STATEMENT OF WORK
Install BeaverFit Turf Flooring METC Fitness Center Exterior Concrete Slab Joint Base San Antonio – Fort Sam Houston
1. PROJECT SCOPE: The contractor shall furnish all supervision, labor, transportation materials, tools, incidentals, equipment, and quality assurance to install a 1,480 sq ft of artificial turf flooring on top of an existing exterior concrete slab. The slab is located in front of Bldg 1369, METC FC.
2. PROJECT DESCRIPTION: The requirements of this project are but not limited to the following: The immediate perimeter surrounding the slab will be cordoned off to prevent patron traffic during the install. The Contractor shall provide barriers and signs to prevent unauthorized personnel from entering the work area while the contractors are installing the flooring.

4. GENERAL: All required tools, parts, materials and supplies shall be supplied by the Contractor and should be on hand before beginning work. The Contractor shall furnish and maintain adequate barricades, and warning signs on the job site.
5. HOURS OF WORK: Normal duty hours are from 5:00 a.m. to 10:00 p.m. Monday thru Friday, excluding weekends and observed Federal holidays and AETC observed family days. Contractor should coordinate work with the Government Representative.
6. PERIOD OF PERFORMANCE: Project should be completed within 24 hours of installer arrival at the work site.
7. CONTRACTOR QUALIFICATIONS: The Contractor shall be bonded, licensed, and properly insured and shall provide proof to the Government upon request.

9. EXISTING FLOOR REMOVAL: NONE
10. INSTALLATION: Slab prep consists of applying adhesive to the concrete surface immediately prior to laying the turf floor.
INSTALL: 1,480 sq. ft. of artificial turf flooring. This flooring shall be rolled out in place and placed on top of the concrete slab already prepared with adhesive glue. The adhesive used will be a mild polyurethane rubber flooring glue recommended by the manufacturer. Proper spread of adhesive will be achieved by use of recommended notch trowel.
11. CLEAN-UP: At the end of the work day, the Contractor shall clean up the work site by removing of all construction debris. Clean up of the work site shall include the construction area(s), stage area(s), parking and eating area(s) and any other area(s), affected by the construction process. All Contractor generated refuse and waste shall be stored in a waste container provided by the Government. Refuse disposal shall comply with the current disposal environmental requirements. The construction site shall be kept neat, orderly and safe for workmen at all times.
12. GOVERNMENT FURNISHED SERVICES: The government will provide utilities (water and electricity), emergency, fire, and security police response if needed.
13. TRAFFIC REGULATIONS: Contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. Contractor’s personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hands-free phones, is prohibited while driving on military installations in San Antonio, TX.
14. CONTRACTOR’S WARRANTY: The contractor shall guarantee all materials and workmanship for a period of 10 years from the date of project completion and shall within that period respond to all defects in materials/workmanship within seventy two (72) hours of being notified. Correction of all defects under this warranty shall be at the expense of the contractor.
15. INSPECTION AND ACCEPTANCE: A representative from the FSH Fitness and Sports staff will have oversight on this project and conduct a final inspection of completed work All discrepancies identified during inspections shall be corrected prior to final payment.
