1. GENERAL DESCRIPTION OF WORK:

The objective of this Statement of Work (SOW) is for the design, procurement and installation/renovation of the exterior of Building 1375 on Joint Base San Antonio – Fort Sam Houston (JBSA-FSH) in San Antonio, Texas, including concrete replacement, emergency egress stairs replacement, cleaning and repair of the exterior walls, and tree trimming.

2. BACKGROUND:

Building 1375 located on Joint Base San Antonio-Fort Sam Houston was constructed in 1972 as an Army Barracks, and has been renovated into a training facility. The exterior brick and EIFS wall surfaces exhibit moderate surface wear. There are several visual cracks and voids.

The facade requires surface cleaning and repair to maintain the integrity and water-tightness of the building’s envelope. The concrete walkways surrounding the perimeter of the building exhibit varied levels of surface degradation and differential settlement. Notable areas are located along the classroom and secondary entries around the building. The exterior metal stairways are aged and there are varying degrees of rust throughout the steps and landings at each of the stairways. The purpose of the exterior metal stairways is to serve as emergency egress.

3. DETAILED SCOPE OF WORK:

A. General:

1) Execution of work shall be in accordance with the applicable documents and any

Installation requirements.

2) The Contractor shall provide all engineering, drafting, plant, supervision, labor, material, equipment, incidentals, and transportation necessary to perform a 100% design in the form of final construction plans and technical specifications and installation of the project. A Lead Based Paint survey was conducted and a report documenting the results is included in Appendix C.

3) The design shall be prepared and submitted as described in the Fort Sam Houston

Installation Design Guide.

4) All design and construction accomplished pursuant to this Contract shall comply with the latest edition of the following documents (including but not limited to) unless the Contractor is directed otherwise by the Contracting Officer. Any exceptions must be approved in writing by the Contracting Officer prior to construction start-up:

5) Any individual performing technical services shall have training certifications on the required systems/components to be installed. All Contractor personnel shall be U.S.

citizens and speak fluent English.

6) Contractor shall follow the latest version of the USACE EM 385-1-1, 15 September 2008 and OSHA’s Safety Standards 29 CFR Part 1926 and 29 CFR Part 1910 for all safety requirements and regulations, included in the required Fire Prevention Plan

IAW NFPA 241.

7) The Contractor shall make no public disclosures of work being performed under this

Contract.

8) All video, still photos, and digital photos taken during the execution of this Contract shall be turned over to the Government with their associated negatives and files at the completion of this project. Digital photographs shall be submitted to the Government on compact discs in Joint Photographic Experts Group (JPEG) format, and the Contractor shall destroy all associated files from their records.

9) The Contractor shall appoint a Project Manager to serve as a single point of contact and liaison with the JBSA-CES PM for all services required under this Contract. The Project Manager will be responsible for the complete coordination of all work developed under this Contract. The Contractor shall provide and maintain an effective quality control program that will assure that all services, designs, drawings, specifications, cost estimates, and design analysis required by this Contract are performed and provided in a manner that meets professional architectural and engineering quality standards.

10) All deliverables shall be marked “For Official Use Only,” and protected accordingly.

11) The Contractor shall submit 95% and 100% design packages for Government comment/approval through the Contracting Office. Refer to paragraph 9.

DELIVERABLES. The Contractor shall follow specifications Section 01 33 16, Design after Award in completing the design documents and coordinate and attend design review meetings per design submission, or as required during the design phase of this project.

12) The Contractor shall be responsible for developing the construction documents/drawings in AutoCAD format and to also provide hard copies. All drawings must conform to the Tri-Services A/E/C CAD Standard Release 5.0

and in compliance with the JBSA-Special Project Procedures for CAD listed in the specifications.

13) The Contractor shall identify all necessary permits and conduct field verification surveys prior to the design. The Contractor shall be responsible for all required licenses to perform design work on a Federal installation, be properly bonded and insured.

14) The Contractor shall be responsible for obtaining hot work permits, if required, from the 502nd Civil Engineering Squadron Fire Department for any hot work required for this project.

15) Contractor shall obtain an approved digging permit from 502d Civil Engineering Squadron, Work Reception prior to any excavation. The Contractor must submit the 100% design drawings to obtain digging permits. Utility locations have been compiled from various sources and require verification by the Contractor to establish the exact location and depth. The Contractor shall be responsible for the damage of the existing above and below ground utilities including those marked during digging permit. Exercise extreme caution during excavation as the actual location may vary on either side of the marking.

B. Civil

1) The design and installation shall include, but may not be limited to:

a) Replace the concrete walkways surrounding the perimeter of the building and leading to the building as shown in the as-built drawings in Appendix N. The new concrete walkways shall be leveled with adjacent concrete walkways, building levels or step elevations to eliminate the tripping hazard caused by the difference in elevation. The method of repair shall include:

-Saw cut, remove, and replace existing pavement, and -Replace and compact base material.

In addition, the contractor shall verify any step off or step up dimension meets standard architectural distances of 7 ½ inches or less or design and install a new step in order to comply with that standard. Appendix N includes as-built drawings showing the specific areas to be replaced and additional sheets for reference.

The replacement of concrete shall improve areas of ponding water along the perimeter of the building and promote positive drainage.

b) Clean the exterior walls and remove any algae and mold growth. Reseal any gaps in the brickwork expansion joints. Some sections of the exterior walls currently have bird netting. Reinstate bird netting to the original setup when cleaning of the walls and reseal of the brickwork is completed.

c) Demolish and replace the nine (9) exterior metal stairs and the concrete landing located at the end of the six pods and mechanical rooms. The Contractor shall design the new exterior, shop fabricated, metal stairs to be slip resistance and meet structural, health, life safety, and architectural codes. Appendix N includes as-built drawings showing the location of the stairs and typical details. Some sections of the exterior walls, adjacent to the exterior stairs, currently have bird netting. Reinstate bird netting to the original setup when replacement of the exterior stairs is completed.

NFPA 101 Life Safety Code Ch 39.2.4 does not allow the use of only one means of egress. Exterior stairs demolition must be coordinated in order to stop the use of classrooms for which second means of egress will not be accessible. The Contractor shall coordinate demolition and installation of the stairwells with the users. The users will rotate the students out of the classrooms that will be affected by the construction. Only one stairwell may be available at a single time for replacement, coordination with the users is required. Additional time may be granted depending on availability of replacement.

4. SUMMARY OF REQUIREMENTS:

A. Site Survey. Contractor shall perform a site survey, and prepare and submit the required deliverables, and perform the procurement and installation of the materials for this facility. The Site Survey is to determine the current conditions which will impact the design of the exterior repairs. Stakeholder representatives to be involved in the site survey are Civil Engineering Squadron (CES), User, and/or equivalent Installation representatives. Contractor shall coordinate with all stakeholders, gather the necessary site data, analyze the facility, perform any required testing of existing equipment, and identify any unique building features, characteristics, or operational limitations. At least ten (10) calendar days prior to performing the survey, Contractor shall provide (via e-mail) to the 502CONS and the Installation representative the names of all Contractor personnel who will be part of the site survey team and a list of any additional data needed.

B. Procure and Install. Contractor shall procure and install products as approved by the

502CONS and 502CES, including the preparation and submittal of all technical data packages and other deliverables.

C. Meetings. Kick-off Meetings. Contractor shall attend onsite kick-off meetings with the

502CONS, 502CES and User representative to insure all requirements are met, all aspects of the work to be done are understood, and work can proceed without issue. Kick-off meetings are required for the site survey and installation mobilization. The Contractor shall take meeting minutes and provide to the 502CONS via email within three (3) working days.

D. Security Requirements. The Contractor personnel performing work on this Installation, and preparing or with access to as-built drawings, shall have as a minimum a National

Agency Check w/Local Checks and Credit (NACLC) investigation or have been properly vetted through the JBSA Security Forces Squadron (SFS) for execution of all applicable security background checks. The Contractor shall provide the 502CONS and Installation Representative with security clearance and/or other information necessary for entry to facilities for employees performing work at least ten (10) workdays prior to their reporting to the work site.

1) The Unit Security Monitor should initiate a DD Form 254, DoD Contract Security

Classification Specification, if required.

2) The AIR FORCE ANTITERRORISM (AT) program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. In accordance with AFI 10-245 paragraphs 2.18 and paragraph 2.25, DOD contractors and sub-contractor personnel shall be offered Level 1 - AT Awareness Training. AT Awareness Training may be accomplished by either a Level I - qualified instructor, or by completing the "Force Protection" computer based training (CBT) course on the Advanced Distributed Learning System (ADLS) https://golearn.csd.disa.mil/kc/login/login.asp or at https://jkodirect.jten.mil for individuals without access to NIPR, possessing a CAC card, nor enrolled in DEERS. Compliance with the training will be monitored by the contracting officer representative.

E. Progress Reports. Contractor shall provide progress reports every two weeks to the

502CONS, beginning with the installation mobilization. These progress reports shall be submitted via e-mail. Progress reports shall include Contract number, project description, work accomplished previous week, work to be accomplished within the next week, problems or unresolved issues that affect ability of the Contractor to perform, whether Contractor is on schedule and percentage complete.

F. Safety. The requirements of the Contract shall be followed during its execution.

1) Contractors and their employees shall provide a safe and healthful workplace for employees while performing Contract work. Contractor accidents involving injury to Government personnel or damage to Government property shall be reported to the Contracting Officer, the 502 ABW Ground Safety Office at (210) 652-1842.

Contractor and their employees are subject to no-notice inspections by the Occupational Safety and Health Act (OSHA) Programs and Department of Labor inspectors.

Contractor and their employees shall comply with applicable guidance set forth by law.

2) Prior to beginning work, the contractor must provide to the Contracting Officer an inventory and Safety Data Sheets (SDS) of all chemical products they will be bringing onto the installation and using for this project. These requirements are outlined in Air Force Instruction (AFI) 32-7086. This includes, but is not limited to, finishes, lubricants, JBSF110839 – SOW sealants, putty/patching compounds, plaster, paints, compressed gases, and/or cleaning supplies. The contractor must also report monthly usage data to the 502 CES.

Upon completion of the project, the contractor must remove any unused/partially used chemical products. The contractor is prohibited from leaving chemicals/hazardous materials on the installation upon completion of the project.

G. Information Assurance (IA) Requirements. Applicable IA requirements shall be identified by the Contactor during the site survey and vetted through the submittal review process. The Contractor shall comply with IA as it applies to each site and system.

H. Site/User Requirements.

1) Coordination of Work. The Contractor shall coordinate all work times with the

Installation Representatives. All work shall be performed during normal work days and hours, Monday thru Friday 0700 – 1700, excluding Federal holidays.

2) Installation Exercises. JBSA conducts regular exercises to ensure readiness in the event there is a terrorist attack or natural disaster. Exercises may last up to four (4) hours at a time. Contractor shall be prepared to participate in exercises if required.

Participation may include stopping work, denied access to the base, moving vehicles away from buildings or participating in stringent identification and/or vehicle inspection. Contractor shall participate as required at no additional cost to the Government.

3) Protection of Government Property. The Contractor shall perform an inspection of the buildings and grounds with the Installation Representative prior to starting work. The Contractor shall protect all items from damage during both demolition and construction. The Contractor shall take precaution against damage to the building, grounds, and other furnishings. The Contractor and the Installation Representative shall perform a final inspection of the buildings and grounds after completing work.

A corrective action plan shall be provided by the Contractor that details type, source and schedule of repairs, if required. A copy of the action plan shall be provided to the Installation Representative and the Contracting Officer within 24 hours of damage assessment.

The Contractor shall, at their expense, repair or replace any items, building components, or grounds damaged due to negligence and/or direct action by the Contractor or subcontractors using materials of the same quality, size, texture, and color to match adjacent existing work. All repairs beyond simple surface cleaning require approval by the Installation Representative before the Contractor performs any significant repair work. The Contractor shall insure that all repairs are performed by licensed or certified workers, as required, to maintain building codes and workmanship warranties.

The Contractor shall be responsible for security of the area in which work is being performed, during and up to completion and transfer to the Government. The

Contractor shall maintain accountability and control of any keys provided, and shall return them to the installation representative upon completion of work.

4) Digging and Trenching. The Contractor shall be responsible for obtaining all permits and coordinating with the Installation Representatives, Environmental, and Civil Engineering (CE) organizations for determination of potential underground installation interference prior to the planned start of the trenching/underground work.

Appropriate Installation personnel will ensure the location of underground utilities are identified and marked. Contractor shall have written authorization prior to performing any digging or trenching. All buried utility installation work shall be documented by means of a photo catalogue. Each run shall be photographed at regular intervals showing the depth of the trench, the pipe laid, and depth of the tracer tape. The catalogue shall be maintained on site by the Contractor and made available upon request.

5) Work Area Requirements. Contractor shall clean up all debris and discarded materials resulting from their operations at the end of each workday. The Contractor will be responsible for the removal and disposal of all trash and residual materials connected with packaging of items installed under this Contract. Government inspection and acceptance at the end of the project with the Installation Representative will not occur ‘until all trash connected with this requirement is removed. The Contractor shall insure that recyclable cardboard products shall be properly disposed of in accordance with federal and Installation regulations. All Packing materials shall be removed from the building(s)/site within 24 hours or upon completion of the installation, whichever is sooner.

5. NOTED CONTRACTOR REQUIREMENTS:

A. Prior to Contract Award:

1) Attend a Pre-Bid site visit to verify measurements, quantities and investigation of existing site conditions.

B. After Contract Award:

1) Job Specific Safety Plan: The Contractor shall submit a safety plan and job hazard analysis for the project to comply with OSHA and US Army Corp of Engineers standards prior to start of work.

2) Fire Safety Plan: Due before start of work.

3) Construction Schedule: Due before start of work.

4) Debris and Cleanup: The Contractor shall keep the work area clear of any construction debris at all times. Upon completion of the workday, the Contractor shall leave the work area in a clean manner. All project generated debris and waste shall be

removed from the site and disposed of at an appropriate location off post, daily.

C. After Contract Award, Prior to Final Payment:

1) Operation and Maintenance Manuals

2) Training

3) List of Standard Equipment and Service Organization

4) Warranty Package

5) System Equipment Testing/Balancing Plan

D. Toilet Facilities:

1) Are NOT available.

6. DESIGN and CONSTRUCTION SERVICES:

A. The AE shall provide 100% design data including design analysis, construction drawing, and specifications.

B. The design for this project shall be accomplished by a registered Architect in Texas with a minimum of five (5) years of experience.

C. Contractor shall obtain all necessary permits and conduct field verification surveys prior to construction.

D. Contractor shall set up small staging area, place barricades, cones, construction fencing, construction tape and signs as needed to prevent public access into the construction areas.

E. Contractor shall take all necessary precautions to protect surrounding buildings and personal property from damage, demolition debris and paint splatters, etc.

F. Contractor shall not interfere with foot traffic in the area and ensure that mobilization activities do not interfere with the normal operations in and around the construction site.

G. Contractor shall use personal protection equipment during the duration of this project.

H. Contractor shall remove or replace at Contractor’s expense any items installed or demolished not agreed to within this Contract or without prior approval from the Contracting Officer or appointed representative.

I. Electrical work shall be done by a qualified and state licensed electrician. Lock out and Tag out procedures shall be followed.

J. The Contractor shall submit a drawing to the government for review and approval with the proposed location of Contractor yard or lay down area. It will be the Contractor’s responsibility to secure and protect this area during construction.

At a minimum, the Contractor shall be responsible for the editing these technical specifications to meet the project scope specific requirements. The Contractor shall add any other specifications required for this project but not listed in this section.

9. DELIVERABLES: (As defined in paragraph 3.A.11).

Deliverables shall be submitted and distributed as shown in the table below. The list shows major milestones and is not exclusive of all other submittals as necessary per Contract. All of the contract deliverables and supporting documentation becomes the property of the government upon delivery and shall remain property of the government at the end of the contract period.

95% Submittal: 30 Calendar Days after NTP

100% Submittal: 105 Calendar Days after NTP

Construction: 315 Calendar Days after NTP

10. COORDINATION REQUIREMENTS:

Work with 502CES, 502CON, and users to coordinate a plan to minimize down time and coordinate weekend work if required.

11. SCHEDULE and PERIOD OF PERFORMANCE:

A. Period of Performance. The Period of Performance will be stipulated in the CLIN.

Period of performance may be adjusted by Contracting Officer if it is justified by the Contractor prior to award.

Performance period for this Contract is in calendar days from the Notice to Proceed (NTP). Liquidated damages will be assessed as provided in the Contract for failure to complete the work within the specified time.
