USMS MEDICAL OVERSIGHT PROGRAM FOR SPECIAL OPERATIONS GROUP

STATEMENT OF WORK

MEDICAL OVERSIGHT FOR SPECIAL OPERATIONS GROUP

A. BACKGROUND

The United States Marshals Service (USMS), a component of the Department of Justice (DOJ), is actively engaged in enforcing the nation’s laws and executing federal court orders. The USMS locates, pursues and arrests state and local fugitives under legislative acts and brings them before the criminal and civil justice systems.

USMS’ mission is to defend the Constitution of the United States through protection of the judiciary and the judicial process. This law enforcement responsibility is carried out through performance of the following functions:

· Providing court security

· Safekeeping protected witnesses of the court

· Apprehending fugitives trying to escape justice

· Ensuring prisoners are securely and humanely confined

· Producing prisoners for court appearances and judicial-related activities in a timely fashion

· Executing court orders to seize and dispose of detainee forfeited assets

SOG is a specially trained and equipped unit deployed in high risk law enforcement situations. SOG’s secondary mission is the tactical support of USMS initiatives during national emergencies, civil disorders, natural disasters, or as ordered by the Attorney General.

B.

OBJECTIVE

The purpose of this acquisition is to acquire medical direction and oversight for SOG’s Operational Medical Support Unit (OMSU).

C.

SCOPE

The Contractor shall provide all administrative, management, supervision, labor, and other services as they pertain to this Statement of Work (SOW). Tasks to be performed may include, but not be limited to, the following:

C.1 Conduct two (2) 40-hour Emergency Medical Technician (EMT) refresher classes per year that meet the 24-hour National Registry of Emergency Medical Technicians (NREMT) guidelines. Each EMT refresher class shall include the following:

· 16 hours of continuing education learning, including a CPR recertification course for OMSU members.

· Instruction by a physician at least 70 percent of the time. Other instructors shall be either a mid-level provider (NP/PA), nurse, or advanced paramedic.

· Expanded scope including, but not limited to, airway procedures (oropharyngeal and nasopharyngeal airways, Combitube device, King airway device and surgical cricothyroidotomy); needle procedures (peripheral intravenous access and fluid therapy, intraosseous access and needle thoracosotomy); medication administration (oral, sublingual and intravenous); hemorrhage control (tourniquet application and topical hemostatic agents); and wound care (local anesthetic administration, tissue adhesive application, wound closure-stapling and wound closure-suturing).

· Cadaver lab learning opportunities for anatomy and procedure training.

C.2 Provide oversight for SOG medics, as follows:

· Approximately ten (10) SOG medics must be authorized to carry approximately 30 medications ranging from over-the-counter medications to narcotics.

· Possess a valid DEA license and the ability to procure narcotic medications for SOG medics for official use.

· Train and update SOG medics in the latest advancements in tactical and operational medicine.

C.3 Provide an Emergency Medicine Physician on-site for the first five (5) days of the annual SOG Selection in Pineville, Louisiana. SOG Selection is a 30-day high intensity selection of SOG personnel held at least once every two (2) years. The physician shall lead the on-site medical pre-screening of invited candidates.

C.4 Provide clinical training for EMTs in an accredited Level 1 adult and pediatric trauma center that includes emergency department and operating room rotations. The OMSU EMT will observe and practice skills taught in an emergency and operating room setting while being overseen by a nurse or physician.

C.5 Provide indoor and outdoor facilities and space for dynamic tactical medicine scenarios instructed by trained tactical medical experts to include physicians, nurses, and paramedics. Training shall accommodate the use of both live and simulated ordnances and available for both daylight and nighttime training at the Contractor’s facility.

C.6 Provide an Emergency Medicine Physician 24 hours per day, seven (7) days per week, for real-time, on-line medical consultation via telephone with OMSU medical personnel. OMSU providers must be able to speak to the Emergency Medicine Physician within five (5) minutes of the initial call. Consultations must be through an existing communications center with live dispatch personnel.

C.7 Develop, implement, and oversee the USMS electronic patient care record and reporting system.

C.8 Develop and oversee agency-wide treatment protocols which shall be provided to all OMSU medics in both hard copy and electronic format.

C.9 Consult with OMSU personnel on the development and operation of a functional telemedicine program.

C.10 Provide written pre-deployment medical briefings and advanced medical threat assessments for domestic and international travel for USMS and SOG missions/deployments.

D.

MEETINGS

All meetings with Contractor personnel shall take place at the USMS Technical Operations Division in Springfield, Virginia or at the Contractor’s facility located within 60 miles of USMS Headquarters in Arlington, Virginia.

E.

PERIOD OF PERFORMANCE

The period of performance shall be from date of award through September 30, 2010 (base period), plus four (4) one-year option periods.

F. PLACE OF PERFORMANCE

All work shall be performed at the Contractor’s facility, unless otherwise specified herein.

G. TRAVEL

The Contractor shall provide a physician trained in tactical medicine to travel domestically and internationally for operational support, as needed, approximately 10 days per year, which includes the five (5) days for SOG Selection. USMS will provide lodging and meals for SOG Selection at no cost to the Contractor. All other travel will be reimbursed according Federal Travel Regulations.

H.

PERSONNEL QUALIFICATIONS

The Emergency Medicine Physician must possess the following mandatory qualifications:

· State licensed

· Board certified

· Medical director

· Documented experience managing a tactical medicine program I.

OTHER MANDATORY REQUIREMENTS

I.1 The Contractor’s facility must be located within 60 miles of USMS Headquarters in Arlington, VA.

I.2 The Contractor must possess classroom facilities for at least 30-40 students with audio and video equipment pre-installed. Classrooms must be equipped with computers, Internet access, MS Power Point capability, television, simulation room, and conference space.

I.3 The Contractor must possess a dedicated Special Operations-type program with formal experience providing medical direction and operational support to federal and/or state law enforcement agencies.

I.4 The Contractor must possess faculty physicians and medical staff experienced in law enforcement and tactical operations.
