PERFORMANCE WORK STATEMENTS FOR
SAFE-T ELEMENT INSTALLATION ON COOK TOPS
FOR DORMS 1158 and 1270 AT PETERSON AFB
1.0 SCOPE OF WORK: The contractor shall provide all labor, parts, tools, materials, equipment, personnel, supervision, and transportation necessary to install Safe-T-elements to cook tops in dorm buildings 1158 and 1270.
1.1 SPECIFICATIONS AND DESCRIPTION OF WORK:
1.2 INSTALLATION: A certified technician shall perform the installation of Safe-T-element cook tops in dorm buildings 1158 and 1270, in accordance with the manufacturer’s installation instructions provided in Attachment 1. The technician shall test all cook tops and ensure that it is operating according to manufacturer’s specifications.
1.3 Escort: The contractor shall schedule work with Contracting Officer Representive (COR) listed in paragraph 4.5 below to coordinate an escort in all the rooms that SAFE-T element will be installed.
1.4 Contractor Qualification/Experience: For the life of this contract, a licensed electrician, shall have undergone the instructions necessary to reliably perform installation and inspection of the Safe-T-element cooking system and provide certificate(s) upon request. The certified person(s) shall have available the appropriate manufacture’s service manual(s), the proper types of tools, and manufacturer’s recommended parts specifically for the Safe-T-element. This is a requirement so that the manufacturer's warranty is not voided. Documentation of training or certification shall be made available to the CO or COR upon contract award.
1.5 Parts: The contractor shall purchase the brand name Safe-T-element from Pioneer Technology. Pioneering Technology is the manufacture and University Electronics, Inc, (800) 262-0101, www.universityelectronics.com is the distributer.
2.0 INSPECTION OF WORK:
2.1 Inspection: Upon completion of all work, the contractor must inform the COR, listed in paragraph 4.5, so work can be inspected for quality assurance.
3.0 GOVERNMENT FURNISHED PROPERTY: There will be no GFP for this contract.
4.0 GENERAL INFORMATION:
4.1 Level of Service: The level of service shall be comparable to that provided private industry.
4.2 Hours of Operation: The normal hours of operation will be 7:30 A.M to 4:30 P.M. Monday through Friday, excluding holidays.
4.2.1. HOLIDAYS. Routine repair work will not be required on the following holidays or any holidays observed in lieu thereof:

President's Day (February)
Veterans' Day (November)
Memorial Day (May)
Thanksgiving Day (November)

4.3 Contact Manager: The contractor shall provide a contact manager and an alternate who shall be responsible for the performance of the work. The name of this person and an alternate shall be designated in writing to the contracting officer by the contract start date. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the installation of the equipment. The contract manager and alternate must be able to read, write, speak, and understand English.
4.4 Contractor Personnel: Contractor personnel shall be courteous, professional, present a neat appearance and be easily recognized. This includes wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name. Employees shall wear base badge visiable at all times.
4.5 Performance of the Service Contract will be monitored by Mr. Tim Famer, MSgt Jesus Revilla (719) 556-1409/3832 or SSgt Cornelius Deems (719) 556-0460.
4.6. SECURITY REQUIREMENTS. The Contractor and all employees shall comply with all Peterson AFB security requirement imposed by the local installation commander at all times while on the premises. Contractor shall obtain necessary badges(s) required for access to Peterson AFB.
4.6.1. Badge Request. Contractor shall send a letter to the Contracting Office on company stationery, which includes address, telephone number, and contract number, listing full names of employee(s), with their social security number, and date of birth, who require access to Peterson AFB. Anytime a badge is requested for a new employee, an updated employee list will accompany a request letter by the contractor to the contracting office.
4.6.2. Contractor employees will apply for individual identification badges at the Peterson AFB Visitor’s Control Center, Bldg 1334. No appointment is necessary. Upon completion or termination of the contract or termination of a contractor employee, the Contractor shall surrender any identification badges and vehicle identification to the CO the following business day.
4.6.3. Vehicle Requirement. Proper state vehicle registration, proof of insurance and a valid driver’s license are required to drive on base. After obtaining the proper badges/vehicle identification at the Visitor Control Center (Bldg 1334), all commercial vehicles must enter through the East Gate (Marksheffel Road), the primary search area for all commercial delivery vehicles. The Contractor and all employees shall comply with all Peterson AFB security requirement imposed by AFI 91-207.
4.7 INVOICING. After completion or within five (5) working days, Contractor shall process invoice(s) via Wide Area Work Flow (WAWF) and forward electronically to the Government.
4.8. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY. In accordance with (IAW)Department of Defense Instruction (DODI) 3020.37, paragraph E.2.1.1, a crisis situation is defined as: Any emergency so declared by the National Command Authority or the overseas Combatant Commander, whether or not U.S. Armed Forces are involved, minimally encompassing civil unrest or insurrection, civil war, civil disorder, terrorism, hostilities buildup, wartime conditions, disasters, or international conflict presenting a serious threat to DOD interests. The contractor is not required to perform services during a crisis unless otherwise directed by an authorized government representative.
