About this file

This is a Contractor Safety Evaluation Form used by the City of Lakeland's Risk Management and Purchasing department to assess the safety qualifications and practices of contractors bidding on municipal contracts. The form requires contractors to provide comprehensive safety documentation including their accident statistics for the past three years, comparing their Frequency Rate of Medical Injuries and Lost Time Injury Rate against Bureau of Labor Statistics national averages for their specific SIC code. Contractors must also disclose their current Experience Modification Rate (EMR) and document their safety program components, including regularly scheduled employee safety meetings, documented safety and health training programs for new employees, written safety programs, and field safety inspection procedures. Additionally, contractors must acknowledge their understanding of and ability to comply with Process Safety Management (PSM) Standard requirements, which mandate that all employees receive training on fire, explosion, and toxic release hazards specific to their work, and that Pre-Work Safety Assessments or Job Hazard Analysis documents be completed and reviewed by all personnel before work begins on systems covered by the PSM Standard.

The form requires contractors to verify compliance with PSM training documentation requirements, including identity of contract employees, training dates, and verification of employee understanding. Any modifications to Pre-Work Safety Assessments or Job Hazard Analysis documents during a project must receive approval from the senior contractor supervisor on site and either the City of Lakeland Safety Manager or the North McIntosh Plant Superintendent or their designees. The form requests that a company officer sign the document to confirm the contractor's understanding and commitment to these requirements. Contractors are also provided an opportunity to supply additional information they believe demonstrates the strength of their company's safety program.
