STATEMENT OF WORK
For
4th Cliff General Construction 2016
Location Building: Fourth Cliff
WORK ORDER NUMBER: 66069 and 66101
DATED: 14 March 2016
1. GENERAL INFORMATION:
This project is intended to do general construction in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of
Massachusetts Building Code and Massachusetts General Law requirements, state and federal codes, Unified Facilities Criteria (UFC), National Fire Protection Association
(NFPA), National Electric Code (NEC 2014) and base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply.
2. SCOPE: 4th Cliff General Construction
Install Utility Meters (Base)
Contractor shall investigate and trace the incoming water and electric service line to building #4.
Contractor shall install Air Force approved electric and water meters on building #4(See
Attached).
Contractor shall provide meters’ connection drawings prior to installation for approval.
The contractor shall install the water meter in the basement and shall be placed where the water service line comes through the basement wall or basement floor.
Contractor shall install the water meter in a horizontal position, a minimum of 30 inches, but no more than 42 inches from where the water service first penetrates the floor or wall of the structure.
Contractor shall install backflow preventer, shut off valves, reducer pressure zone valves and pressure gauges to the incoming water supply.
Contractor shall make sure all valves shall be installed near the inlet and discharge of the water meter to isolate the meter from the waterline.
Contractor shall perform and provide a chlorination test report in accordance with
Massachusetts regulations.
Contractor shall install the wiring and equipment in accordance with UFC, NEC and local ordinances.
Electric meter shall have a minimum of 3 feet or a maximum of 6 feet from final grade.
Electric meter shall be installed outdoor in a weatherproof enclosure and shall be securely mounted on a solid wall or structure.
Ground rod shall be established as required by the N.E.C 2014 regulations.
Contractor shall Install a weatherproof disconnect from the meter to the service panel.
Contractor shall label all the conductors and installed equipment in accordance with NEC codes.
The electric and water meters shall be in an accessible location and it shall be no obstruction or storage of other materials preventing access to the meters.
Contractor shall coordinate with building manager prior to any service interruption and
14 days’ notice prior to any outage.
Contractor shall submit electrical and plumbing drawings by a register professional engineer.
Contractor shall test the meters after installation and provide a written test report for both electric and water meter along with a backflow preventer test report in accordance with code of Massachusetts regulations.
Contractor shall coordinate with the program manager for a final walk through inspection prior to project close out.
Install ADA Automatic Door Bldg. 7 (Option 1)
Remove existing door, frame and sill.
Install new 3’-0” x 7’-0” x 1-3/4” pre-hung ADA compliant LH outswing door and frame in previous door’s location.
Install 6” x 6” pressure treated post set in concrete next to the ramp and extending at least
40” above the ramp and 5”-0” from the building wall (see drawing).
Install two ADA compliant actuator buttons: One inside building on wall adjacent to latching edge of door where there is electrical service for room lighting. Outside actuator to be located on ramp side of 6” x 6” post adjacent to ramp (see attached drawing) with electrical service running underground from building. Both actuator buttons should be located 36” above the adjacent surface.
Install a small ramp at door opening to transition from the existing concrete ramp up to the inside finish floor making it easier for a wheelchair to enter the building.
Verify proper operation of door opening and closing from each actuator button as well as manual operation of the door.
3. GENERAL REQUIREMENTS:
Work includes, but is not limited to the following:
a. The work shall be compliant with (but not limited to):
1) UFC 3-420-01 Change 10-, October 28 2015 Plumbing systems.
2) UFC 3-230-01 Change 2, 1, July 2014 Water storage, distribution, and transmission.
3) UFC 3-550-01 Change 1, July 1. 2012 Exterior electrical power distribution.
b. Provide resolutions of any technical problems that arise during the construction.
c. The period of performance (POP) of this project is (60) sixty days after award of contract.
d. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.
e. Provide a full work schedule to Project POC upon approval of contract. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.
f. The Contractor is solely responsible for compliance with the Occupational Safety and
Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.
g. The contractor shall follow Air Force Unified Facilities Criteria (UFC) requirements, National Electric Code 2014 (NEC), National Fire Protection Association (NFPA), and all the local regulations.
h. Provide three (3) copies of O&M manuals and warranty letter.
i. Complete the “builder data inventory and assessment sheet” for newly installed assets, ensuring the bar code number is attached to the new equipment. Transfer bar codes from any removed equipment to the builder assessment ½ sheet form.
