Performance Work Statement

FLASHJET TECHNICAL SUPPORT

16 August 2016

1.0 DESCRIPTION OF SERVICES. The Contractor shall provide technical expertise, training, and engineering support as required to enable the Government to maintain, repair, and align two FLASHJET system equipment sets at Robins AFB, GA. All associated software maintenance and upgrades shall be accomplished by the Contractor. The Contractor shall provide support to keep the FLASHJET equipment functioning safely, effectively, and efficiently with the goal being to reduce downtime. The Contractor is responsible for providing any required specialized equipment, technical expertise, and engineering support necessary to support Robins AFB’s FLASHJET system equipment sets and associated software. This support includes telephone, email, and on-site software support, technical assistance, user support, development and integration of additional capabilities, and testing of all modifications to these systems. All maintenance except for off-site repairs will be accomplished by the Government with the Contractor providing support and oversight within Buildings 20180 and 673.

This contractual requirement includes the equipment listed in Attachment 1, FLASHJET Systems Equipment Listing. The period of performance will be a basic 12-month period with four annual options.

1.1 The required support shall include all engineering and technical expertise support to enable the Government to perform corrective maintenance type repairs and preventative maintenance to the equipment listed in Attachment 1. For on-site work at Robins AFB, the Contractor shall oversee PM and CM requirements with Government personnel actually performing the hands-on work as instructed by the Contractor. Based on the resulting evaluation, the necessary repairs shall be made to return the component back to a serviceable condition. The Contractor shall also provide maintenance support and necessary modifications to the FLASHJET system software on the equipment listed in Attachment 1 to ensure successful system configuration changes, programming changes, file system maintenance, system backup/restoration, and group/user authority management.

1.2 Preventative Maintenance. The Contractor shall be on site every 3 months (during the first two weeks of each quarter after award of contract) for a minimum of five (5) consecutive business days (Monday through Friday, excluding Federal holidays) to provide the required support services, oversee the performance of required preventative maintenance (PM), and review the equipment preventative maintenance instructions (PMI). The Contractor shall coordinate each 5-day quarterly scheduled site visit with the MXSG Equipment Specialist/Supervisor (402 MXSS/MXDVBB) to ensure the site visit coincides with the Government PM schedule. The Government will provide all consumable items required for PM (i.e. belts, oil, grease).

1.2.1 Preventative Maintenance Instructions (PMI). During the basic (initial 12-month) period of performance, the Contractor shall review all preventative maintenance procedures and develop any new procedures as needed. This shall include drafting, editing, integration, and revision of procedures for equipment assets currently being supported as listed in Attachment 1 (see also Attachment 2, Current Preventative Maintenance Inspection [PMI] Schedule). All PMI updates shall be completed by the end of the basic 12-month period. The Contractor shall provide copies to the On-Site Subject Matter Expert (SME) (402 CMSS/MXDQBE) and MXSG Equipment Specialist/Supervisor (402 MXSS/ MXDVBB) of any and all data pertaining to maintenance manuals, operator manuals, drawings, and software files or other maintenance documentation that are modified or created in the performance of this contract. Data shall be provided in the original product format within 45 calendar days after any change. The Government requiring office shall provide written approval/disapproval within 15 calendar days. The Contractor shall provide the final data product within 25 calendar days. 1.3 Corrective Maintenance Repair (Work Stoppage). Corrective Maintenance [CM] Repair is performed outside of scheduled preventative maintenance. Examples include but are not limited to chiller malfunctions, loose electrical connections that cause controller faults, power supplies going down, deionized/distilled water supply system issues, CO2 Pellet system issues, lamp issues, camera issues, and physical damage to the scooper duct.

1.3.1 Level-one support. Level-one support shall consist of the Contractor providing technical support by telephone to the Government. The Contractor shall provide technical telephone support from 7:00 am to 5:00 pm (EST), Monday through Friday. The Contractor shall provide priority status for technical support inquiries and shall respond by telephone within four (4) hours of being contacted by the Government Program Manager.

1.3.2 The Government will first attempt to perform corrective repairs to the CNC equipment by utilizing telephone technical support provided by the Contractor. If Contractor telephone support does not resolve the problem within eight (8) hours, level-two support will be required.

1.3.3 Level-two support. Level-two support occurs when level-one support is not adequate to return the system to operational status within eight (8) hours. If a work stoppage occurs and a solution cannot be provided via telephone, the Contractor shall dispatch a service technician to respond on-site. When level-two support is required, the Government Program Manager will contact the contractor and notify the primary COR. The Contractor shall provide on-site response no later than 48 hours after notification, and shall continue working until the problem is resolved.

1.3.4 New Parts. When new parts are required to accomplish corrective repair, the Contractor shall identify and price all required parts within two (2) business days of the notification . The Contractor shall provide the information to the Government Program Manager and Contracting Officer Representative (COR) for approval before proceeding with the repair. The COR will obtain Contracting Officer approval for all new parts. (CDRL A003, DI-ADMN-81401B NOT 1, Contract Change Proposals (CCP))

1.4 Off-site Repair. After an initial assessment, if an item cannot be repaired on-site, the item will be shipped by the Contractor to an evaluation facility. The Contractor shall provide a written repair estimate to the Government Program Manager and COR by email no later than two

(2) business days from receipt of the item by the evaluation facility. The COR will obtain Contracting Officer approval before the repairs are performed. (CDRL A003, DI-ADMN- 81401B NOT 1, Contract Change Proposals (CCP))

1.4.1 The information included in the repair estimate shall include equipment ID #, date of evaluation, description of findings and recommendations, required part(s) and part(s) number, cost of part(s), number of parts needed, estimated labor hours required to complete repair, and a proposed timeline for completion of the repair. The timeline must be agreed upon by the Government and will be adhered to by the Contractor. The Contractor shall be responsible for shipping repaired items back to the Government.

1.5 Acceptance. After all repair work is complete, the Contractor shall perform a functional test of the equipment to ensure proper operation. The Government Program Manager will observe and approve the functional test results. The Contractor shall provide a maintenance log detailing all tasks completed. This information shall be submitted via email to the Government Program Manager and COR. (CDRL A004, DI-MISC-80508B, Technical Report - Study Services – Maintenance Log)

1.6 Contractor Contact Information. The Contractor shall provide the Government Program Manager and COR with technical support contact information to include telephone numbers, fax numbers, and email addresses to enable the Government to submit requests for level-one and level-two support.

1.3.2.1 No later than five (5) business days after receipt at the Contractor’s facility, the Contractor shall evaluate the item’s condition and provide a written repair quote to include all required parts/materials by e-mail to the technical On-Site SME. The Contractor shall notify the Government of any anticipated delays to request an extension.

3.0 Government Furnished Property (GFP)/ Government Furnished Equipment (GFE). Contractor will track, secure, and account for all GFP/GFE IAW FAR 45 and DFARs 245. Items that may generate as reparable GFP under this agreement are listed in Attachment 3, Scheduled Government Furnished Property (SGFP). The Government point of contact for delivery and return of GFE is the On-Site SME.

3.1. Government Inventory/Tracking

3.1.1 The Contractor shall document and account for all Government property. Upon return shipment of each item/component received for service in performance of this agreement, the Contractor shall submit a physical inventory status report to the Government Property Administrator (WR-ALC/PZIO) and the Government POC. The report shall detail the identifying information, condition, and status of each item of GFP. (CDRL A005, DI-MGMT-80269/T, Status of Government Furnished Equipment [GFE] Report)

3.1.2 Equipment. The Contractor shall maintain records evidencing receipt, accountability, preventative maintenance, and return/disposal as defined within the government property clauses. At the end of the contract, the Contractor shall return all GFP to the Government.

4.0 GENERAL INFORMATION

4.1 Place of Performance: The place of performance for this contract will be a combination of Robins AFB, GA and the Contractor’s facility.

4.2 Core Hours. For services performed at Robins AFB, core hours shall be Monday through Friday from 0700 to 1600, excluding Federal holidays. The Contractor shall coordinate on-site activities with the On-Site SME and COR.

4.3 Essential Services During Crisis: The Functional Director (FD) has determined, by placing in writing their decision, that this requirement is not Mission Essential (M-E) in accordance with AFI 10- 403, para. 1.9.1.33, Deployment Planning and Execution, DoDI 1100.22, Policy and Procedures for Determining Workforce Mix and DFARS 237.76, Continuation of Essential Contractor Services.

4.4 Security Requirements

4.4.1 Government Security Regulations: The Contractor shall comply with all security policies and regulatory requirements. Contractor employees must be US Citizens or must present VISA documentation indicating they are allowed to work in the United States/Georgia before contractor credentials will be issued.

4.4.2 Requirements for Contractor Credentials: All personnel requesting installation contractor credentials must present a valid state issued identification card or driver's license and a social security card before issued a credential. Persons must present previously issued contractor identification card if applicable.

4.4.3 Security Clearance: Actual knowledge and/or generation of classified and/or controlled unclassified information are not expected to be necessary for performance of the contract/order. No contractor personnel are required to have any type of clearance in order to perform this contract.

Contractor personnel are required to comply with Government security requirements to preclude potential security incidents or violations including potential unauthorized disclosure or compromise of controlled unclassified information. If a change in the security classification of the contract/order from an unclassified to a classified status becomes necessary after contract/order award, the Contractor shall make a reasonable effort to continue performance of work under the contract/order in compliance with the change in security classification in accordance with Security Requirements.

4.4.3.1 Access to the Robins 2K network is not expected to be necessary for performance of this contract/order.

4.4.4 Security Incident or Violation: The Contractor shall immediately notify the Government Security Office of any potential or actual security incident or violation including potential or actual unauthorized disclosure or compromise of sensitive, but unclassified, information.

4.4.5. Security of Contractor Systems(s): The Contractor shall ensure the security of automated information systems at the Contractor’s facility to preclude potential security incidents or violations including potential unauthorized disclosure or compromise of controlled unclassified information.

4.4.6 Access to Government Facilities: The Contractor shall ensure Contractor personnel who require access to a Government facility comply with the security procedures and security education requirements of the facility. If Contractor personnel require repeated access to Robins AFB or another Government facility over the life of the contract/order, the Contractor shall ensure they complete, as applicable, a DD Form 1172-2 (Application for Department of Defense Common Access Card – DEERS Enrollment) to obtain a Common Access Card (CAC), an Air Force Materiel Command (AFMC) Form 496 (Application for AFMC Identification Card) to obtain an AFMC Identification Card, or an AFMC Form 387 (Contractor ID Card/Badge) to obtain a Contractor Identification (ID) Card/Badge.

4.4.7 All visitation requests will be submitted through the Joint Personnel Adjunction System (JPAS) to the contract security manager Security Management Office (SMO) (SMO code: RX1MF128).

Visitations will be made available for review by the Group Security Manager (GSM) of the area where the task will be performed.

4.5 Safety Requirements

4.5.1 Contractor’s Safety Requirement: The Air Force is part of the OSHA Voluntary Protection Program (VPP). All shall familiarize themselves with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. Additionally, the Contractor is responsible for the safety and health of all subcontractor employees.

4.5.2 The Contractor shall comply with safety provisions, i.e., technical specifications, technical publications, Federal Occupational Safety and Health Administration (OSHA) standards, and other nationally or locally recognized sources of safety, health, and fire prevention standards and practices.

The Contractor will be responsible for the safety and accountability of all their personnel and the Government facilities/property and are subject to enforcement authority by federal and state safety and health officials. The AF is not responsible for ensuring that contractors comply with "personal" safety requirements that do not present the potential to damage Government resources (e.g., hearing protection, safety shoes, AF IMT 55, etc.). The Contractor shall provide all personal protective equipment (i.e., hearing protection, gloves, steel-toed shoes, safety glasses, etc.) In addition, the Government is not responsible for medical examinations, hearing testing, substance abuse testing, etc., for contractor employees. (AFI 91-204 and AFMC Supplement 1, RAFBI 32-2001and Appendix C)

4.5.3 Safety and Health Plan: Within 10 calendar days after contract award, the Contractor shall provide a copy of their company Safety Plan that is required to ensure compliance with the Occupational Safety and Health Act (OSHA) (Public Law 91-596). The Safety Plan shall be provided to the Safety Office for review. The Safety Office will provide acceptance or non-acceptance within 10 working days of receipt to the Government point of contact who will notify the CO. 4.5.4 The Contractor Safety Plan shall address in detail the items listed in the contract Appendix C as applicable. For task specific procedures identified in the contract Appendix C, the Contractor’s internal procedures for those tasks must be identifiable within the submitted Safety Plan or shall be provided in an amendment or addendum to the company’s Safety Plan. If task specific procedures identified in the contract Appendix C are not submitted with the Contractor’s Safety Plan, the submitted Safety Plan will be returned as NOT ACCEPTED. The Contractor will have 5 working days to resubmit the Safety Plan, amendment or addendum to address or correct issues identified for non-acceptance. The WR-ALC Safety Office will provide acceptance or non-acceptance of the resubmission within 10 workings days of receipt to the Government POC who will notify the CO.

4.5.5 Safety Mishap/Incident Reporting: The Contractor shall report all mishaps/incidents in accordance with the Contract Appendix C. The Contractor shall immediately secure the mishap scene and damaged property and impound pertinent maintenance and training records until released by the Government’s Safety Office (78ABW/SE). Such release shall be accomplished through the CO. The Contractor shall cooperate and assist Government personnel in the investigation of the incident and submit an Accident/Incident Report within three (3) calendar days of the mishap. The Contractor shall require all subcontractors to comply with required safety, health and fire standards. (CDRL A007, Accident/Incident Report, DI-SAFT-81563 NOT 1)

4.5.6 Contract Safety Manager. As part of the Contractor Safety Program, the Contractor shall assign in their plan, by name and phone number, a person who shall be the primary point of contact for safety and health issues for the On-Site operation. The Contractor shall keep the contract safety manager information current by notifying the contracting officer of any change in personnel or contact information.

4.5.7 Quality Control: Contracting Officer’s Representative will inspect contractor performance in accordance with the Quality Assurance Surveillance Plan (QASP) and other technical guidance and regulations.

4.6 Environmental Management System (EMS): Any contractor employee working on Robins Air Force Base will require EMS training. The prime contractor is responsible for ensuring that all subcontractors comply with this requirement. Computer-based training is provided by the Base Civil Engineering Group Environmental Management Office at https://geobase.robins.af.mil/emstraining/EMSTrainingOptions.aspx. 4.7 Green Procurement Program (GPP): The Contractor shall comply with the Green Procurement Program in accordance with Federal (EO 13423) and DoD and AF (GPP) policy. Green Procurement is the purchase of environmentally preferable products and services. These products are preferred because they conserve resources, create less pollution and waste, or eliminate health and safety risks. The Green Procurement Program (GPP) includes these mandatory components:

(1) Recycled content products, also known as Comprehensive Procurement Guideline (CPG) Items http://www.epa.gov/cpg/products.htm

(2) Energy Star® and energy-efficient products; energy efficient standby power devices http://www.eere.energy.gov/

(3) Bio-based products http://www.biobased.oce.usda.gov/fb4p/

(4) Non-ozone depleting substances http://www.epa.gov/ozone/snap/lists/index.html

4.8 FOD Requirements. Contractor shall adhere to AFI 21-101(paragraph 14.19), Foreign Object Damage (FOD) Prevention Program. The purpose is to eliminate the introduction of any foreign object that could cause damage to any aircraft. FOD can be caused by not accounting for hardware, safety wire, tools, supplies, etc, during operations and maintenance. Any or all - loose objects, regardless of their origin, can cause catastrophic and costly damage to an aircraft or major end item. These regulations apply to contracted services where work is performed in, on, around, or requires travel through areas near aircraft, support equipment, engines, or components including personnel operating vehicles and equipment on AFMC flight lines, runways, taxiways, ramps, and in aircraft hangars or maintenance areas. Prevention of FOD is a critical item, which must be accomplished by the Contractor during the on-site portion of this task. This may apply to other Government Installations as directed by the TPC.

4.9 Contractor Manpower Reporting: The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Engineering Data Support Services in a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

4.9.1 Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY) which runs 1 October through 30 September. While inputs may be reported anytime during the FY, all data shall be reported no later than 31 October of each calendar year.

Contractors may direct questions to the CMRA help desk.

4.9.2 Reporting Period: Contractors are required to input data by 31 October of each year.

4.9.3 Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

4.9.4 User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the CMRA link at http://www.ecmra.mil (select Dept of the Air Force option for AF requirement).

4.10 Contract Manager. The Contractor shall provide a program manager and alternate who shall be responsible for the performance of the work and shall have full authority to act for the contractor.

Immediately after contract award, the Contractor shall notify the CORs and On-Site SME of the names and phone numbers of these personnel.

DRIPPING ONTO THE GROUND.

E. PUMP OUT THE REMAINING WATER IN EACH CHAMBER.

B. When changing hepa filters apply a thin coat of petroleum jelly to the rubber seals. This will help with the installation and removal.

3rd Stage Hepa Filter P/N 240‐010‐19 4ea.

3. TUB SCRUBBER:

A. Inspect the tub scrubber exhaust for sufficient air flow and any unusual odors. Change the Purakol pellets as needed.

B. Dispose of contaminated pellets as hazardous waste.
