STATEMENT OF WORK

DEOMI Phase II Carpet Replacement

1. SCOPE OF WORK

1.1. The contractor shall provide all labor, equipment, tools, accessories and materials to properly remove and dispose of 3347 square yards (SY) of carpet.

1.2. The contractor shall be responsible for moving/lifting 935 SY of standard office furniture

The unit library is included in this requirement which contains 12 each book stacks measuring

18.5’ long x 1.5’ wide and 7’ high; however, lifting or moving of book stacks is not required.

1.3. The contractor shall be responsible for moving/lifting 2261 SY of system furniture as required to perform proper installation.

1.4. The contractor shall be responsible for the removal and replacement of approximately 580 linear feet (LF) of vinyl base cove (wall base) and installation of 115 LF of transition strips in those areas were carpet meets tile flooring.

1.5. The contractor shall perform 29,963 square feet (SF) of surface preparation to insure base floor surface is level prepared for new carpet installation.

1.6. The contractor shall install carpet tiles in areas outlined in section 2 of this Statement of

Work (SOW) in the Defense Equal Opportunity Management Institute (DEOMI) facility located at 366 Tuskegee Airmen Drive (Building 352), Patrick AFB, FL 32925.

3. EXECUTION

3.1. The performance period will be 90 calendar days after delivery of contract (ADC).

Working hours will be between 7:30 AM and 4:30 PM, Monday through Friday.

3.2. The contractor shall coordinate all activities with the Government Inspector (contact information will be provided upon award) to include dates and times of performance and coordinate any revised schedules/plans if any changes are necessary.

3.3. The contractor’s performance must meet or exceed the Carpet and Rug Institute’s Standard for Installation of Commercial Carpet. Installation must be performed in strict compliance with all local, state and federal regulations. The Occupational Safety and Health Administration

Hazard Communication Standard must be followed.

3.4. The government inspector will inspect completed areas at the end of each workday. If any areas are found to be unacceptable, the contractor shall re-perform until the newly installed area meets standard commercial practices.

4. CLEAN UP

4.1. Clean up of the job site shall be the responsibility of the contractor and shall occur at the end of each workday. The flooring will be vacuumed and cleaned before inspection by the

Government Inspector.

4.2. Replaced carpet, remnants, waste and/or byproducts generated by the requirements outlined in this SOW shall be removed and disposed of by contractor off base in compliance with all state and local laws/ordinances.
