About this file
This is a Construction Bid Submission Review Form and supporting documentation issued by the State of West Virginia Department of Environmental Protection (DEP) for RFQ DEP23-27 McCoy Well Plugging. The document serves as an informational guide for potential bidders and includes mandatory bidding requirements, submission procedures, and required documentation. The form emphasizes that all prospective bidders must review the complete Request for Quotation and all associated bid documents in their entirety to understand project requirements and bidding procedures. Failure to comply with stated requirements may result in bid disqualification. The document does not specify project quantities, response dates, bid opening dates, site visit requirements, or mandatory bidders' conferences, leaving these details to be contained in the full RFQ documentation.
Key disqualification criteria include failure to attend a mandatory pre-bid meeting, failure to supply a valid bid bond, non-compliance with mandatory RFQ requirements, failure to submit bids prior to the closing deadline, federal or state debarment status, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additional requirements that must be resolved prior to contract award include outstanding state debt, workers' compensation or unemployment compensation delinquency, and failure to obtain required bonds and insurance. Bidders must submit a West Virginia contractor's license number with their bid or within one business day of DEP request, provide a signed drug-free workplace affidavit, and furnish a subcontractor listing within one business day of bid opening. The bid bond must equal a minimum of five percent of the total bid amount and must be accompanied by a dated power of attorney with surety seal. Contractors must also submit certified drug-free workplace reports annually or upon project completion with specified content requirements.
