B.2 PRICE SCHEDULE
The Contractor shall provide services to homeless Veterans as part of the Healthcare for Homeless Veteran (HCHV) Contract Emergency Residential Services (CERS) program services for the Roudebush VA Medical Center in Indianapolis, IN. Contractors will provide short-term residential treatment to Veterans who need an immediate housing placement as they seek permanent housing and/or additional care services.
The total minimum, or guaranteed minimum, for the resultant contract is one (1) bed per day for 365 days. The contract maximum, inclusive of all options exercised, shall not exceed 58,220 days.
Reimbursement will be made to the contractor by issuance of individual task orders for the base year and each option year, if exercised. Only the Contracting Officer is authorized to place annual task orders against this contract for the estimated amount of services for the specific timeframe.
Pricing Instructions: Fill in the Unit Cost and Total Cost for the Base Year and each Option Year. Add the aggregate total for the base and option years at the bottom of the pricing schedule in the space provided. In addition to pricing for normal operations, due to the uncertainty of the COVID-19 pandemic, it is requested that quoters also submit a separate estimated per diem rate to cover the costs related to COVID-19 if isolation measures are determined necessary by the Contracting Officer. (See ADDENDUM TO FAR 52.212-1, INSTRUCTIONS TO OFFERORS – COMMERCIAL ITEMS for the isolation options.)
The regular rate for normal operations will be utilized when it is determined by the Contracting Officer that isolation options are not necessary. The estimated COVID-19 per diem rate will be utilized when the Contracting Officer determines it is necessary to implement isolation options. For costs related to COVID-19, supporting documentation will be required to ensure fair and reasonable pricing.
Accordingly, any change to the per diem rate on the resultant contract will be made via a bi-lateral modification.
