COMBINED SYNOPSIS SOLICITATION
| Carpet Supply and Installation |
| FA2517-19-Q-5943 |
i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation will not be issued.
(ii) This solicitation FA2517-19-Q-5982 is being issued as a Request for Quote (RFQ).
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-05 effective 13 August 2019.
(iv) This effort is being procured as a 100% Small Business set-aside. The North American Industrial Classification System (NAICS) code is 314110 – Carpet and Rug Mills, and the size standard is 1,500 employees.
(v) Prospective Contractor quotes shall price in accordance with (IAW) the Statement of Work (SOW) dated 6 August 2019 under the following five CLINS:
CLIN 0001: CMAFS (Cheyenne Mountain Air Force Station) – Shall be the quoted price total of the carpet inclusive of labor, materials, & cove.
CLIN 0002: CMAFS Rubber - Shall be the quoted price of the rubber flooring for the entryway of Bldg 617, inclusive of labor, materials, & cove. Pricing shall also include LABOR & COVE ONLY for Rm 1207 Bld 1000. Rubber flooring for Rm 1207 is provided by the Government as Government-Furnished Property (GFP).
CLIN 0003: 21 CPTS (21st Comptroller Squadron) – Shall be the quoted price total of the carpet inclusive of labor, materials, & cove for PAFB Bld 350 CPTS.
CLIN 0004 (OPTION): 21 CONS Open Area Suite 2225 (21st Contracting Squadron) – Shall be the quoted price total of the carpet inclusive of labor, materials, & cove for PAFB (Peterson Air Force Base) Bldg. 350 21CONS.
CLIN 0005 (OPTION): 21CONS Command Section Suite 2204 (21st Contracting Squadron) – Shall be the quoted price total of the carpet inclusive of labor, materials, & cove for PAFB (Peterson Air Force Base) Bldg. 350 21CONS.
(vi) As applicable to each CLIN, the Contractor shall provide tools, supplies, equipment, and labor required for the supply and installation of carpet in accordance with the Statement of Work.
(vii) The Period of Performance is 12 months after date of contract.
(viii) Provision FAR 52.212-1 -- Instructions to Offerors -- Commercial Items (Oct 2018), applies to this acquisition.
(a) To assure timely and equitable evaluation of the quote, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I – Price and Part II – Technical Information.
(b) The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offerors, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional price information will be necessary. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information other than pricing data to support a determination of price reasonableness.
(c) Specific Instructions: Documents submitted in response to this must be fully responsive to and consistent with the following:
(1) Requirements of the CLINs and Statement of Work (SOW) and government standards and regulations pertaining to the SOW and Floor Plan.
(2) Evaluation Factors for Award in section, 52.212-2 EVALUATION – COMMERCIAL ITEMS and addendum of this quote.
(d) PART I – PRICE (1) The Contractor shall provide the following:
(1) Pricing for the effort by providing Attachment 2 – Contractor Submittal Form
(e) PART II – TECHNICAL INFORMATION (1) Format
• The quote will be on 8 ½ x 11 paper
• A page is defined as one single sided, double-spaced sheet of paper containing information
• Typing shall be Times New Roman Font, size 12
• Electronic files must be in Adobe (.pdf), Microsoft Word, version 6.0/95 or Excel 2010 or earlier format
• 10 page limit
(2) Technical Acceptability – using the format described above, please provide the following:
Contractor shall provide sufficient documentation to demonstrate their ability to provide the qualifications as stated in the SOW, Attachment 1.
(ix) FAR 52.212-2 – Evaluation – Commercial Items (Oct 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
(1) Technical capability of the item offered to meet the Government requirement
(2) Price
(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(End of Provision) Addendum to FAR 52.212-2 Evaluation – Commercial Items Basis for Award: (1) The resulting award will be a Firm Fixed Price Purchase Order to a single contractor. By submission of its offer, the Offeror accedes to all solicitation requirements, including terms and conditions, representations and certification and requirements, in addition to those identified as evaluation factors or sub factors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
(2) The Government will first rank all quotes from lowest to highest price. The lowest priced quote will then be evaluated for its technical acceptability. If the lowest priced quote is technically acceptable, the evaluation process stops. The lowest priced offeror who is technically acceptable will receive the award. The award will be made to that offeror without further consideration of any other offerors.
(3) Technical capability will be determined by the following:
a) The Contractor must show how they will meet the requirements of the SOW, including carpet and cove materials that will be used.
b) These are the definitions for the Evaluation Rating:
1. Acceptable – Meets all solicitation requirements.
2. Unacceptable – Does not meet all solicitation requirements.
(4) Price Reasonableness will be determined as follows:
a) Prices will be determined to be fair and reasonable by adequate price competition among offers unless the contracting officer has requested other pricing data per 52.212-1 above.
(5) Offerors are cautioned to submit sufficient information and in the format specified in the quote preparation instructions to permit a meaningful assessment of technical capability. Offerors may be asked to clarify certain aspects of their quote. Communication conducted to resolve minor or clerical errors will not constitute discussion and the contracting officer reserves the right to award a contract without the opportunity for quote revision(s).
(6) The Government intends to award a contract without discussions with respective offerors. The Government however, reserves the right to conduct discussions if deemed in its best interest.

(d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

FAR 52.204-21 – Basic Safeguarding of Covered Contractor Information Systems (Jun 2016)
(a) Definitions. As used in this clause-- “Covered contractor information system” means an information system that is owned or operated by a contractor that processes, stores, or transmits Federal contract information.
“Federal contract information” means information, not intended for public release, that is provided by or generated for the Government under a contract to develop or deliver a product or service to the Government, but not including information provided by the Government to the public (such as on public Web sites) or simple transactional information, such as necessary to process payments.
“Information” means any communication or representation of knowledge such as facts, data, or opinions, in any medium or form, including textual, numerical, graphic, cartographic, narrative, or audiovisual (Committee on National Security Systems Instruction (CNSSI) 4009).
“Information system” means a discrete set of information resources organized for the collection, processing, maintenance, use, sharing, dissemination, or disposition of information (44 U.S.C. 3502).
“Safeguarding” means measures or controls that are prescribed to protect information systems.
(b) Safeguarding requirements and procedures.
(1) The Contractor shall apply the following basic safeguarding requirements and procedures to protect covered contractor information systems. Requirements and procedures for basic safeguarding of covered contractor information systems shall include, at a minimum, the following security controls:
(i) Limit information system access to authorized users, processes acting on behalf of authorized users, or devices (including other information systems).
(ii) Limit information system access to the types of transactions and functions that authorized users are permitted to execute.
(iii) Verify and control/limit connections to and use of external information systems.
(iv) Control information posted or processed on publicly accessible information systems.
(v) Identify information system users, processes acting on behalf of users, or devices.
(vi) Authenticate (or verify) the identities of those users, processes, or devices, as a prerequisite to allowing access to organizational information systems.
(vii) Sanitize or destroy information system media containing Federal Contract Information before disposal or release for reuse.
(viii) Limit physical access to organizational information systems, equipment, and the respective operating environments to authorized individuals.
(ix) Escort visitors and monitor visitor activity; maintain audit logs of physical access; and control and manage physical access devices.
(x) Monitor, control, and protect organizational communications (i.e., information transmitted or received by organizational information systems) at the external boundaries and key internal boundaries of the information systems.
(xi) Implement subnetworks for publicly accessible system components that are physically or logically separated from internal networks.
(xii) Identify, report, and correct information and information system flaws in a timely manner.
(xiii) Provide protection from malicious code at appropriate locations within organizational information systems.
(xiv) Update malicious code protection mechanisms when new releases are available.
(xv) Perform periodic scans of the information system and real-time scans of files from external sources as files are downloaded, opened, or executed.
(2) Other requirements. This clause does not relieve the Contractor of any other specific safeguarding requirements specified by Federal agencies and departments relating to covered contractor information systems generally or other Federal safeguarding requirements for controlled unclassified information (CUI) as established by Executive Order 13556.
(c) Subcontracts. The Contractor shall include the substance of this clause, including this paragraph (c), in subcontracts under this contract (including subcontracts for the acquisition of commercial items, other than commercially available off-the-shelf items), in which the subcontractor may have Federal contract information residing in or transiting through its information system.

(c) WAWF access. To access WAWF, the Contractor shall—

a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, name of employee, the last four of the employee’s social security number and their date of birth.
The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate to obtain a vehicle pass.
(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Air Force Installation Security Program, AFI 31-501, Personnel Security Program Management, and the Performance Work Statement as applicable.
(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.
