STATEMENT OF WORK

FOR

MODULAR CATH LAB

AT

KEESLER AFB, MISSISSIPPI

26 JULY 2018

1. SCOPE:

The Contractor shall provide all services, supplies, transportation, equipment, test sets, tools, materials, labor supervision, management and other incidentals necessary to Furnish, Install and Test (FI&T) fully functional modular cath lab with the prescribed requirements for a duration Six months. While the Keesler Medical Group (MDG) Cath Labs are under construction it is imperative for the MDG to maintain a functional Cath Lab for inpatient and outpatient operations for our DOD and VA populations. The Modular Cath Lab shall allow for adequate space for operations, maximizes services, support personnel training demand. Currently the 81 MDG Cath Lab uses the GE Innova 3100 Digital Flat Detector Cardiac/Vascular System. The Modular Cath Lab should be equipped with the same equipment.

All accessories or incidental items not specifically shown and detailed in the specifications herein, which are necessary and/or required to complete the work within the intent of the this SOW, shall be included by the Contractor without additional cost to MDG

REQUIREMENTS:

1.1 MODULAR CATH LAB & CATH LAB SYSTEM REQUIREMENTS:

1.1.1 GE Innova 3100 Digital Flat Detector Cardiac/Vascular System including Uninterruptable Power

Supply (UPS), 31fm digital flat panel detector combo cardiac/peripheral imaging system, and complete X-ray system C-arm, and table

1.1.2 8’ x 48’ Mobile Lab Coach (Trailer) with procedure room width of 15’ across.

1.1.3 Hydraulic patient lift capable of carrying 1500 lbs.

1.1.4 On-Board emergency backup generator

1.1.5 Storage and Catheter Cabinetry

1.1.6 Stereo and intercom systems

1.1.7 Numerous configurable data/network jacks

1.1.8 GE Centricity CA 1000 Image Review Workstation

1.1.9 GE MacLab IT Hemodynamic monitoring system with integrated Fractional Flow Reserve (FFR) and/or Sidestream Co2 options installed

1.1.10 Medrad Mark V Provis Injector

1.1.11 Interfaces with X-ray systems to automatically upload demographics to the imaging systems and receive radiological parameters such as angulations and dose area product

1.1.12 Collect registry data at the point of care, helping to minimize post-procedure chart pulls.

1.1.13 Comprehensive data export to GE and third-party registry and data management systems

1.1.14 Lead-lined window and door to separate procedure and control rooms during x-ray use

1.1.15 On-board fire warning system and be equipped with smoke and heat detectors throughout the entire lab

1.1.16 On-board self-contained dry fire suppression system

1.1.17 Space for Pyxis system at 22.8” W, 27” D, 55” H

1.1.18 Space for Crash Cart at 47” W, 19” D, 76” H

Upon completion of all work the Contractor shall request a final inspection. The Government shall inspect all work before acceptance. All work identified during the final inspection not meeting the criteria specified by this statement of need shall be repaired or replaced by the Contractor at no additional cost to the Government.

1.2 CATH LAB PROCEDURAL CAPABILITIES

The modular Cath lab must maximize services available to our patients. The Modular Cath Lab must enable the continuation of safe patient care for the following procedures below:

1.2.1 Cardiac Cauterization

1.2.2 Peripherals/Angiography procedures

1.2.3 Cardiac Inventions

1.2.4 Peripheral Interventions

1.2.5 ST-Elevation Myocardial Infarction (STEMI)

1.2.6 Complex and High Risk Procedures

1.2.7 Implantation of devices

1.2.8 Fractional Flow Reserve (FFR) procedures

1.2.9 Chronic Total Occlusion (CTO) procedures

1.2.10 Multi-vessel Revascularization procedures

1.2.11 Utilization of advance hemodynamic support

1.3 GOVERNMENT FURNISHED EQUIPMENT

1.3.1 Network connections (i.e. Fiber Optic Infrastructure) are available to all site

1.3.2 Utility connections available at installation site

1.3.3 Subnet and workstation in advance and trailer equipment after the modular arrival

1.4 MODULAR INSTALLATION REQUIREMENTS

1.4.1 Contractor shall install temporary construction barriers to accommodate the proposed renovation

A/C power is available at installation site

1.4.2 Contractor shall core drill the walls in the adjacent electrical rooms to accommodate the proposed

200 amp electrical service from the facility Emergency Power panel to the Modular Building

1.4.3 Contractor shall core drill the walls in the adjacent electrical rooms to accommodate network connections from the facility electrical room to the Modular Building

1.4.4 Contractor shall provide labor and material to install temporary bollards to provide traffic protection for the Modular Building.

1.4.5 Contractor shall install an aluminum ramp and platform unit to accommodate the patient entrance to the proposed modular building Procedure Room.

1.4.6 Contractor shall install an aluminum stair unit to accommodate the staff entrance to the proposed modular building Control Room.

1.4.7 Contractor shall install an unenclosed aluminum canopy system to provide weather protection for the path between the facility and the proposed modular building. The trailer must remain 15’ at a minimum away from the facility.

1.4.8 Contractor shall seal wall penetrations in accordance with NEC, NFPA, and facility standards.

1.4.9 Contractor shall install a 3/4” hose bib adjacent to the new electrical connection.

1.4.10 Contractor shall install a temporary grey water storage tank for drainage from the Modular Building

Scrub sink.

1.4.11 Contractor shall provide coordination to empty the grey water storage tank weekly.

1.4.12 Contractor shall install one 200 amp breaker for 480 volt breaker in panel 1AHCDP1 or 1AHCDP2 to accommodate the proposed Modular Building.

1.4.13 Contractor shall install conduit and conductors from the distribution panel to the new disconnect with Russelstoll connector to accommodate the proposed Modular Building.

1.4.14 Contractor shall install conduit and junction boxes with pull strings from the adjacent electrical rom

1D-301 to the building exterior near the disconnect for facility installation of network and telephone connections for the proposed modular building.

1.4.15 Contractor shall install exterior lighting, connected to the existing adjacent lighting circuit to accommodate the new patient canopy to the proposed modular building.

1.4.16 Contractor shall repair damaged wall surfaces after removal of the proposed Modular Building.

1.4.17 Contractor shall repair damaged concrete drives, sidewalks, and curbs after removal of the Modular

Building.

2. GENERAL:

2.1 SCHEDULING:

Scheduling of all work shall be coordinated and approved by the Medical Center Facility Management Department (FM) and end user prior to the start of any work. The service provider will be required to perform as required to meet the schedule. Prior to performing any work all service provider personnel are to sign in at Building 468, Room 5A125.

2.2. POINTS OF CONTACT:

2.3 COMPLETION TIME:

The completion date for the installation and delivery of the mobile cath lab will be all encompassing.

2.4. WORKING CONDITIONS:

The Contractor shall take all necessary and prudent safety precautions to ensure the safety of the workforce and other exposed personnel.

2.5. BUILDING OCCUPANCY:

The building area involved will be vacant. However, adjacent rooms will be occupied. Sound and dust barriers will be required at all openings into occupied spaces and all infection control barriers and permits will be up before you start.

2.6. MAINTENANCE OF TRAFFIC FLOW:

Work performed under this contract shall be planned to minimize disruption to normal traffic flow. Partial or complete interruption to normal traffic flow shall be approved by the Contracting Officer. Barricades shall be utilized where necessary to properly route vehicular or pedestrian traffic.

2.7. UNDERGROUND/ EXISTING UTILITIES:

Protect all active utilities. Any damage to existing utility lines caused by the Contractor will be the Contractors responsibility to repair.

2.8. HAZARDOUS ELECTRICAL EXPOSURE:

Provide Safety protection and precautions for all contractor electrical exposure.

2.9 PERMITS:

Contractors will be required to obtain permits as required by Keesler in the performance of their work.

Contractors shall post or have readily available all permits before work is commenced

The following permits must be obtained anytime referenced work is required.

2.10 WORKING HOURS:

Work performed shall be during normal work hours defined as 7:00 am - 5:00 pm Mondays – Fridays.

No work shall be accomplished outside of the normal base working hours, on weekends, or on federal holidays without the prior written approval of the FM and the Contracting Officer. Approval shall be requested at least 10 days in advance of the unusual hours. However, the FM may require certain work to be performed outside of normal working hours in order to minimize disruption of base functions and activities.

2.11 MATERIAL DELIVERY AND STORAGE:

Material: The materials, products, and equipment described in the Bidding Documents establish a standard of required function, dimension, appearance, and quality to be met by any proposed substitution. Equals can be substituted provided they meet all standards of the originally specified product. The Contractor shall make every effort to submit model numbers, data sheets, drawings, and any other information required to compare products prior to submitting a bid. Additionally, any changes required to the Scope of Work (and their associated costs) due to a substitution shall be the responsibility of the Contractor and shall be documented with the submittal.

Delivery: The Contractor shall have all materials for his work delivered during normal working hours or shall have a representative present to receive shipments. The contractor should be particularly aware that all delivery vehicles must have sufficient information to locate project site.

This includes name and address of the project site and name of prime contractor and a point of contact for delivery.

Storage: The Contractor shall be responsible for the storage of all material and equipment. All items shall be properly stored to maintain their original condition until actually installed.

2.12 INTERRUPTION OF UTILITY SERVICE:

Interruptions to utility services shall be minimized. Necessary outages shall be coordinated with the FM a minimum of 3 weeks in advance of the planned outage.

2.13 TEMPORARY SANITARY FACILITIES:

If portable sanitation units are called for in this project, they shall be supplied by the contractor. This includes maintenance, transportation to and from job site, secure placement of unit as not to obstruct normal public activity, and unit clean-out as needed. The type units provided and the placement location(s) shall be approved by the FM

2.14 STORM DAMAGE:

Should warnings of winds of gale force or stronger be issued, the Contractor shall take every practical precaution to minimize danger to persons and damage to property. These precautions shall be coordinated through the FM and shall include closing all openings; removing all loose materials, tools, and equipment from exposed locations; as well as removing or securing scaffolding and other temporary work. Contractor will follow Keesler AFB requirements for Hurricane Conditions in force.

2.15 SAFETY:

The Contractor shall observe all Safety, Security, Traffic and Fire regulations presently enforced at Keesler

Air Force Base. And comply with latest issue of OSHA requirements. Contractors shall comply with the Occupational Safety and Health Act (OSHA, Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926. The detailed provisions of the act for accident prevention are directly applicable to all Contractor operations. Prime Contractors are also responsible for ensuring any sub-contractors also adhere to OSHA Guidance. OSHA compliance officers can visit Contractor worksites on the installation.

2.15.1 Contractors are responsible for ensuring all industrial safety provisions are followed.

2.15.2 Contractors must report all accidents involving Air Force people, property, or equipment damage, and any Contractor personnel injured in the performance of the Air Force contract to the Contracting Officer.

2.15.3 Take immediate corrective action to comply with safety regulations if informed of a violation by the Contracting Officer.

2.16 CONSTRUCTION MATERIALS & CLEANING:

2.16.1 Material: All material furnished by the Contractor for this job shall be new and unused material of high quality.

2.16.2 Cleaning: The Contractor shall maintain the premises, including any staging area or storage areas, free from accumulations of waste, debris, and rubbish caused by the Contractor’s work and shall minimize the spread of dust and flying particles. As work is completed, or at the end of each day, the site shall be cleaned and all waste material shall be properly disposed of.

2.17 JOBSITE MAINTENANCE AND STORAGE AREAS:

The contractor shall maintain the jobsite and staging areas in conformance with the following.

2.17.1 Lay down/staging areas shall be kept neat and free of loose debris at all times. Grass shall be cut and maintained regularly. Height and maintenance shall be consistent with normal Base standards and consistent with the immediate area.

2.17.2 Jobsites and lay down/staging areas shall be enclosed with a 6’ chain link fence with brown fabric screening. Materials and equipment may be stored inside storage units. Items stored in the lay down area shall be arranged neatly.

2.17.3 All temporary storage trailers and storage containers shall present a neat and clean appearance and shall be in a state of good repair and shall be located within the fenced area described in paragraph above.

2.17.4 If allowed by the contract and if approved by the FM Contracting Officer, an office mobile unit may be located outside of a fenced area. At a minimum, this unit shall be in a paved area and shall present a neat, professional appearance. Unit shall be of Keesler standard colors, shall have presentable skirting and access stairs. Ancillary items, such as porches and canopies, shall be neat and painted Keesler brown. Contractor shall submit a picture of the proposed unit for approval and before unit is moved to Base.

2.17.5 Execution of work may require excavation or other type of work both at and away from primary work area. These areas shall be secured and work times shall be kept to a minimum. Open excavations shall be directly in progress or shall be covered directly after work complete. Open excavations requiring extended period of inactivity shall be temporarily backfilled. In no case shall an excavation be open for more than 72 hours. Barricades, fences and other warning devices shall be maintained neatly at all times.

2.18 DELIVERABLES:

Submittals shall be provided as outlined in these specifications. At a minimum all major materials used shall be submitted for documentation which shall include the manufactures pre-printed cut sheets showing all salient qualities of the items specified. The contractor shall highlight the pertinent salient characteristics with a brightly colored transparent marker. The contractor may use his normal submittal form however, all submittal documents received that do not clearly delineate and display the information which is being submitted will be returned to the contractor. The contractor shall ensure that pertinent information showing compliance with the drawings and specifications is shown on the documents submitted.

List of Specific submittals required for this project in addition to the materials submissions:

Schedule – within two days of award Modular Layout – within two days of award Covered walkway – within two days of award

2.19 RECYCLED CONTENT AND BIO BASED PRODUCTS:

Executive Order 13514 dated 5 Oct 09, outlines the federal government’s special responsibility to lead the way in building markets for recycled goods. Green procurement is the name given by the Air Force to this buy-recycled purchasing program. The intent of the program is to stimulate recycling by providing a market for new products manufactured with bio based or recycled materials. The legal authority for the Green Procurement Program comes from the Resource Conservation and Recovery Act (RCRA) Section 6002. It requires federal agencies to give preference in their purchasing programs to products and practices conserving and protecting natural resources and the environment. If applicable, material and product submittals for all recycled-content items should list the recycled and recovered materials used and the percentage content (by weight). Submittals for bio-based products should describe the bio-based materials used and the percentage content (by weight). This data is required to be submitted by the subcontractor on an AF Form 3000 (Material Approval Submittal) to the Contracting Officer upon completion of performance (construction).

2.20 ENVIRONMENTAL PROTECTION:

Contractor personnel shall, at all times, perform all work and take such steps required to prevent any interference or disturbances to the ecological balance of the environment. All work must be performed in accordance with applicable Federal, State, Local, and Air Force environmental regulations. Use good management practices to protect air, water, land and wildlife and to prevent noise, solid waste, radiant energy, dust and radioactive pollutants. In the event of a chemical or hazardous material spill, the contractor must immediately notify the Keesler Fire Department at 911 (identify that you are located on Keesler AFB) and the FM environmental representative at 228-377-1262.

2.21 WASTE MATERIALS:

Unless otherwise specified, all waste materials, excess dirt and rubble, empty containers, etc., shall be disposed of at an approved off-base location in accordance with all applicable Federal, State, County and Municipal laws.

2.21.1 Solid Waste Management:

Designate individuals on the contractor’s staff who are responsible for Construction and Demolition (C&D) waste prevention and management.

Develop procedures for collecting and storing C&D waste on project sites, including designating locations for waste containers, sorting or separating methods, handling and transporting of wastes, special handling requirements, and scheduling for waste and recycling collection.

Develop descriptions of how the C&D materials will be recycled or reissued. The information should include any on-site storage and processing areas and a description of the processing and equipment.

Records shall be maintained for all C&D projects to document the quantity of waste generated;

the quantity of waste diverted through sale, reuse, or recycling; and the quantity of waste disposed by land filling or by incineration.

Contractors shall provide these records to the Contracting Officer in accordance with contract requirements.

In accordance with Executive Order 13514, Keesler AFB will divert 60% of all C&D waste.

Manage solid waste in accordance with the Keesler AFB Integrated Solid Waste Management Plan.

2.22 HAZARDOUS MATERIALS:

In the event that hazardous materials will be used in the execution of this contract, the contractor will be required to provide a list of materials, quantities, and Safety Data Sheets (SDS) to the Contracting Officer before work begins. The hazardous materials must be approved through the Keesler Hazardous Materials Management Program prior to being brought onto Keesler AFB. This approval process may take up to two weeks, which should be considered in the performance period of this contract. Unless otherwise specified, all unused hazardous materials should be removed from the base at the end of the project.

2.23 SECURITY REQUIREMENTS:

2.23.1 Contractor Notification Responsibilities: The Contractor shall provide a written notification to the FM Business Management Office Contracting Officer who will establish coordination with the applicable FM Sub-Contractor Technical Monitor. The notification shall include:

• The names and contact information (address, phone number, and email) of a minimum of two

Contractor representatives (preferably the contracting agent and the project supervisor) that will be responsible for this project and have 24 hour contact availability to answer any questions or address any issues that may arise that are related to this project.

• Length of time for the contract and/or duration that the employee will be performing work

• Full name of Contractor employees assigned to work this project and their employment status

(Full-time/Part-Time/Temporary) of employee with the Contractor.

• The last six digits of the Social Security number for each employee assigned to work this project.

The above information will immediately be updated by the Contractor as changes occur during the duration of this contract and provided to the FM

2.23.2 Base Pass and Identification Items: The Contractor shall ensure the following pass and identification items required for the contract performance period are obtained for employees and their use of non-government owned vehicles:

• For vehicle operators: 1] Current driver license for each employee in support of work on KAFB

2] Vehicle Registration and Proof of Insurance

• For non-vehicle operators:

Employee identification: Current driver license, official ID card, or passport

2.23.3 Security Reporting Requirements: The Contractor shall comply with AFI 71-101, Volume-1

Criminal Investigations, and Volume-2 Protective Service Matters, requirements. Employed personnel shall report to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, their employees, resources, and classified or unclassified defense information. Each employee shall be briefed by their supervisor upon initial on-base assignment and provided updates as required.

2.23.4 Anti-Terrorism Training. Level I Anti-Terrorism (AT) Awareness Training is available upon request and will be conducted by the Installation Anti-Terrorism Officer (ATO). Contact the 81st Security Forces Squadron (81 SFS/CCA), at 228-376-6601.

2.23.5 Traffic Laws: All Contractor employees shall comply with all federal, state, local and base traffic regulations while on KAFB properties.

2.23.6 Contractor Employees: The Contractor shall ensure employees have a current and valid driver’s license and commercial permits/licenses for the type of vehicle operated, before allowing the employee to operate a Contractor-owned or rental vehicle on Keesler AFB. The Contractor shall not employ persons for work on this contract if such employee is identified by the government or FM officials as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population. Employees of the US Government shall not be employed if the employment creates a conflict of interest. Additionally, the Contractor shall not employ any person who is an employee of the Department of their Air Force, either military or civilian, unless such person seeks and receives approval according to the appropriate ethics regulations. The Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106 Air Force Industrial Labor Relations.

2.23.7 Vehicles: The Contractor shall provide all vehicles to perform the requirements of the contract.

All vehicles shall be in operable condition and meet local, state and federal safety requirements, and be specifically designed for the purpose intended for use. Vehicles shall have, clearly visible on each side, the Contractor company name, a unique vehicle identification number, in addition to all required Department of Transportation (DOT) markings and information.

2.23.8 Cellular Phones: The use of cellular phones while operating a vehicle is prohibited while on Keesler

AFB property.

2.23.9 Keesler Medical Facilities Badge Requirements:

A separate KMC badge is required for all service providers performing work at the KMC. In order for the Hospital Security Office to provide quality badge service to the service provider the following requirements must be met:

1] Each service provider employee must complete the KMC Safety Orientation training (1/2 hour duration) prior to performing work. The FM Technical Monitor will coordinate this training with the appropriate KMC staff. Upon successful completion of this orientation the service provider employee(s) will be directed to the KMC Security Officer.

2] Employee(s) must present the letter described at the beginning of this section 1.24 to the KMC Security Officer in order to obtain a KMC Badge.

NOTE: The FM Technical Monitor will coordinate badge requirements with the KMC Security Officer. The only rights assigned to these issued badges will be determined by the FM Technical Monitor and will be coordinated with the KMC Security Officer. No walk in request will be honored unless it comes from the KMC Facility Management.

3] Employee must present proof of identification to the KMC Security Officer. Valid forms of identification include: Current driver’s license, official ID card, or passport.

4] The KMC Security Officer will provide the employee with a KMC Badge which must be displayed appropriately at all times while performing work at the KMC.

5] Service provider employees are required to sign in and sign out on the Service provider Sign In Sheet each work day when work is performed. The Service provider Sign In Sheet is located in Building 0468 Room GC643.
