About this file
This document is an Administrative Site Monitoring Form for the Ohio Department of Youth Services (Ohio DYS) Personal Responsibility Education Program (PREP). The form outlines a comprehensive monitoring process for subgrantees, which includes an announced administrative site visit to examine current year compliance with PREP funding requirements. The monitoring visit aims to ensure subgrantee contract compliance and provide ongoing technical assistance to develop high-quality and effective PREP programs. During the site visit, Ohio DYS will review documentation from the past year, including fiscal and programmatic records, and conduct a program observation to monitor curriculum implementation quality. Additionally, the department will hold monthly subgrantee team calls to discuss program progress and updates.
The monitoring form covers multiple aspects of program management, including grants management, budget oversight, personnel documentation, training and program fidelity, community partnerships, data collection, and quality improvement. Key areas of review include verifying the accuracy of fund balances, ensuring expenditures match approved budgets, checking personnel background checks, documenting staff time and duties, reviewing sub-contractor agreements, and assessing the subgrantee's ability to track attendance and administer entry/exit surveys. The form provides a structured approach for Ohio DYS to evaluate PREP program implementation, with checkboxes and sections for documenting findings, identifying promising practices, noting areas of concern, and determining any technical assistance needed to support program success.
