STATEMENT OF WORK
Replace Well #1 (Bldg. 2194) and Well #5 (Bldg. 3186)
JBSA-FT. SAM HOUSTON, SAN ANTONIO, TX
PROJECT INFORMATION: Applicable provisions of the contract clauses will govern work under each section of the specifications.
PROJECT SCOPE Well #1 Above Ground Pump: The contractor shall furnish all supervision, labor, transportation, materials, tools, incidentals, equipment, personnel and other services necessary to replace above ground pump. Contractor shall remove old steel piping in well #1, Diameter -12 inches and replace with new steel pipe 320 feet with 7 stages along with 150 horsepower motor, speed-1770 RPM Phase-3 Voltage-460V. The pump will be tested to ensure proper operation before acceptance. Contractor shall disconnect and connect all the piping and 7 stage and all the plumbing with electrical. In addition, all services shall be performed in accordance with the latest version of the Uniform Building Code, International Building Code (IBC), Federal, State, local building codes and Texas Administrative Code (Title 30, Part 1, Chapter 334, Subchapter C, Rule 334.49).
PROJECT SCOPE Well #5 Submersible Pump: The contractor shall furnish all supervision, labor, transportation, materials, tools, incidentals, equipment and personnel and other services necessary to replace submersible pump. Contractor shall remove old steel piping in well # 5 Diameter-14 inches and replace with new steel pipe 272 feet with 4 stage along with 200 horsepower motor, speed-1750 RPM Phase-3 Voltage-460V. The pump will be tested to ensure proper operation before acceptance. Contractor shall disconnect and connect all piping, submersible 4 stage and all the electrical. Contractor shall install all new pump with pipe and electrical.
PROJECT DESCRIPTION: The service includes, but is not limited to items identified below: Contractor shall comply with all specifications to the National Building Code and the State Plumbers code of Texas. The contractor shall coordinate with facility manager and 502d CES Project Manager prior to work commencing. This coordination shall be done during normal duty hours, Monday through Friday 7:30 a.m. to 4:30 p.m. All required tools, parts, materials and supplies should be on hand before commencement of work.
DEMOLITION AND INSTALL (Well #1 and Well #5)
Facility 2194 – Well #1 Above Ground Pump
Findings:
a) Remove the motor (driver) Model- 5k6277XH171A, Serial # CDJ313113
b) Remove all the piping (12 foot by 320 foot), all wire, 7 Stage and Mud the chambers for no water return.
• Install a new motor, 150 horsepower speed-1770 RPM
• Replace new 7 Stage pump with 320 feet of piping.
• Replace all wire going to the stages
• Mark/Remark the new well with the new plates.
Contractor must provide warranty information once installed work has been completed to the 502 CES Project Manager.
Facility 3186, Well #5 Submersible Pump
Findings:
a) Remove the motor (driver) Model- V14-100-4, Serial # 83922
b) Remove all the piping (14 inch by 272 feet), all wire, 4 Stage and Mud the chambers for no water return.
• Install a new motor the same as the one at is out 200 Horsepower speed-1750 RPM
• Replace new 4 Stage pump with all piping of 272 feet
• Replace all wire going to the stages
• Mark/Remark the new well with the new plates.
Provide warranty for installed work completed.
GENERAL INFORMATION:
GOVERNMENT FURNISHED SERVICES: The Government shall furnish access to electrical power, water, rest room facilities, emergency, and fire and security police response within the project area. Contractor shall coordinate with 502d CES prior to the start of work to ensure coordination of above items.
POLICE PROTECTION: The 502d Security Forces Squadron shall provide security and law enforcement response and services to the contractor.
FIRE PROTECTION: The Lackland Fire Departments shall provide initial response, EMS, and fire protection services to the Contractor.
TRAFFIC REGULATIONS: Contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. Contractor's personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hands-free phones, is prohibited while driving on military installations in JBSA – Fort Sam Houston.
HAZARDOUS AND/OR TOXIC MATERIALS: A hazardous material (HAZMAT) is one that poses a risk to the health and safety of the public, and/or the environment if not properly controlled during the handling, storage, manufacture, processing, packaging, use, disposal, or transportation. Hazardous materials may be elements or compounds, and can be found as gases, solids, or liquids. Hazardous chemical are defined under Sec. 1910.1200(c) of Title 29 of the Code of Federal Regulation. Contractor shall identify and report HAZMAT usage to the HAZMAT Pharmacy according to local procedures. All HAZMAT brought to the job site shall require the contractor to fill out an AF Form 3952 for each item for submission to the Hazardous Material Pharmacy. Additionally, the contractor is required to submit Material Safety Data Sheets (MSDS) as directed by the clause entitled Hazardous Material Identification and Material Safety Data (FAR 52.223-3). The Contractor shall submit an actual usage report of all hazardous/toxic materials brought onto the base during the preceding month to the HAZMAT Pharmacy (IAW AFI 32-7086, Hazardous Material Management). The phone for the JBSA Lackland AFB Hazardous Material Pharmacy is 671- 5389/0332 for more information.
CONTRACTOR QUALIFICATIONS: The contractor shall be properly insured and shall provide proof to the Government upon request.
SITE CONDITIONS: Upon site visitation, existing conditions of the area will be field verified by the contractor with members of 502d Contracting Officer and 502d CES. The Contractor shall furnish and maintain adequate barricades, and warning signs, on the job site during demolition and installation.
CODES AND REGULATIONS: All work shall be accomplished in accordance with the Civil Engineering, Contract Specifications, applicable Uniform Building Codes, Joint Base San Antonio-Fort Sam Houston requirements, and OSHA requirements.
HOURS OF WORK: Normal duty hours are from 7:30am to 4:30pm, Monday through Friday, except on government holidays. Contractor should coordinate phases of work with 502d CES/CEOER/RP.
CLEAN-UP: Contractor shall clean up the job-site daily during the progress of work.
Contractor shall collect all debris and excess material and dispose of it in a contractor's dumpster. All contractor generated refuse and waste shall be hauled off and disposed of at an off base landfill site by the contractor at no expense to the government. Site shall be completely cleaned and restored to initial condition prior to final acceptance. Refuse disposal shall comply with the current disposal environmental requirements. The service site shall be kept neat, orderly and safe for workmen at all times. On-site dumpsters, if furnished by the Contractor, shall be coordinated with the Government inspector.
