1. DESCRIPTION OF SERVICES. The contractor shall provide an individual to perform professional health, wellness and fitness programs and services to all Government beneficiaries, including active duty military, family members, retirees and DoD civilians and is responsible for developing, planning, organizing, coordinating, implementing, conducting, tracking and evaluating comprehensive preventative and functional fitness programs and services as required by AFI, guidelines and population needs.

1.1. Scope of Work. The Exercise Physiologist (EP) will provide installation level subject matter expertise on physical activity programs to all government beneficiaries including active duty military, family members, retirees, and Department of Defense (DoD) civilians.

1.2. EP will assess the fitness level of the base population using sound epidemiological principals. Uses evidenced based exercise and behavior change theories to develop, coordinate, implement, conduct, track and evaluate a comprehensive integrated and effective fitness programs aimed at fitness improvement, human performance, disease intervention and prevention.

1.3. EP will administer fitness assessments, fitness improvement programs, exercise prescriptions, medical exercise/post rehabilitation fitness programs, pre/postnatal fitness programs, preventative services and other health and fitness programs and activities that meet the community needs.

1.4. EP will assist commanders in development, analysis, modification, and formulation for implementing, conducting, administering, managing, and enhancing the operations of the AF FP. Provides consultation to unit commanders on developing and implementing physical activity programs to maximize Airman physical performance. Performs unit fitness quality checks as requested.

1.5. EP will provide training and support to units to administer and implement effective physical fitness training programs. Trains and provides ongoing technical support to designated unit Physical Training Leaders in the areas of safety and injury prevention. EP will provide health education expertise, consultancy and advocacy to support a comprehensive AF FP to include: EP Will Maintain certification to assist Fitness Assessment Staff instruction by instructing PTL courses, First Aid/CPR/AED classes and training Fitness Center staff on Fitness Assessment procedures. EP will regularly ensure training for Physical Training Leaders follow safe unit fitness training/group exercise techniques. For consistency of Fitness Assessment staff EP will use the specific training channels for instructor certification as the Fitness Assessment Staff to include American Red Cross and/or American Heart Association provided by 72nd Medical Group Training Flight.

1.6. EP will design, develop, and establish appropriate administrative and management procedures and policies to ensure the highest professional level of fitness programming within the community. Conducts review of current research in the field of fitness and health and is able to interpret and apply as based on community needs. Serves on committees and working groups, per referenced AF written instruction related to fitness, to inform and involve other health professionals and installation representatives in community fitness and health promotion issues.

1.7. EP will conduct worksite physical activity education on various topics to include occupational exercise opportunities to enhance performance of Air Force Specialty Code duties.

Develops and instructs periodic educational modules on various topics to Fitness Center staff at Fitness Center Director’s request. Develops and coordinates base-wide program publicity and marketing media for all physical activity initiatives.

2. GOVERNMENT-FURNISHED PROPERTY.

2.1. The Government shall provide the facilities, equipment and materials (desk, chair, phone, computer, printer, monitor, office supplies and all fitness equipment) necessary to perform work under this contract IAW AFI 23-111 Section B. A joint equipment inventory will be conducted by the Fitness Assessment Cell Manager and the EP on the first day of duty and the last duty day. The EP will be responsible for maintaining all equipment listed above in good working order Problems with the equipment will be reported to the immediate supervisor immediately. Items of equipment missing or not in working order shall be recorded and the contracting officer notified in writing. The contractor shall be liable for loss or damage to government furnished property beyond normal wear and tear.

2.2. Government Furnished Services.

2.2.1. Government-Furnished Utilities. Electricity, water, sewage, heating, and air conditioning.

2.2.2. Postal/Installation Distribution. The government will furnish normal distribution for official government business only.

2.2.3. Telephone Services. The Government will provide telephone service. Phone use shall be limited to matter related to the performance of this contact. All other telephone requirements shall be at the contractor’s expense.

2.2.4. Custodial Service. The Government will provide custodial service to the extent provided in the Base Custodial Contract(s) for the facilities provided.

2.2.5. Refuge Collection. The EP is responsible for transporting individual trash to dumpster or centralized collection point, if applicable.

2.2.6. Maintenance. The government will provide maintenance for all government-furnished equipment.

2.2.7. Equipment Maintenance. The contractor shall notify the 72d ABW Communications Squadron for all equipment owned by the Government and provided to the contractor in need of repair or maintenance; the contractor shall notify the government supervisor upon discovery. Response time(s) for these repair or maintenance requests will be subject to base wide priorities.

2.2.8. The Government will provide fire prevention and protection, inspection and maintenance of Government-Furnished fire extinguishers and systems. Fire Department telephone extension is 911 for emergencies.

2.2.9. Emergency Medical Service. The Government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract. The contractor shall reimburse the Government for the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.

2.2.10. Security. The Government will provide general on-base Security Forces service.

Security Forces phone extension is 911 for emergencies (from a base land line), and 846- 7926/7913 for routine calls.

2.3. The government will provide access to government on line training for contractor access to government computers and/or equipment.

2.4. The EP will be required to take the following recurring training.

DoD Information Assurance Awareness Force Protection Human Relations Information Protection Operations Security (OPSEC) First Aid CPR/AED Instructor Training(American Red Cross)

3. GENERAL INFORMATION

3.1. Duty Hours. EP duty hours can be flexible Monday-Friday, excluding federal holidays.

The following federal holidays are observed:

January 1 New Years Day

Last Monday in May Memorial Day

July 4 Independence Day

1st Monday in September Labor Day

2nd Monday in October Columbus Day

3.2. Records. Records. The EP shall maintain records generated by the Fitness Assessment Cell in accordance with AFI 33-322, Records Management Program, dated 4 Jun 12. The EP shall assist Fi tness Assessment staff in coordinating record keeping actions with the Base Records Manager. The EP shall manage all records created for Government use in accordance with records management procedures outlined in AFI 33-364, Records Disposition Procedures and Responsibilities, dated 22 Dec 06. All records containing Privacy Act material when collecting, safeguarding, maintaining, using, accessing, amending, and disseminating personal information in a system of records, shall be protected as such in accordance with AFI 33- 332, Privacy Act Program, dated 16 May 11. The E P will attend records training with 90 days of being hired.

3.3. Access to Base Network. The following actions will take place immediately upon reporting for the first duty day: Contractor personnel will require access to the Air Force network and shall complete a National Agency Check (NAC) verified through the Joint Personnel

Adjudication System (JPAS). This is accomplished by completing the Electronic Personnel Security Questionnaire (EPSQ) or if applicable, completion a SF 85P, Questionnaire for Public Trust Positions. Personnel shall submit all required forms through the base’s Security Forces Squadron for submission to the Office of Personnel Management (OPM) in accordance with AFI 31-501, Personnel Security Program Management, dated 27 Jan 05 paragraph 3.24. Upon receipt of proof that contract personnel forms have been submitted to OPM, the commander responsible for the contract can grant interim Local Area Network (LAN) and e-mail access in accordance with AFI 31-501, paragraph 3.27. All personnel requiring access shall comply with the requirements of AFI 33-200, Information Assurance (IA) Management, dated 23 Dec 08 paragraph 3.38, AFI 33-115 V2, Licensing Network Users and Certifying Network Professionals, dated 14 Apr 04 paragraph 5. All contract personnel shall complete initial training in the Information Assurance Awareness Program (IAAP) and perform all refresher training as required.

3.4. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS. IAW DoDI 1100.22 (Policy and Procedures for Determining Workforce Mix), and any applicable Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that all services and procedures identified in this PWS are not mission essential during a crisis

3.4.1. Safety. The EP shall perform work in a safe manner as required by OSHA safety and health requirements. The E P shall operate all government equipment in a safe manner at all times. Report all unsafe conditions to immediate supervisor immediately. The Contractor shall provide a verbal report to the Contracting Officer and Government Supervisor as soon as possible, but no later than 12 hours for each occurrence of damage to Government property or illness resulting in calling 911 or for each accident resulting in death, injury, occupational disease, or adverse environmental impact. The Contractor shall provide a completed copy of required Accident Investigation Reports to the Contracting Officer and copy to supervisor within one (1) calendar day of each occurrence. An Accident Investigation Report shall not be submitted for illness.

3.4.2. In the event of an accident, the contractor shall take reasonable and prudent action to establish control of the accident scene. If the Government elects to conduct an investigation of the accident, the contractor shall cooperate fully and assist Government personnel until the investigation is complete.

3.5. Education Requirements. The EP shall have a Master of Science in Health and Physical Education with 4years of experience as an Exercise Physiology on a military installation.

The contractor shall ensure the EP meets this requirement and have the ability to provide prove to the government if asked.

100% Inspection will be conducted by the Chief, A&FRC on a monthly basis.

2a. Perform task without error.

Task is performed without errors 85% of the time. The remaining 15% is reworked until error free.

Task is performed within five minutes time 90% of the time. The remaining 10% is coordinated with the Government and performed within 10 minutes.

Government supervisor observations reported to the FAC Manager on a monthly basis.

3. Policies and Procedures 1.2.5, 1.2.6, 1.2.7

Contractor shall abide by written Policies and Procedures regarding the work area 100% of the time.

All written or verbal customer complaints will be reviewed monthly by the FAC Manager.

4.2. Criteria for Evaluating Performance. Performance of service will be evaluated to determine whether it meets the performance objectives of the contract. When the Performance Thresholds are not met, the FAC Manager will notify the EP’s contract supervisor of the deficiencies for correction. The contractor shall be given appropriate time to correct the deficiencies. If deficiencies are not corrected within the required time frame, the FAC Manager, should notify the CO for action. If the contractor disagrees with the discrepancy and an agreement cannot be reached with the FAC Manager the CO shall be notified for a final decision.

4.3. Quality Control. The contractor shall establish and maintain a complete Quality Control Plan (QCP) to ensure the requirements of the contract are provided as specified. One copy of the contractor’s Quality Control Plan shall be provided to the contracting officer within five calendar days after contract award. Once the QCP is accepted in writing, the contractor shall provide an additional copy to the FAC Manager. Updates to the QCP shall follow the same protocol.

4.4. A description of records to be kept to document inspections and corrective or preventive actions taken. This documentation shall be made available to the government throughout the term of the contract.

4.5. Quality Assurance. The government intends to observe the contractor's performance under this contract. When an observation indicates unacceptable performance, the FAC Manager shall notify the contract manager or representative at the site.
