About this file
This is a standards of conduct and security acknowledgement form issued by the Ohio Attorney General's Office Information Technology Services division for non-interactive or service account users requesting access to AGO Products and Services. The document establishes mandatory security policies and user responsibilities for individuals and their employers who require access to AGO systems, data, and network services including FTPS, email, databases, applications, and user account management. The acknowledgement applies to users accessing sensitive data classifications such as Personally Identifiable Information (PII), Criminal Justice Information (CJI), Protected Health Information (PHI), and other confidential data as defined in governing contracts or memoranda of understanding. Users must comply with specified password requirements including a minimum 20-character length with three of four character types, 365-day maximum password age, and account lockout protocols after three failed login attempts. The form requires completion of user information including printed name, title, employer, contact details, requested access period, specific applications or resources needed, and two unique identity control information entries for account verification purposes.
The acknowledgement document imposes strict security obligations on both users and employers, including prohibitions against sharing credentials, monitoring unauthorized communications, circumventing security systems, and copying or disseminating AGO data without authorization. Users must report security events immediately to their employer's IT department, which must then notify the AGO of any potential compromises. Employers acknowledge responsibility for ensuring computer security through endpoint protection, perimeter defense, audit logs, physical security measures, and incident response policies. The document explicitly states that user access may be terminated immediately for policy violations and that users remain personally liable under applicable federal and state laws including IRS Publication 1075, Ohio Revised Code Chapter 1347, HIPAA, HITECH Act, and CJIS Security Policy. Inactive accounts are disabled after 60 days and removed at 120 days, requiring users to reapply for access. Both the user and an authorized employer representative must sign the acknowledgement, with the AGO Chief Information Officer or Chief Information Security Officer providing final authorization and assignment of system access rights.
