Statement of Work (SOW) Psycho-Educational Assessments Services
Rocky Ridge Boarding School
Psychoeducational Assessment/Evaluation
Scope of Work (SOW):
1. The contactor shall provide Psycho-Educational Assessments Services to students who are identified to receive this service. Student receiving this service will be disabled requiring re-evaluations or newly referred students requiring initial evaluations who may be disabled.
2. The Psycho-Educational Assessment Services will be in accordance with P.L. 105-17 and 34 CFR, Part B, 25 CFR 45, #1-80 and Section 504 of the Rehabilitation Act of 1073, 29 USC 794.
3. The Individual Psychoeducation Assess Services will include:
a. The administration of a battery of tests chosen in collaboration with the Special Education
Department.
b. The scoring of all tests administered.
c. The interpretation of the test results.
d. The provision of a Comprehensive written report describing the results of the assessments to the school within 30 days of the parent’s permission to evaluate.
e. Documentation that parents and the school received a copy of the individualized report.
f. Will be available to participate in MET/MDT/IEP meetings and interpret results of the testing for parents and staff within 60 days of the parent’s signed permission to evaluate.
g. Follow BIE Evaluation Forms.
h. Provide current state certification to to Rocky Ridge Boarding School NASIS Registrar.
4. The Individualized written Psycho-Educational Assessment Report will include:
a. Basic student data as furnished by the referral packets from the Special Education Department, Rocky Ridge Boarding School.
b. Diagnostic statement categories listed in34 CFR and by ISEP.
c. Reason for referral
d. Background information: educational status, history, health assessment and prior evaluation history
e. Behavioral observations
f. Assessment of academic behavior
g. Assessment of social behavior
h. Assessment of perceptual abilities,
i. Assessment of emotional behavior
j. Summary report
k. Recommendations for placement and instruction
5. The Individualized Written Psycho-Educational Assessment Report may include the following:
a. Weschler Intelligence Scale of Child or similar Intelligence Assessment
b. Woodcock-Johnson Psycho-Educational Battery Achievement
c. Wide Range Achievement Test or similar appropriate test
d. Goodenough-Harrs Drawing Test
e. Developmental Test of Visual Motor-VMI-A and
f. AADM Adaptive Behavior Scale, when necessary or other assessments as deemed necessary based on individual needs of the students.
6. The Contractor will:
a. Make twenty visits for consultation for the contract period SY 2023-2024. The school psychologist will arrange to meet with the MDT or IEP Committees for annual review of re-evaluations, and other concerns.
b. Follow BIE Evaluation Requirements
c. Attend all school IEP and MDT meetings to review documents and provide professional input concerning the service delivery for psychological evaluations at these meetings.
7. Scheduling will be determined by weather conditions, school testing periods, school-based activities, as well as other unforeseen circumstances.
8. Service Summary:
• Contractor(s) shall sign in and out at front office for attendance and cross referencing
• Invoice shall contain the following information
• Contractor shall submit a monthly-summarized invoice
• Invoice shall contain the following information: o Purchase Order Number Date of Service
KEY PERSONNEL MINIMUM REQUIREMENTS:
The contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractors’ quote/proposal.
Education: Masters degree in Psychology # of Years Directly Related Professional Experience: 5 years experience with Native American students is required.
Certification: National Certified School Psychologist (NCSP) preferred License(s) Arizona State License (License in state where services are to be provided Security Requirements: New consultants shall comply with Background investigation as required prior to commencement of any work to be performed. 25 – Chapter 32-Section 3201 (P.L. 101- 630); Contact with or control over Indian children are subject to a federal background investigation and determination of suitability for employment. Contractor personnel shall present government PIV issued identification card to the front desk and sign-in with the School Administrative Office prior to entering the school building/classroom.
All Contracting candidates are required to complete background check before onsite services.
Replacement of key personnel will require prior coordination and approval of the Rocky Ridge Boarding School.
