STATEMENT OF WORK
4 May 2018
1. GENERAL
Project No. /Title: SXHT 16-1100 Repair Lift Station 725 Provide all labor, materials and equipment necessary to replace Lift Station 725 and force main in accordance with the drawings and specifications, dated February 2018, and the Florida Department of Environmental Protection (FDEP) general permit, issued March 2018 at Hangar 750 to provide the government a complete and usable facility. The contractor is required to obtain a professional engineer registered in Florida to complete and certify the “Request for Approval to Place a Domestic Wastewater Collection/Transmission System into Operation” using Form 62-604.300(8)(b) in accordance with Permit Number 0080443-018-DWC/CG to be provided as a material submittal coordinated through contracting officer to the base environmental office and FDEP for approval. The contractor is responsible for submitting an approved dewatering work plan to the contracting officer. All work will comply with current Unified Facilities Criteria, International Building Code, National Electric Code and manufacturer’s recommendations. The contractor’s proposal and subcontractor’s costs must include a detailed cost breakdown of material, labor and equipment.
2. CONSTRUCTION
· Pre-construction conference
· Notice To Proceed (NTP) given to the contractor by the Contracting officer Procurement of all applicable permits and temporary construction waiver by the contractor and material submittals given to the government for approval
· Submit dewatering plan for approval (15 days prior to work start)
· Install Stormwater Best Management Practices (BMPs) for erosion and sedimentation control
· Demolition Activities
· Construction Activities
· Obtain Florida registered professional engineer to complete and certify the “Request for Approval to Place a Domestic Wastewater Collection/Transmission System into Operation” using Form 62-604.300(8)(b) available at https://floridadep.gov/sites/default/files/62-604.300_8b.pdf
· Final inspection and Project Closeout
