Statement of Work (SOW) Psycho-Educational Assessments Services
Seba Dalkai Boarding School
1. BACKGROUND
The Non-personnel services shall ensure that Seba Dalkai Boarding School is meeting requirements of the Individual with Disabilities Education Act (IDEA). IDEA requires a free appropriate public education to eligible children with disabilities throughout the nation and ensures special education and related services to those children. Specifically, the non-personnel services are intended to help Seba Dalkai Boarding School meet all requirements for conducting and documenting the evaluations described in 34 CFR Subpart D, as well as preparation and maintenance of each Individual Education Program (IEP) for qualifying students.
2. SCOPE
Contractor shall provide qualified personnel, equipment, materials and supplies to perform student Psycho-Educational Assessment Services. Students receiving this service are those requiring 3 years re-evaluation or newly referred students who may be disabled. The individual psycho-educational assessment service will include:
a. The administration of a battery of tests chosen in collaboration with the Special Education Department.
b. The scoring of all test/assessment administered.
c. The interpretation of all test/assessments results.
d. The provisions of a comprehensive written report describing the results of the assessment to the school within 30 days of the evaluation.
e. Documentation that parents and staff receive a copy of the individualized report.
f. Availability of the psychologist to participate in MDET/MDT/IEP meetings where the results of the testing are interpreted to parents/guardians and staff within 60 days of the evaluation.
g. Follow the state evaluation requirement format.
The individualized written psycho-educational report will include:
a. Basic student data as furnished by the referral packet at Seba Dalkai Boarding School.
b. Diagnostic statement categories listed in 34 CFR.
c. Reason for referral.
d. Background information; educational status and history, health assessment, academic assessments, classroom work/documents, and prior evaluation history.
e. Behavioral Observations.
f. Assessments as requested on permission to evaluate with may include:
o Intelligence o Academic achievement o Social behavior o Perceptual abilities
2 | P a g e o Emotional Behavior o ADHD o Vision & Hearing assessments o Other assessments relevant to the performance of the individual child
g. Summary Report
h. Recommendation for Placement to include instruction, social, medical, etc.
Services shall be provided and coordinated with the school for approximately 160 hours calculated to be scheduled over 40 weeks. Service hours may increase or decrease based on student evaluations and will be scheduled and coordinated on an on-going basis.
3. KEY PERSONNEL MINIMUM REQUIREMENTS
The contractor shall provide key personnel that meet or exceed the following minimum requirements.
Evidence of meeting these requirements shall be provided to the Government with the Contractors’ quote/proposal.
Education: Masters degree in Psychology # of Years Directly Related Professional Experience: 5 years experience with Native American students is required.
Certification: National Certified School Psychologist (NCSP) preferred License(s) Arizona State License (License in state where services are to be provided Security Requirements: New consultants shall comply with Background investigation as required prior to commencement of any work to be performed. 25 – Chapter 32-Section 3201 (P.L. 101-630);
Contact with or control over Indian children are subject to a federal background investigation and determination of suitability for employment. Contractor personnel shall present government PIV issued identification card to the front desk and sign-in with the School Administrative Office prior to entering the school building/classroom.
All Contracting members are required to complete background check before onsite services.
Replacement of key personnel will require prior coordination and approval of the Seba Dalkai Boarding School.
4. DELIVERABLES
The contractor shall provide the following deliverables within the specified timeframe.:

5. PLACE OF PERFORMANCE
Contractor shall provide services at Seba Dalkai Boarding School which is located 35 miles north of Winslow, Arizona. Cost for travel and per diem are the responsibility of the contractor, the firm-fixed-price shall include these amounts for scheduled trips in Price Summary.
6. PAST PERFORMANCE ASSESSMENT
The School Administrator along with certified Special Education teacher will evaluate the Contractor’s performance under this contract, which will be shared with BIA contracting agencies and used as past performance references.
7. INVOICING REQUIREMENTS
All invoices shall follow the Three Way Match Process is method of matching the vendor or supplier invoice to the purchase order and getting confirmation of this match with goods/services received note before issuing payment.
