STATEMENT OF WORK

For

Remove Waste Oil Burner Systems Location Building: 1639 & 1642

WORK ORDER NUMBER: 67188 & 67189

DATED: 23 August 2018

1. GENERAL INFORMATION:

This project is intended to remove waste oil burner systems in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, Unified Facilities Criteria (UFC) state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. Effective immediately: Due to safety concerns the use of manhole fixed ladders on Hanscom AFB is prohibited for the purpose of entry and egress is prohibited. An alternative means is authorized (i.e. appropriate portable ladder/personnel lowering device).

2. SCOPE: Remove Waste Oil Burner System 1639

See attached drawing for information on boiler Remove Clean Burn Model CB1800 Remove all associated piping and electrical End electrical at junction box Remove outside vent pipe and fill hole with waterproof sealant Remove all hangers and sensors associated with waster oil burner Remove 250 gal AST from building and properly dispose of Remove and properly dispose 55 gallon drum and spill container Excavate and dispose of properly 4500 Waste oil Regulatory tank and piping Backfill the hole to grade Replace asphalt where tank was Provide written documentation of removal Remove stack pipes and repair pan in roof with weatherproof and waterproof material Contractor must forward Hanscom AFB Contractor Environmental Guide Remove Waste Oil System in building 1642 See attached drawing Remove 2 Clean Burn Model 2800 Remove all electrical at junction box Remove all conduit

Remove all associated piping and sensors Repair roof and pan to be watertight and weatherproof Remove all hangers and sensors Excavate and remove 2500 Waste Oil Tank located in lot Fill in excavated hole to grade level Asphalt over excavated area to match existing Remove 2 500 gal AST in building Remove 2 spill containers and dispose of properly off base See attached Hanscom AFB Contractor Environmental Guide Dig Safe is required for this project

3. GENERAL REQUIREMENTS:

Work includes, but is not limited to the following:

a. Provide resolutions of any technical problems that arise during the construction.

b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.

c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.

d. The Contractor is solely responsible for compliance with the Occupational Safety and

Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.

e. Any modifications and addition on a facility that contains Lightning Protections System (LPS) This includes paint projects, roofing projects, projects requiring roof penetrations, installation of new HVAC or other metallic equipment, antenna installation, or other work.

Additions and modifications to the facility envelope require re-evaluation of the LPS for the completed facility as a whole. The LPS required by the addition or modification shall not be a simple addition to the LPS. The LPS must be considered a single entity and must comply with requirements of a single LPS upon completion. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order. Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final LPS in the initial design. Projects of this type shall ensure the final LPS as a whole is compliant with AFI 32-1065 and NFPA 780, in that priority order. The same third-party inspector requirements shall apply.

f. Complete the “builder data inventory and assessment sheet” for newly installed assets, ensuring the bar code number is attached to the new equipment. Transfer bar codes from any removed equipment to the builder assessment ½ sheet form

g. Provide three (3) copies of O & M manuals and warranty letter

h. Period of Performance 90 days after award of contract
