Offeror shall provide its references with the attached letter and questionnaire.
References are to complete the questionnaire and submit it directly to the Contracting Officer before the due date of the Request for Quotes. Questionnaires submitted by the Offeror will not be considered.

SUBJECT: Request for Current/Past Performance Information Solicitation #DE-SOL-0008317: RMR Administrative and Warehouse Support Services
The Western Area Power Administration (Western) will be evaluating quotes received in response to the subject solicitation for contractor to provide Administrative and Warehouse Support Services for Western’s Rocky Mountain Region. One of the integral considerations in the evaluation is the verification of the offeror’s past and present performance on contracts that reflect the offeror’s ability to perform on the proposed effort. We depend on information received from agencies/firms such as yours, which have had first-hand experience with the offeror, for the evaluation of their performance.
Our areas of interest in the offeror are summarized in the enclosed questionnaire.
You are urged to submit your comments on other similar contracts your entity has/had with the offeror. Any additional information that you think may benefit the Government in our evaluation of the offeror's quote is appreciated. Please note that we may contact the offeror on problem areas or concerns identified, to determine whether corrective actions have been taken or are in process. The offeror is required by our solicitation to provide a point of contact for each reference provided to the Government. The name of the individual completing the questionnaire will not be disclosed to the offeror.
Due to time constraints, it is imperative that the questionnaire be completed and returned no later than May 28, 2015 to Anne Anderson, Contracting Officer, via e-mail at aanderson@wapa.gov .

When completing the Questionnaire, please use the following definitions as a guide:
Exceptional (E) Performance meets contractual requirements, and exceeds many, to the Agency’s or Firm’s benefit. The contractual performance of the services was accomplished with no problems or few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good (G) Performance meets contractual requirements, and exceeds some, to the Agency’s or Firm’s benefit. The contractual performance of the services was accomplished with no problems or some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory (S) Performance meets contractual requirements. The contractual performance of the services was accomplished with some problems for which corrective actions taken by the contractor appear satisfactory.
Marginal (M) Performance does not meet some contractual requirements. The contractual performance of the services reflects problems for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory (U) Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the services contains serious problem(s) for which the contractor’s corrective actions do not appear to be effective.
Unknown (N/A) Does not apply to your contract, or you did not observe performance in this area.
Please respond to the items below regarding this contractor’s past performance by checking the appropriate box. For all ratings, please provide a narrative explaining how the contractor either exceeded, or failed to meet, your requirements. Unsatisfactory Contractor. We would not want to contract with this firm again under any circumstances.
Marginal Contractor. Numerous problems developed that were a result of their lack of cooperation and failure to perform work as required.
Satisfactory Contractor who met the minimum requirements of the contract.
Performance deficiencies, if observed, improved when identified by the Government.
Rework/warranty issues, if applicable, were performed; however, an aggressive inspection program was required to ensure compliance.
Very Good Contractor with whom we would not hesitate to enter into another contract. Problems encountered were minor and solutions were found with little difficulty.
Exceptional Contractor in every respect. Problems were solved in a spirit of teamwork. Quality work, timely service, and complete documentation were routinely achieved. We would pay premium prices to contract with them again.
