Homeless Services Scorecard Overview
The Homeless Services Scorecard displays VAMC, VISN and national-level performance data for VHA Homeless Programs by month, quarter, and year.
• The “Drill Down” reports provide more detailed information for each measure, including Veteran SSN-level data and program performance data. To access these reports, click on the measure’s name, then select your VISN and site.
• The “Chart Builder” feature provides you with the ability to create customized charts based on any Scorecard metric.
• “Stop Light” coding for the scores and ranges are included with each measure’s description.
o Green = meeting or exceeding the target.
o Yellow = below the target but within reach.
o Red = performance is significantly below the target and the site is at risk of not being able to meet the target by year’s end.
• A measure will not display on the Scorecard if it is not applicable to a particular site.
For example, a site that does not have a CWT/TR program will not have these measures included on its scorecard.
Quick guides and other technical assistance resources are available on the Homeless Programs Operational Planning Hub. Please address any questions or concerns regarding the Scorecard and performance measures to the Homeless Programs Support Team via the HOMES Help Desk, available on the HOMES Main Page. For additional information on using the Help Desk, please review the HOMES Help Desk User Quick Guide.

INDICATOR DESCRIPTION
Percentage of VHA Homeless Program Office-funded positions that are currently filled.
POPULATION
N/A
TARGET AND BUSINESS RULES
The FY 20 target for this measure is 90%
ESCALATING TARGETS AND STOP LIGHT FORMATTING
90% or above (Green), 89% or below (Red)
NUMERATOR
Total number of positions indicating a “filled” status in the Mental Health Initiatives Reporting System (MHIRS, also known as the “Homeless Staffing Database”).
DENOMINATOR
Number of positions funded by the VHA Homeless Programs Office in FY 2020.
o Data source: MHIRS.
DENOMINATOR EXCLUSIONS
The following positions are not included in the denominator:
1. VAMC-funded or VISN-funded positions providing homeless services at the VAMC or VISN, for which no VHA Homeless Program Office funding has been issued.
2. Positions listed with a “0” FTE in the database (contract positions) are excluded from the denominator.
3. Positions funded during FY 2020 that have not been active for at least 90 days.
RATIONALE
The hiring of homeless program positions is essential to VHA’s initiative to end homelessness among Veterans. Position vacancies result in reduced services for homeless Veterans and inefficient use of resources. This measure is intended to provide VACO, VISNs, and VAMCs feedback on hiring rates within their homeless programs and to reinforce hiring efforts.
OP1: Percent Hired
Page: 4 of 49 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2019
This measure is a point-in-time measurement of the percentage of Housing and Urban Development-VA Supportive Housing (HUD-VASH) vouchers issued to the VAMC that result in housing.
POPULATION
Veterans currently in HUD-VASH housing.
TARGET AND BUSINESS RULES
The target for September 30, 2020 is 92%.
ESCALATING TARGETS AND STOP LIGHT FORMATTING
92% or above (Green), 91% or below (Red)
NUMERATOR
Number of Veterans currently in HUD-VASH housing. Veterans who have signed a lease and moved into an apartment unit or house resulting in a homeless Veteran achieving resident status and no longer being counted as homeless. This includes Veterans or their family members who are graduated from HUD-VASH case management services and have retained a HUD-VASH voucher. This is a point-in-time measure. Therefore, if a Veteran is housed at the end of a reporting period, the Veteran will be included on the Veteran-level report in the numerator for the reporting period.
o HOMES data source: Most current HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.
o ODC data source: Legacy Housing Progress Report 2 (HPR2) item 15 has a date entered and Legacy Case Management Progress Report (CMR) items 6 and 8 are entered as option 1 and item 20a is entered as option 3.
Veterans who have been discharged from HUD-VASH case management with a HUD-VASH voucher or have a family member who continues to use the voucher after discharge are included in the numerator:
o HOMES data source: HUD-VASH Exit Form, item 6 has option 1 or 2 entered, or item 5 has option 12 and 5a has option 1.
o HOMES data source: HET, Section III Voucher Status, item 2 = 9.
Veterans housed at the end of each reporting period are included on the Veteran-level report and in the numerator the reporting period. If the Veteran leaves housing, the Veteran is not included during the month the Veteran was not housed and is removed from the YTD numerator. In all cases, date of move out must be considered. When a subsequent move-in date is documented the Veteran is considered re-housed and will be counted during the move-in month. Veterans are not housed if the following is documented in HOMES:
o HOMES data source: HUD-VASH Exit Form, item 4 has a date entered and item 6 is entered as option 3 – 9.
o HOMES data source: HUD-VASH HPF Form, item 14 = 1, 2, 3, 5, 6, 7, or 8 and 14a has a date entered.
o HOMES data source: HET, Section III Voucher Status, item 1 has a date entered and item 2 = 1, 2, 5, 6, 7, or 8.
NUMERATOR EXCLUSIONS
1. Veterans are excluded from the numerator if they are indicated as deceased on the HUD-
VASH Exit Form or VA enrollment file.
HMLS3: Percent Housed in HUD-VASH
Page: 5 of 49 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2019 o HOMES data source: HUD-VASH Exit Form item 5, option 12, or the VA enrollment file, unless exit form item 5a indicates that the voucher will be used by another household member (option 1).
2. Any legacy records for Veterans that are in the numerator via HOMES data are excluded.
3. Using a matching procedure between VA and HUD databases, Veterans previously counted in the HMLS3 numerator who had been discharged from case management services with a HUD-VASH voucher and are no longer housed in HUD-VASH according to HUD’s data systems are removed from the numerator. This matching procedure will occur on a monthly basis.
4. Veterans previously counted in the numerator who are no longer housed according to HOMES data.
o HOMES data source: Housing Progress Form (HPF), item 14 = 1, 2, 3, 5, 6, 7, or
8 on the most recently submitted HPF, indicating that the Veteran is no longer housed in HUD-VASH as result of a change in voucher status, and item 14a has a date entered.
o HOMES data source: HUD-VASH Exit form indicates that the Veteran or family member exited without a voucher, item 4 has a date entered, and item 6 = 3-9 or 5a=0.
o HOMES data source: Section III of the Housing & Employment Tracker (HET) = 1, 2, 5, 6, 7, or 8, indicating that the Veteran is no longer housed in HUD-VASH as result of a change in voucher status. This rule only applies if the Veteran does not have an open admission in HUD-VASH Case Management.
DENOMINATOR
Number of vouchers allocated to the VAMC between FY 2008 – FY 2019. If FY 2019 HUD- VASH vouchers are allocated to PHAs after September 30, 2019, they will not be included in the denominator for this metric.
DENOMINATOR EXCLUSIONS
1. Vouchers that are set aside for project-based housing developments that are not yet activated are excluded from the denominator. These vouchers will be included 90 days after operations begin at the project-based site and will be reflected in the following month’s denominator.
2. Vouchers allocated to PHAs from the FY 2020 voucher allocations will be excluded from the measure. If FY 2019 HUD-VASH vouchers are allocated to PHAs after September 30, 2019, they will not be included in the denominator for this metric.
3. Vouchers released to Continuum Programs as a part of the HUD-VASH Continuum Pilot will be excluded from the denominator. Note that in order for the denominator to reflect this exclusion, VAMCs participating in the pilot are responsible for notifying the HUD- VASH Program Office as vouchers are released.
SCORECARD DISPLAY EXCLUSION CODES
1. HUD Table Exclusion
2. HPF Voucher Status Exclusion
3. Additional Legacy Exclusion
5. HPF Form Exclusion
6. HOMES Current Status Table Exclusion
7. Veteran Deceased at Program Exit
8. HUD-VASH Exit Form Exclusion
10. External Date of Death
The number of Veterans who are homeless will decrease with each Veteran who obtains permanent housing through HUD-VASH. Placement into permanent housing is critical to ending Veteran homelessness, and HUD-VASH is VHA’s primary resource for accessing permanent supportive housing.
Page: 6 of 49 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2019
Number of Veterans with a HOMES Assessment who were unsheltered in the preceding 30 days.
POPULATION
Veterans engaged by VA Homeless Programs.
TARGET AND BUSINESS RULES
Targets will be set at the VAMC level. For VAMCs that exceeded the FY 2019 target, the FY 2020 target will be the total number of unsheltered Veterans engaged during FY 2019. Target adjustments will be considered for those sites that had an end-of-FY 2019 score of 95% or below and had a FY 2019 target of 100 or greater. Any adjustments to the FY 2020 targets must be requested by VAMCs that meet criteria for an adjustment. Adjustments will be made by the Homeless Program Office based on an analysis of Point in Time (PIT) Count, By-Name List (BNL), and estimated inflow data in the FY 2020 Gap Analysis tool. In addition to target adjustments, full exemptions from the measure will also continue to be applied for sites that do not have the unsheltered Veteran population to meet the target.
ESCALATING TARGETS AND STOP LIGHT FORMATTING
Q1: 25% or above (Green), 24% or below (Red) Q2: 50% or above (Green), 49% or below (Red) Q3: 75% or above (Green), 74% or below (Red) Q4: 100% or above (Green), 99% or below (Red)
NUMERATOR
Number of assessments conducted during FY 2020 in which Veterans spent at least 1 night in a place not meant for habitation in the 30 days prior to assessment.
o HOMES data source: Intake Assessment Form, item 20, option 25 (Place not meant for habitation).
o HOMES data source: Intake Assessment Form, item 21, is yes.
For VAMCs that exceeded the FY 2019 target, the denominator for this measure is the total number of Veterans assessed during FY 2019 who spent at least 1 night in a place not meant for human habitation in the 30 days prior to assessment. The denominator for sites that did not achieve the FY 2019 target will remain consistent with prior FY targets for this measure.
The denominator values for each VAMC included in the metric are as follows:

DENOMINATOR EXCLUSIONS
During FY 2016, the Health Care for Homeless Veterans (HCHV) Program established a mechanism by which VAMCs can be exempted from this measure if there is evidence there is not enough of an unsheltered Veteran population to meet the target. Sites with existing exemptions as of 10/1/2019 and guidance for requesting an exemption can be found here.
RATIONALE
Though the VA is making progress in ending Veteran homelessness, much of the gain has come in reducing the number of sheltered homeless Veterans, while the number of unsheltered homeless Veterans has remained relatively stable. Reducing the unsheltered homeless Veteran population is a very high priority for VA and other federal agencies, such as HUD and the United States Interagency Council on Homelessness. This measure is intended to prioritize outreach to unsheltered homeless Veterans, engaging them in the care needed to ultimately obtain and maintain permanent housing.

De-duplicated number of Veterans receiving Residential Treatment (RT), HUD-VASH, and Supportive Services for Veteran Families (SSVF) Rapid Re-Housing (RRH) homeless services who obtain permanent housing (PH).
For this metric, PH placements are captured for three possible cohorts:
1. Veterans who move in to PH through HUD-VASH in FY20.
2. Veterans with a HOMES program exit to PH in FY20 from RT programs (including GPD, HCHV CERS, and HCHV LDSH).
3. Veterans who move-in to housing or exit SSVF RRH services to PH in FY20.
TARGET AND BUSINESS RULES
There is no target for this measure.
N/A
Number of permanent housing placements in FY20.
1. Number of Veterans with a HUD-VASH move-in date between 10/1/19 and 9/30/20.
o HOMES data source: HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.
2. Number of Veterans with an RT program exit to PH between 10/1/19 and 9/30/20.
o HOMES data source: Residential Treatment Exit Form item 3, options 1 – 7 or 9.
3. Number of Veterans with an SSVF RRH housing move-in date between 10/1/19 and
9/30/20. If no move-in data is available, the Veteran will be counted in the measure if their SSVF RRH program exit is to a permanent destination type.
o HUD HMIS data source: Field 3.20, Housing Move-In Date and Field ID 3.12, Destination options 3, 10, 11, 19-23, 26, 28, and 31-34.
NUMERATOR EXCLUSIONS
A PH placement will only be counted once if it falls within the timeframes outlined below.
Duplicate placements to PH, due to Veterans receiving services from multiple programs, will be excluded from the numerator. These are the following cases in which this de-duplication will occur:
1. Veteran moves into PH through HUD-VASH and also has an RT program exit or SSVF RRH move-in/exit to PH within 30 days of the HUD-VASH move-in. The RT or SSVF RRH placement will be excluded from the numerator.
2. Veteran has an RT program exit to PH and an SSVF RRH move-in/exit to PH within 14 days of the HOMES program exit. The SSVF RRH placement to PH will be excluded from the numerator.
OP3 counts the number of placements rather than a percentage, so there is no denominator.
RATIONALE
VA and community PH placements are key to ending Veteran homelessness. This metric establishes a methodology to identify how many Veterans exit homelessness to permanent housing, thereby ending homelessness for those Veterans.
OP3: Permanent Housing Placements
Page: 10 of 49 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2019
Percentage of Veterans discharged from VA Grant and Per-Diem (GPD) Transition in Place (TIP) programs to permanent housing (PH).
POPULATION
Veterans who exit GPD TIP programs during FY 2020.
TARGET AND BUSINESS RULES
The FY 20 target for this measure is 75%. This target is intended to guide the field in the direction of greater prioritization of placement in PH in the community.
ESCALATING TARGETS AND STOP LIGHT FORMATTING
75% or above (Green), 70-74% (Yellow), 69% or below (Red)
NUMERATOR
Number of Veterans who exit directly to PH upon leaving a GPD TIP program in FY 2020.
o HOMES data source: Residential Treatment Entry Form, item 3b = 1 and 3e = Transition in Place.
o HOMES data source: Residential Treatment Exit Form, item 3, options 1 – 7 or 9.
Veteran is included in the denominator if Veteran has an exit date from a GPD TIP program between 10/1/19 and 9/30/20.
o HOMES data source: Residential Treatment Entry Form, item 3b = 1 and 3e = Transition in Place.
o HOMES data source: Residential Treatment Exit Form, item 1, exit date is between 10/1/19 and 9/30/20.
Veteran is excluded from the denominator under any of the following circumstances:
1. Veteran has a Length of Stay (LOS) of seven days or less.
o HOMES data source: Residential Treatment Exit Form, item 1 and exit date-entry date is < or = 7.
2. Veteran is ineligible for VA health care.
o Patient Enrollment File data source: The Health Eligibility Center(HEC) is the official source of enrollment data, and time to make an official determination of eligibility is dependent on a number of factors outside of the local VA medical center’s control. Therefore, Veterans with an eligibility status of “not eligible,” “pending,” “unverified,” “not applicable,” or “rejected” will be excluded from the denominator for this measure. Veterans with an eligibility status of “verified” and those who have never been processed through the HEC will be included.
3. Veteran is deceased at discharge.
o HOMES data source: Residential Treatment Exit Form, item 2, option 8.
4. Veterans who have been discharged from GPD programs due to threatened/actual violence to self or others.
o HOMES data source: Residential Treatment Exit Form, item 2a, option 1.
GPD-TIP1: Percent Exits to Permanent Housing
Page: 11 of 49 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2019
5. Veteran is placed into a hospital or another residential non-psychiatric medical facility, long-term care facility or nursing home, or psychiatric hospital or other psychiatric facility.
o HOMES data source: Residential Treatment Exit Form, item 3, options 17, 18, or
19.
6. Veteran is transferred to another residential program due to temporary or permanent suspension of program operations. Please submit a Veteran-level list and explanation for these requested exclusions to the GPD National Program Office at gpdgrants@va.gov for approval. Note: If the Veteran is readmitted to GPD, after the noted exclusion date, the Veteran will be included in the measure based on the new admission date.
1. Veteran has a Length of Stay (LOS) of seven days or less.
2. Veteran is ineligible for VA health care.
3. Veteran is deceased prior to discharge.
4. Veteran is placed into a hospital, long-term care facility or nursing home.
5. Veteran is discharged due to threatened violence to self or others.
6. Veteran is transferred to another residential program.
RATIONALE
VHA’s Plan to End Veteran Homelessness calls for residential and transitional housing programs to place a primary focus on discharging Veterans directly to permanent housing.
