STATEMENT OF WORK (SOW)
FOR
Brook Army Medical Center (BAMC)
Prefabricated Storage Units
30 May 2023
1. GENERAL INFORMATION. The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to remove three (3) existing prefabricated storage units and to deliver and install two (2) prefabricated storage units to be utilized to store radioactive and hazardous waste for Brook Army Medical Center (BAMC) The delivery and installation shall be performed in accordance with all applicable federal, state, and local regulations.
2. DESCRIPTION OF REQUIREMENT. The Government shall make existing units available and free of all clutter, debris, rubbish, and mission related items along with the relevant statement of non-contamination/radioactivity before work commences. The contractor shall ready old storage units for removal (disconnect fire suppression system, disconnect power, and remove connections, remove overhanging AC, lights, or other components for transport, unbolt and separate modular units, and remove anchors). The contractor shall remove and dispose of the existing units (prepare a site-specific lift plan, provide receipt of disposal/turn-in from DLA, and clean and prepare concrete pads for new units). The contractor shall follow the guidance at 32 CFR 273.7 Excess DoD property and scrap disposal processing for disposal. The Contractor shall schedule a turn-in with Defense Logistics Agency (DLA) Disposition Services San Antonio at https://www.dla.mil/Disposition-Services/Find- Location/San-Antonio/.
The contractor shall furnish and install two new prefabricated storage units (install, level, and anchor in place with stainless steel anchors). The prefabricated storage units must have the following salient characteristics (physical, functional and performance requirements as outlined under Appendix B.
2.1 ELECTRICAL. The contractor shall coordinate the electrical requirements with the Government. The contractor shall provide detailed electrical requirements specifications on each prefabricated storage unit current demands and wire gauge. The contractor shall install all electrical components within the prefabricated storage units in accordance with the latest edition of NFPA 70, NFPA101, National Electrical Safety Code, and UFC 3-520-01 Interior Electrical Systems with Change 3. The references can be found on the Whole Building Design Guide Website, www.wbdg.org. The Government will make the connection to the new units.
2.2 FIRE SUPPRESSION. The Contractor shall coordinate the fire suppression installation with the Government. The Contractor shall engineer, furnish, install, and test a self-contained suitable Dry Chemical/Powder based, self-triggering/fusible fire suppression system within the new, prefabricated storage units, which must meet all applicable standards, including NFPA 13, 72, 70, UFC-3-600-01, UBC and AF Standards. The Contractor shall provide a test plan as to how the system shall be pre-tested and post-tested to demonstrate to the Government that the system is fully operational and meets or exceeds the specified requirements and that the system is fully ready to be connected to the existing infrastructure. These tests shall be accomplished prior to the system being connected to the existing infrastructure. The Government will make the connection to the new units.
3. LOCATION AND SITE LAYOUT. The storage units are located outside of BAMC on a concrete pad.

See Appendix A for layout of the area. Two (2) units are required on one concrete pad. The concrete pad is surrounded by a fence that will need to remain in place after installation. The concrete pad is split up into two fenced areas that need to remain in place after installation. The storage units must fit on the existing concrete pad, within the existing fences, where the current storage units are located.

4. QUALITY CONTROL. The contractor shall be responsible for quality control for all work accomplished during the performance of this project.
5. QUALITY ASSURANCE. The government shall inspect all work performed by the Contractor. Work performance shall be deemed satisfactory and acceptable to the government when the Government representative declares the project satisfactory.
6. HOURS OF OPERATION. Monday Through Friday (0700-1700)
7. HOLIDAYS OBSERVED. New Year’s Day, Martin Luther King, Jr. Birthday, Washington’s Birthday, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day.
8. SECURITY REQUIREMENTS. Person(s) not in possession of military IDs, Department of Defense civilian or contractor IDs, or previously issued and active base access passes must enter JBSA-Fort Sam Houston through the following visitor control centers: Walters Street Visitor Center: 6 a.m. to 5:30 p.m., Monday-Friday, closed holidays. BAMC/I-35: 24/7. All contractors with personnel that require base passes for unescorted entry to JBSA Fort Sam Houston must submit the following information on all personnel requiring access: Last name, first name, middle initial; Date of Birth (DD/MMM/YY); ID Type (i.e. driver’s license, etc.), ID Number and issuing State.
9. SITE MAINTENANCE. The Contractor shall protect adjacent property, offices and their contents from dust, dirt, or other materials. Work areas shall be maintained in a neat, clean, and safe condition and shall, at a minimum, be cleaned with all waste and packing materials removed at the end of each shift.
10. STORAGE UNIT REMOVAL/INSTALLATION TIMEFRAME. Once the Contractor initiates the de-installation and removal of the three (3) existing prefabricated storage units from the physical location, the Contractor shall deliver and install the new prefabricated storage unit I within 21 calendar days, and storage unit II within 3 calendar days, 72 hours. This timeframe does not include lead time for any off-site fabrication and/or testing of the units prior to removal and delivery operations.
11. RECORDS MANAGEMENT. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF records management procedures in Air Force Instruction (AFI) 33 - 322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 37-123, paragraph 7.4, Air Force Manual (AFMAN) 37-138, paragraph 2.13.
12. AIR FORCE ANTITERRORISM (AT) PROGRAM. The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. In accordance with AFI 10-245 paragraphs 2.18 and paragraph 2.25, DOD contractor and sub-contractor personnel shall be offered Level 1 – AT Awareness Training. Compliance with the training will be monitored by the contracting officer representatives.
13. SAFTEY. The Contractor shall follow the USACE EM 385-1-1 15 September 2008 and OSHA’s Safety Standards 29 CFR Part 1926 and 29 CFR Part 1910 for all safety requirements and regulations, included in the required Fire Prevention Plan IAW NFPA 241.
14. WARRANTY. The contractor shall provide any original equipment manufacturer (OEM) pass through warranty and standard commercial warranties applicable to the products being purchased at no cost. At a minimum, the contractor shall provide a one-year warranty, including parts and labor.

APPENDIX B
SALIENT CHARACTERISTICS REQUIRED
o The concrete slab area is 61’x 62’. The area where unit 1 will be located is 61’x 41’. The area where unit 2 is located is 61’x 21’.
The units shall have area dimensions that follow:
Unit I – Radioactive Decay-In-Storage o Must be no less than 922 sq. ft. of usable floor space.
o Exterior: 40’x 24’x 9’6” with Length and Width variance not exceeding ± 1’ and Height not exceeding ± 6.”
o Interior: 39’5” x 23’ x 8’10” with Length and Width variance not exceeding ± 1’ and Height not exceeding ± 6.” The interior must be completely open, no partitions.
o Must have one (1) manual roll-up garage door that is accessible by forklift or other pallet convenience devices, one (1) industrial standard service/inspection door, and one (1) fire rated 60” x 80” double leaf steel access door.
o Must have a raised weather lip/ramp, navigable by pallet conveyance devices. Any structurally sound durable material is acceptable, which can be affixed to prevent shifting/movement during use.
o The floor shall have R8.4 insulation in the floor, protected by a steel lining (should not have a sump). The steel lining must be a minimum of 1/8” thickness of A36 steel or greater, and installed both above and below the insulation.
o Must have a steel and skid proof floor made of A36 steel or greater, free from rough edges, kinks, and sharp bends. The Contractor shall be responsible to ensure that the steel floor is structurally sound and suitable for normal storage operations of medical waste.
Unit II – 90-Day Hazardous Waste o Must be no less than 392 sq. ft. of usable floor space.
o Exterior: 40’x 8’x 9’6” with Length and Width variance not exceeding ± 1’ and Height not exceeding ± 6” o Interior: 39’5”x7’8” x 8’10” with Length and Width variance not exceeding ± 1’ and Height not exceeding ± 6” o Must have four (4) fire rated 50” x 80” steel access doors.
o Must have one or more steps for OSHA compliant entry and egress IAW 29CFR 1917.120 which states that riser heights shall be from 6” – 7.5” with a width minimum of 22”, and landings shall be at least 20” in depth. Any structurally sound durable material is acceptable, which can be affixed to prevent shifting/movement during use.
o Must be a 4-bay storage unit with grating and leak proof spill-containment sump. Each bay floor shall be grated in its entirety. Each bay must be separated to form sealed compartments, with a capacity of no less than one hundred (100) gallons each, to total four hundred (400) gallons.
Both Units I and II shall:
o Have an anticipated lifespan of at least 15-20 years.
o Units need to be insulated specified to IECC standards to at least R value R-12 (mineral fiber in wall cavity).
o All devices, systems, and components shall meet/ exceed OSHA NRTL standards as applicable.
o Be outfitted with suitable electrical connections to meet the requirements of the local safety office and current supply. (Current supply: 208 Volt single phase, two hots/neutral/ground, 100-amp service). One connection to each building is required.
o Each storage unit must comply with the recommendations as outlined in the OSHA Technical Manual, Section III, Chapter 2, of maintaining an internal temperature of 68-76 degrees Fahrenheit.
o Each unit must have the capability of supplying power for items such as computers, additional lights and a freezer for waste that may contain food or bodily fluids to prevent foul odor (at least 2 outlets are required).
o Each unit must have sufficient lighting, as stated in OSHA Standard No. 1915.82, to avoid tripping hazards as well as task specific lighting for administrative duties such as hand entering information into logbooks, typing on a computer keyboard, and reading instrumentation displays.
o Each unit must be equipped with a fire suppression system, in the form of a self-contained suitable Dry Chemical/Powder based, self-triggering/fusible system, with the necessary dampers and explosion proof venting, and power alert equipment as inspected and certified by either the local safety office or the Installation Fire Chief and afforded for in NFPA 3.
o Each unit must have a fire rating in accordance with the local safety office or the Installation Fire Chief as being sufficient for the intended use.
o The units must have suitable entry and egress paths/openings that are sufficient to meet the guidelines of the local safety office or the Installation Fire Chief for the intended use.
o Each unit must be grounded in accordance with the local safety office for the intended use.
o Each unit must be able to be secured in a manner that the standard hospital master key does not work for entry. This key must not be one that would be used typically by housekeeping or other facilities personnel as this is a restricted area.
o Each unit will have the interior and exterior surfaces protected with high solid alkyd universal metal primer and high solid acrylic alkyd enamel topcoat providing proven resistance to exterior abrasion, corrosion, UV resistance and exceptional durability.
o Signage Required: Permanent D.O.T. metal flip placard with rust proof aluminum holder and stainless-steel clips on each building. One (1) pressure sensitive NFPA 704 Hazard rating sign for Unit I and, and one (1) on each door to each compartment for a total of four (4) for Unit II. This is an overall total of five (five) signs.
o The floor load of the facilities must be 250 pounds per square foot (psf) or greater.
o The snow load of the facilities must be 5 psf or greater.
o The wind load of the facilities must be 115 MPH or greater.
o Welding procedures must be in accordance with American Welding Society standards and meet the standards of ASTM. All welds must be sufficient in size and shape to accommodate wall designs and loads.
o Roof must meet or exceed UL 263 and ASTM-E119. Roof must be sloped to facilitate runoff and doors(s) must be equipped with rain shields on exterior.
