PERFORMANCE-BASED WORK STATEMENT (PWS)
FOR
MEDICAL WASTE REMOVAL/DISPOSAL
45TH MEDICAL GROUP, PATRICK AFB, FL 32925

1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to operate and maintain the storage, pickup, transportation, and disposal of packaged medical waste generated from the Patrick Air Force Base Medical Clinic and the associated 45th Medical Group activities, Patrick AFB, Florida as stated in this Performance-Based Work Statement (PWS). The contractor shall perform to the standards in this contract.
1.1. BASIC SERVICES.
1.1.1. Purpose: This is a non-personnel services contract to provide Medical Waste Removal and Disposal for all medical waste generated from the 45th Medical Group.
1.1.2. Background: The 45th Medical Group Medical Waste Removal and Disposal contract has been in place for several years and is necessary to ensure proper removal and disposal of packaged medical waste.
1.1.3. Historical Workload Data: See Appendix C.
1.1.4. Scope: The contractor shall perform these services in accordance with applicable federal, state, and local laws and regulations. The contractor shall be a state registered/permitted medical waste transporter. The method of disposal shall be any approved method (i.e., incineration, autoclave/pulverize, hyper chlorate/pulverize, etc.) recognized by the state, in which the medical waste is treated and disposed. The contractor shall provide the Contracting Officer and MDG Facilities Management with copies of disposal permits from all treatment facilities, transporter licenses, and list of landfills where “treated waste” is subsequently transported. These documents shall be provided within 30 days after beginning services, at renewal of contract, or whenever renewed during the contract period.
1.1.4.1. Outer Packaging. The contractor shall provide a sufficient quantity (approximately 7-10 per pickup) of clean (and disinfected – if reusable) 30 gallon corrugated or plastic containers for the packaging of generated medical waste. The containers shall be rigid, leak-resistant, puncture-resistant, and impervious to moisture, strong enough to prevent tearing or bursting under normal conditions of use and handling, and sealed to prevent leakage during transport.
The contractor shall label all containers with the words “Medical Waste” or “Infectious Waste” or display the universal biohazard symbol. The Contractor shall label all containers with the generators name and address (45th Medical Group, 1381 South Patrick Drive, Patrick AFB, FL), The transporters name, transporters state permit number, or address.
1.1.4.2. Spill Cleanup. The contractor shall be responsible for cleaning and decontaminating the area of any medical waste spill occurring during pickup operations at the Patrick Clinic.
1.1.5. Project Approach:
1.1.5.1. Routine Pickup Schedule. The contractor shall pickup all properly packaged containers of medical waste every Tuesday or the next scheduled business day after a Tuesday federal holiday, or more frequently, if necessary. The contractor shall remove and exchange the containers between the hours of 0800 - 1500. Deviations from this schedule shall be approved by the Contracting Officer and coordinated through the MDG Facilities Management.
1.1.5.2. Emergency/Contingency Collections. Contractor shall provide out-of-cycle emergency collection services, as required, to meet unforeseen medical contingencies or other medical emergencies. Expected frequency for this service is not greater than once per calendar year.
1.1.6. Constraints:
1.1.6.1. Transport Receipt. The contractor shall provide MDG Facilities Management with a manifest listing all containers and weight of each container for each shipment of medical waste picked up and transported off-site from the Patrick Clinic. The Contractor and the Facility Management representative will sign the manifest and a copy will be given to Facility Management prior to transport. The manifest shall include the contractor’s registered permit number, date of pickup, generator’s name, & address, contract number and account number. The waste will be transported in a fully enclosed vehicle approved by federal, state, and local regulatory authorities to transport regulated medical waste.
1.1.6.2. Disposal Invoice. The contractor shall provide Facilities Management with a disposal invoice for each shipment of medical waste transported off-site, treated, and disposed. The invoice shall include the total pounds for each shipment, the applicable transport receipt number, the generator’s name & address, contract number and account number, date of destruction, means of destruction, and name of person destroying the waste. The contractor may review invoice instructions provided by the Defense Finance and Accounting Office at the following web address: www.dfas.mil.
1.1.6.3. Other Documentation. The contractor shall provide the Patrick Clinic (Facilities Management) with a certification that scales used to weigh the collection/storage container are state certified, and a certification on how often the scales are calibrated and how the tare weight for each shipment is established. These certifications shall be provided within 30 days after beginning services.
1.1.7. Considerations of Satisfaction:
1.1.7.1. Failure to Perform. Failure to meet the terms and conditions as specified within this PWS and elsewhere in the contract constitute a breach of contract and authorize the government to seek all remedies authorized by law.
1.2. SPECIAL REQUIREMENTS.
1.2.1. Definitions of Waste. Medical waste to be destroyed shall include the following categories as defined in Title 64 of the Florida Department of Health, Division of Disease Control and the Department of Transportation:

1.2.1.2. Cultures and stocks. Cultures and stocks of infectious agents and associated biological;
waste from the production of biological; discarded live and attenuated vaccines; and culture dishes and devices used to transfer, inoculate, and mix cultures.
1.2.1.3. Pathological wastes. Human pathological wastes, including tissues, organs, body parts, and body fluids that are removed during surgery, autopsy or other medical procedures.
1.2.1.4. Human blood and blood products. Discarded waste human blood or blood components (e.g., serum and plasma) and materials containing free-flowing blood and blood components.
1.2.1.5. Sharps. Sharps that have been used in animal or human patient care and treatment or in medical and research laboratories, including hypodermic needles, syringes (with or without attached needle), Pasteur pipettes, scalpel blades, blood vials, test tubes, needles with attached tubing, and culture dishes (regardless of presence of infectious agents). Also included are other types of broken or unbroken glassware such as slides and cover slips.
1.2.1.6. Animal wastes. Contaminated animal carcasses, body parts, and bedding of animals that were known to have been exposed to infectious agents, production of biological, or testing of pharmaceuticals.
1.2.1.7. Isolation wastes. This category covers biological waste and discarded materials contaminated with blood, excretions, exudates, or secretions from humans who are isolated to protect others from certain highly virulent organisms, or isolated animals known to be infected with highly virulent diseases. Highly virulent diseases are those derived from class IV etiologic agents as defined by the Center for Disease Control.
1.2.1.8. Other. Non-hazardous chemotherapy wastes generated during the preparation and administration of chemotherapy drugs.
1.2.1.9. Outdated Drugs. Non-hazardous outdated drugs that must be incinerated due to federal and state regulations.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall provide a holding area for the medical waste. This location is in building 1380, room 1166.
3. GENERAL INFORMATION.
3.1. Security Requirements. The 45th Medical Group Facilities Management Office or the 45th Medical Group Security Manager will complete a local 45 SFS Badge/Visitor Request form for each contract employee. This form will be hand-carried to the Pass and Registration Office where a background check will be initiated. The contractor should allow two business days for processing. Upon approval, contract employees will be issued an entry badge.
3.2. QUALITY CONTROL
3.2.1. The Government Representative(s) or alternate(s) will inform the medical service contract manager when discrepancies occur and will request corrective action. The Government Representative(s) or alternate(s) will make a notation of the discrepancy with the date and time the discrepancy was noted and will request the contract manager (or authorized representative) notify the Contracting Officer.
3.2.2. Any matter concerning a change of the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer.
3.2.3. The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the Contracting Officer and or authorized Government representative(s).
3.3. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, property, and materials shall be secured.
3.4. HOURS OF OPERATION.
3.4.1. Normal Hours of Operation. The normal hours of operation for the 45th Medical Group are Monday thru Friday from 7:30 a.m. to 4:30 p.m. The operating hours associated to this contract are stated in Section 1.1.4.1 above.
3.4.2. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
3.4.3. Emergency Services. Outlined in Section 1.1.4.2 above.
