ft., 30-amp circuit .The Government shall provide electrical power at 208 volts AC, 60 Hz. The contractor shall provide the Government with the manufacturer warranty details, and utilize the warranty for covered parts during the duration of the contract.
1.8. Deliverables. The Contractor shall submit the following deliverables in accordance with the timelines in the below table. All deliverables must meet professional standards and meet the requirements set forth in the contract documentation. The Contractor shall submit all deliverables in electronic soft copy format and all deliverables must be compatible with AF-standard desktop applications.

1.8.2. Dryer Vent Cleaning/Maintenance Log. The contractor shall complete the Dryer Vent
Cleaning/Maintenance log attached as Appendix 3, and submit the Dryer Vent
Cleaning/Maintenance log every first business day following the end of the quarter and submitted to the COR. The location and the type of dryer vents are listed in Appendix 2, which shall be maintained by the contractor.

SECTION 2
2.0 SERVICE SUMMARY (SS). The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance thresholds briefly describe the minimum acceptable levels of service required for each performance objective. These thresholds are critical to mission success and constitute acceptable (satisfactory) performance. Procedures set forth in FAR 52.212-4(a), Contract Terms and Conditions – Commercial Items, “Inspection/Acceptance” will be used to remedy all deficiencies.
SERVICES SUMMARY (SS)
*Note: PWS paragraph references shall be inclusive of all subparagraphs within that section.
2.1.1 Performance Assessment. Periodic validations is a performance assessment method which requires the CORs to employ a “spot check” style of evaluation based on the contractor’s schedule. Periodic validation will be conducted on both a regular and random basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted unilaterally by the
Government without notice to the contractor and without contract modification, based on performance quality trends.
2.1.2 Contractor non-conformance with contract requirements is considered a “defect”. The term
“defect” is used in reference to a service output which does not meet the standard of performance specified in the contract for that service.
2.1.3 Any unsatisfactory inspection, “defect” result, will be recorded in writing by the COR

2.1.4 COR if practical. Contractor re-performance is required no later than 48 hours after notification from COR Office.
2.1.5 Not meeting the performance threshold for any performance objective may result in a warning or letter of concern from the Contracting Officer.
2.1.6 Not meeting the performance threshold of the performance objectives listed with the
Service Summary tables for any two or more consecutive or non-consecutive months during a contract period may constitute an immediate progress meeting with the Multi-Functional Team
(MFT). All remedies shall be IAW FAR Clause 52.212-4(a), Contract Terms and Conditions-
Commercial Items, Inspection/Acceptance.
SECTION 3
QUALITY CONTROL
3.1 QUALITY CONTROL: The Government may inspect the contractor’s units at any time and direct the removal of any items not meeting industry standards. The contractor, at the contractor’s expense, shall replace items failing to meet required standards.
3.1.1. Quality Control Plan. The contractor shall submit a Quality Control Plan (QCP) covering all contract requirements identified within the PWS, including each task and service listed within the Services Summary (SS) with their proposal. The QCP shall include acceptable metrics for tracking quality trends. The contractor shall submit quarterly metrics to the COR Office electronically according to the agreed upon schedule. Metrics shall include, but are not limited to, weekly repair totals, work order totals, and unit’s totals. The contractor’s QCP shall have the capability to detect quality program problems and defects, identify root causes for quality related problems/defects, correct the root causes of detected problems/defects, and implement checks and balances to ensure quality related problems/defects do not continue to occur. The contractor shall be proactive on quality issues concerning performance identified within this contract and shall not rely on COR surveillance to detect their non-conformance with the PWS requirements.
3.1.2 The QCP shall include specifics as to the methods to be used to assure compliance with standards, all areas to be inspected on both a scheduled and unscheduled basis, frequency of inspections and the title and organizational placement of inspection personnel. Additionally, control procedures for Government Provided Facilities, consumable supplies and units shall be included.
3.1.3 The QCP shall include a description of the methods to be used for identifying and preventing defects in the quality of services performed. A description of the records to be kept to document inspections and corrective or preventative action taken shall also be included.
3.1.4 The QCP shall cover all services and include scheduled and unscheduled inspections, frequency of inspections, name of inspectors, and periodic security checks. Records of inspections and corrective or preventative actions shall be maintained and made available to government personnel until the end of the contract’s period of performance.
3.1.5 The QCP shall include a description of the methods to be used for identifying and preventing defects in the quality of services performed. A description of the records to be kept to document inspections and corrective or preventive action taken shall also be included.
3.1.6 Changing/Updating the QCP. All changes or updates to the contractor’s QCP shall be submitted to both the COR Office and the Contracting Office for approval prior to implementation. Each version of the QCP shall be dated to clearly depict which is the most recent version and ensure all parties are following the same version.
SECTION 4
GOVERNMENT FURNISHED SERVICES
4.1 GENERAL INFORMATION: Except for those items or services specifically stated in this section as Government furnished, the contractor shall furnish all materials, units and supplies required to perform this contract according to the terms and conditions in the PWS.
4.2 UTILITIES: The Government will provide electricity and water, if available, for the performance of services by the contractor for the duration period of the contract. Use of these utilities shall be approved and monitored by Government personnel and shall be conserved by the contractor. Any receptacles, vents, piping, and plumbing malfunctions will be repaired by the
Government. The contractor shall notify the COR within eight (8) hours of discovering any issues related to Government services.
4.2.1 Pest Management Services. The Government will provide insect and rodent control on all
Government provided facilities. The contractor shall notify the COR when an insect or rodent problem is detected.
4.2.2 Police and Fire Protection. The Government will provide security forces and fire protection to the extent necessary to ensure a secure and safe installation. In the case of an emergency situation, the Security Forces’ and the Fire Department’s telephone number is 911.
4.2.3 Medical Services. In the event of an injury requiring medical treatment, the contractor shall dial 911 and inform the operator that the injured individual is located on Hurlburt Field.
Ambulance transport responds from the Hurlburt Field Clinic Monday through Friday. On weekends and holidays, Okaloosa County ambulance transport will respond. The contractor shall reimburse the respective agency for these services.
SECTION 5
GENERAL INFORMATION
5.1 CONTRACT MANAGER. The contractor shall provide a contract manager who shall be responsible for the performance of work to ensure it meets the requirements of this contract. The name of this person and an alternate and/or alternates, who shall act for the contractor when the manager is absent, shall be designated in writing to the COR and the Contracing Officer within fifteen (15) days of contract award. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.
5.2 CONTRACT EMPLOYEES.
5.2.1 Contractor personnel shall present a neat appearance; and be easily recognizable while on the installation. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
5.2.2 Smoking and Eating. The contractor shall smoke and eat only in designated areas.
5.2.3 All contractor personnel shall be U.S. Citizens, and must be able to read, write, speak and understand English.
5.2.4 NORMAL HOURS OF OPERATION. The contractor shall perform required services
Monday through Friday between the hours of 0800 to 1700 CST (8:00am – 5:00pm).
5.2.5 Federal Holidays. The contractor shall not perform work during the following holidays unless the dates and times are coordinated in advance with the Government.

* If the holiday falls on a Saturday, it is observed on the preceding Friday. If the holiday falls on a Sunday, it is observed on the following Monday.
5.3 SECURITY REQUIREMENTS. The contractor and all employees shall comply with all
Hurlburt Field security requirements imposed by the local installation commander at all times while on the premises. The contractor shall provide an up-to-date roster of contractor personnel, by employee name and position title, authorized to work on the installation to the Contracting
Officer before contract start date and monthly if changes occur for base access/passes.
5.3.1 The contractor personnel shall NOT operate vehicles on the flight line.
5.3.2 Place of Performance. The Contractor shall perform the required support at the facilities listed in Appendix 1 on Hurlburt Field, FL. Contractor personnel shall comply with all installation and facility safety and security regulations, as well as, the health, safety, and security provisions of the contract. Contractor personnel shall report security or safety problems to the Contracting
Officer and/or Security Police as appropriate.
5.3.3 Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle Passes:
The contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations and other requirements stated herein. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.
5.3.4 When reporting to the registration office, the authorized contractor individual(s) shall provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. Hurlburt Field adheres to the guidelines of the REAL ID Act outlined in Air Force
Guidance Memorandum to AFMAN 31-113.
5.3.5 Driver’s Licenses from the following non-compliant states are unauthorized IAW the REAL
ID Act will not be accepted for entry into Hurlburt Field:
- Maine
- Montana
- Minnesota
- Missouri
5.3.6 Current Identification which may be used in place of an unauthorized State Driver's License are the following:
- US Passport
- Foreign Passport
- Veterans Identification Card
- Permanent Resident card
- Federal personal identification Cards (Contractor ID)
5.4 CONTRACTOR EMPLOYEE LIST. This form shall list every employee requiring base access. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee
List to the Visitor Control Center.
5.4.1 The contractor shall submit the employee listing the Contracting Officer no less than 3 days prior to the performance start date and at least one day prior as changes occur. Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access.
Contractor employees shall be required to provide two (2) forms of identification.
5.4.2 If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate
Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.
5.4.3 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
5.4.4 Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.
5.5 PROTECTION OF REAL PROPERTY: The contractor shall protect all government property, including grounds, resulting from his/her operations. The contractor shall immediately report any damage to government property to the COR or CO. The contractor shall repair or replace all damage to real property, including grounds, as approved by the CO or COR at no additional expense to the Government. All damage shall be repaired within five (5) workdays by the contractor with materials of like type, quality, and finish by skilled craftsmen of the trades involved.
5.6 MANPOWER REPORTING:
5.6.1 Reporting Full-Time-Equivalent (FTE) Data: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields at the Contractor Manpower Reporting Application (CMRA) site http://www.ecmra.mil (select the US Air Force logo).
5.6.2 Reporting Period: Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.
While inputs may be reported any time during the FY, all data shall be reported no later than 31
October of each calendar year. Contractors may direct questions to the CMRA help desk.
5.6.3. Uses and Safeguarding of information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
