EXERCISE PHYSIOLOGIST

This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

This solicitation is being issued as a request for quotation (RFQ) IAW FAR Part 13.

This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20161222, and Air Force Acquisition Circular (AFAC) 2017-0127.

Contractor is required to sign this document and return 1 copy to issuing office. Contractor agrees to provide all services set forth or otherwise identified above and on any additional sheets subject to the terms and conditions specified herein.

The Air Force Sustainment Center (AFSC/PZIOA) at Tinker Air Force Base Oklahoma, intends to award one firm fixed price contract for the acquisition of the service listed below:

Description of Items:

The contractor shall provide an individual to perform professional health, wellness and fitness programs and services to all Government beneficiaries, including active duty military, family members, retirees and DoD civilians and is responsible for developing, planning, organizing, coordinating, implementing, conducting, tracking and evaluating comprehensive preventative and functional fitness programs and services as required by AFI, guidelines and population needs.

Delivery See Period of Performances listed above.

Terms and Conditions FAR 52.212-1. Instructions to Offerors – Instructions to Offerors -- Commercial, applies to this acquisition.

FAR 52.212-2 Evaluation Commercial items, Evaluation -- Commercial Items, (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal (Only complete and return if there are changes to current SAM Registration. If 52.212-3 is not returned the contractor is affirming that the current SAM registration is current, accurate, and complete). If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision.
