This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ). Submit only written quotes for 246-22-Q-0124 for a Modular Building. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. This solicitation is a 100% Buy Indian, SB Set- Aside. The associated NAICS code is 321992 with a small business size standard of 500 Employees.

Questions regarding this solicitation must be submitted via email to sean.long@ihs.gov and shelton.bruce@ihs.gov NLT 4PM, Central Time, September 8, 2022. Questions will not be accepted after this date/time. All questions will be combined and posted as an amendment to the solicitation.
Evaluation: FAR 52.212-2 Evaluation -- Commercial Items (completed as follows):
(a) The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

- Price will be evaluated on a total award amount.
- Technical Specifications. The contractor shall submit a sheet of the technical specifications of the building that meet or exceed the specifications contained herein.
Evaluation will be on items 1 through 17 of the specifications Contractor shall submit enough details (cut sheet or proposed products on items 1 through 17 of the specifications to be evaluated.
- Delivery Schedule – The contractor shall provide a delivery schedule outlining delivery within 365 days ARO. The contractors shall submit a contingency schedule showing the manufacturing and delivery process to ensure delivery within in 90 days.
- Past Performance – The contractor shall provide three reference for projects of like or similar size in dollar value that occurred within the last three years. The references need to address delivery, cooperation with clients, efforts of like or similar requirements.

This solicitation will result in a firm fixed price purchase order pursuant to the terms and conditions below. Terms and conditions other than those stated herein will not be accepted. The above pricing is all inclusive.

C.11 HHSAR 326.226-4 NOTICE OF INDIAN SMALL BUSINESS ECONOMIC ENTERPRISE
SET-ASIDE
Under the Buy Indian Act, 25 U.S.C. 47, offers are solicited only from Indian Economic Enterprises (HHSAR 326.606) that are also small business concerns. Any acquisition resulting from this solicitation will be from such a concern. As required by HHSAR § 352.226-7(b), offerors shall include a completed Indian Economic Enterprise Representation form in response to Sources Sought Notices, Request for Information (RFI) and as part of the proposal submission. The Indian Economic Enterprise Representation form, available on the IHS DAP public website ( www.IHS.gov/DAP ), shall be included in synopses, presolicitation notices, and solicitations for the acquisitions under the Buy Indian Act. Offers received from enterprises that are not both Indian Economic Enterprises and small business concerns will not be considered and will be rejected.

C.12 HHSAR 326.226-4 NOTICE OF INDIAN ECONOMIC ENTERPRISE SET-ASIDE
(a) Definitions as used in this clause:
Alaska Native Claims Settlement Act (ANCSA) means Public Law 92-203 (December 18, 1971), 85 Stat. 688, codified at 43 U.S.C. 1601-1629h.
Indian means a person who is an enrolled member of an Indian Tribe or “Native” as defined in the Alaska Native Claims Settlement Act.
Indian Economic Enterprise means any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit provided that: The combined Indian or Indian Tribe ownership must constitute not less than 51 percent of the enterprise; the Indians or Indian Tribes must, together, receive at least a majority of the earnings from the contract; and the management and daily business operations of an enterprise must be controlled by one or more individuals who are Indians. To ensure actual control over the enterprise, the individuals must possess requisite management or technical capabilities directly related to the primary industry in which the enterprise conducts business. The enterprise must meet these requirements throughout the following time periods:
(i) At the time an offer is made in response to a written solicitation;
(ii) At the time of the contract award; and
(iii) During the full term of the contract.
Indian Tribe means an Indian Tribe, band, nation, or other recognized group or community that is recognized as eligible for the special programs and services provided by the United States to Indians because of their status as Indians, including any Alaska Native village or regional or village corporation under the Alaska Native Claims Settlement Act (Pub. L.
92-203, 85 Stat. 688; 43 U.S.C. 1601).
Representation means the positive statement by an enterprise of its eligibility for preferential consideration and participation for acquisitions conducted under the Buy Indian Act, 25 U.S.C. 47, in accordance with the procedures in Subpart 326.606.
(b) General. (1) Under the Buy Indian Act, offers are solicited only from Indian Economic Enterprises.
(2) The CO will reject all offers received from ineligible enterprises.
(3) Any award resulting from this solicitation will be made to an Indian Economic Enterprise, as defined in paragraph (a) of this clause.
(c) Required submissions. In response to this solicitation, an offeror must also provide the following:
(1) A description of the required percentage of the work/costs to be provided by the offeror over the contract term as required by section 352.226-6, Subcontracting Limitations clause; and
(2) Qualifications of the key personnel (if any) that will be assigned to the contract.
(d) Required assurance. The offeror must provide written assurance to the CO that the offeror is and will remain in compliance with the requirements of this clause. It must do this before the CO awards the Buy Indian Act contract and upon successful and timely completion of the contract, but before the CO accepts the work or product.
(e) Non-responsiveness. Failure to provide the information required by paragraphs (c) and (d) of this clause may cause the CO to find an offer non-responsive and reject it.
(f) Eligibility.
(1) Participation in the Mentor-Protégé Program established under section 831 of the National Defense Authorization Act for Fiscal Year 1991 (25 U.S.C. 47 note) does not render an Indian Economic Enterprise ineligible for contracts awarded under the Buy Indian Act.
(2) If a contractor no longer meets the definition of an Indian Economic Enterprise after award, the contractor must notify the CO immediately and in writing. The notification must include full disclosure of circumstances causing the contractor to lose eligibility status and a description of any actions that the contractor will take to regain eligibility. Failure to give the CO immediate written notification means that:
(i) The economic enterprise may be declared ineligible for future contract awards under this part; and
(ii) The CO may consider termination for default if it is in the best interest of the government.
(g) Representation. Under the Buy Indian Act, 25 U.S.C. 47, offers are solicited only from Indian Economic Enterprises (326.606). As required by HHSAR 352.226-7(b), offerors shall include a completed Indian Economic Enterprise Representation form in response to Sources Sought Notices, Request for Information (RFI) and as part of the proposal submission. The Indian Economic Enterprise Representation form, available on the IHS DAP public website ( www.IHS.gov/DAP ), shall be included in synopses, presolicitation notices, and solicitations for the acquisitions under the Buy Indian Act. C.13 HHSAR 326.226-6 ECONOMIC ENTERPRISE SUBCONTRACTING LIMITATIONS
(a) Definitions as used in this clause.
(1) Indian Economic Enterprise means any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit provided that: The combined Indian or Indian Tribe ownership must constitute not less than 51 percent of the enterprise; the Indians or Indian Tribes must, together, receive at least a majority of the earnings from the contract; and the management and daily business operations of an enterprise must be controlled by one or more individuals who are Indians. To ensure actual control over the enterprise, the individuals must possess requisite management or technical capabilities directly related to the primary industry in which the enterprise conducts business. The enterprise must meet these requirements throughout the following time periods:
(i) At the time an offer is made in response to a written solicitation;
(ii) At the time of the contract award; and
(iii) During the full term of the contract.
(2) Subcontract means any contract, as defined in FAR subpart 2.1, entered into by a subcontractor to furnish supplies or services for performance of the prime contractor or subcontractor. It includes, but is not limited to, purchase orders, and changes and modifications to purchase orders.
(3) Subcontractor means any supplier, distributor, vendor, or firm that furnishes supplies or services to or for a prime contractor or another subcontractor.
(b) Required Percentages of work by the concern. The contractor must comply with FAR 52.219-14, Limitations on Subcontracting clause in allocating what percentage of work to subcontract. The contractor shall not subcontract work exceeding the subcontract limitations in FAR 52.219-14 to a concern other than a responsible Indian Economic Enterprise.
(c) Any work that an IEE subcontractor does not perform with its own employee shall be considered subcontracted work for the purpose of calculating percentages of subcontract work in accordance with FAR 52.219-14 Limitations on Subcontracting.
(d) Cooperation. The contractor must:
(1) Carry out the requirements of this clause to the fullest extent; and
(2) Cooperate in any study or survey that the CO, Indian Health Service or its agents may conduct to verify the contractor's compliance with this clause.
(e) Incorporation in Subcontracts. The contractor must incorporate the substance of this clause, including this paragraph (e), in all subcontracts for general services, A&E services and construction awarded under this contract.

C.14 HHSAR 352.226-7 - INDIAN ECONOMIC ENTERPRISE REPRESENTATION
(a) The offeror must represent as part of its offer that it does meet the definition of Indian Economic Enterprise (IEE) as defined in HHSAR 326.601 and that it intends to meet the definition of an IEE throughout the performance of the contract. The offeror must notify the contracting officer immediately, via email, if there is any ownership change affecting compliance with this representation.
(b) The representation must be made on the designated IHS Indian Economic Enterprise Representation form or any successor forms through which the offeror will certify that the ownership requirements defined by HHSAR 326.601 are met.
(c) Any false or misleading information submitted by an enterprise when submitting an offer in consideration for an award set-aside under the Buy Indian Act is a violation of the law punishable under 18 U.S.C. 1001. False claims submitted as part of contract performance are subject to the penalties enumerated in 31 U.S.C. 3729 to 3731 and 18 U.S.C. 287.

SPECIFICATIONS
GENERAL:
A. All applicable documents shall be distributed as follows:
B. All work shall conform to, and comply with, 2014 FGI Hospital and Outpatient
Facility Guidelines, the latest edition/version of the IBC and any other required federal, state, and local laws, rules, regulations, codes, and standards.
C. Vendor shall provide training on the temporary modular building of its internal
HVAC / Electrical /Plumbing / Mechanical equipment of their operations, maintenance, cleaning, and any pertinent information.
D. All new materials and equipment shall be furnished with any owner’s manual, parts list, local authorized dealer contact information, and warranty ending date.
E. All new materials and equipment shall meet specifications listed below and shall have approved certificates of installation if required by ALL applicable federal, state, and local codes, standards, or regulations.
F. Vendor shall provide any manufacturer warranties on the manufacturer’s standard form as required.
G. Vendor shall provide any applicable technical care training to the Lawton Service
Unit facility staff and engineering for maintenance and preventive maintenance.
General Modular Building Site-Installations Requirements
This Scope of Work shall be to obtain a modular building to be installed on the
Lawton Indian Hospital campus.
The modular building shall be required to house the following:
Modular Building: 9,285 Approximate GSF
To house the following services:
Lawton Indian Hospital COVID Modular Building (approx. 9,285 GSF)
Modular Building to include: contractor shall provide and install
The contractor shall provide the following services for the Modular Building:
o Structural design reviewed and stamped by a licensed Structural PE o Electrical design reviewed and stamped by a licensed Electrical PE o Mechanical design reviewed and stamped by a licensed Mechanical PE o Fire sprinkler system design reviewed and stamped by a Licensed PE o Conceptual Interior design with furniture and dimensions.
o Sub-floor ventilation design o Provide Utility locations for exterior of the building (water, sewer, electrical, internet, gas) o Consult with the Lawton Indian Hospital and provide interior/exterior design and floor plan layout based on the needs of the owner, this shall include all MEP (mechanical, electrical, and plumbing), data communication, security systems, fire systems, all colors and paints for exterior and interior, the approximate GSF is conceptual, will be selected by the final design and floor plan shall be based on the consultation meetings.
o Consult with the Lawton Indian Hospital and provide security camera locations on the floor plan layout based on the needs of the owner, the final location and number of cameras will be determined based on the consultation meetings o Consult with the Lawton Indian Hospital and provide security door system locations on the floor plan layout based on the needs of the owner, the final location and number of card readers will be determined based on the consultation meetings o Provide all interior/exterior utility hook ups to modular building: electrical, plumbing (water and sewer), telephone and internet connections, Fire alarm connections, gas o Provide owner specifications for the building’s foundation, utilities- MEP(mechanical, electrical, plumbing), phone, internet, (specifications must be provided before the construction of the modular building) o IT/Networking/Tele-communication design and layout o installing the modular building on the foundation the owner provides per the contractor’s specification for the modular building
1. Anchoring system/Foundation contractor shall provide and install o Anchoring System shall be designed for 110 mph wind rating.
o Modular sections shall be designed and have lifting points for future leveling adjustments.
2. Exterior Access contractor shall provide and install o Provide 10 lockable access hatches to access the crawl space 3 front length, 3 back length, 2 side width, 2 side width of the building exterior access. Hatches to be a minimum of 24”x36” and of steel construction with an integrated latch.
o Provide 2 access ladders to the roof on the building (corners of the building owner will work with contractor to provide the locations of the ladders)
3. Data/communications: (must meet the hospitals specifications for all IT equipment) contractor shall provide and install o All data cables will be plenum rated, contractor shall install all data communications o Each office/cubicles will have cat-6 double drops on two sides of the office walls and two sides of the cubicles walls.
o Conference room will have electrical and data connections in the floor per the owner’s specified locations, two drops per data box o Installation of data/communication systems (Wiring/Punch Blocks, punches will type b).
o Final connection and testing of data/communications systems including test report.
All cat-6 drops will be tested and certified.
o All operator, installation, and technical manuals must be provided for all equipment, modules and units installed.
o Wires, cables, switches, indicators, boxes, units, and other peripheral devices are to be permanently labeled or marked.
4. Camera system: contractor shall provide and install o Contractor shall install and run all cat-6 network drops for camera system installation.
o Per the camera system layout determined by the owner, the contractor shall provide and install 24 Axis cameras.
o Contractor shall connect the camera system to the Lawton Indian Hospital’s existing camera system, contractor shall certify the system is operational.
o Contractor shall provide approximately 24 cameras (8-interior/12-exterior) to the following (4 additional cameras):
12 exterior cameras on the outside of the building 8 interior cameras on the interior of the building Locations will be determined in the contractor’s design meeting.
5. Security Door System: contractor shall provide and install (Contractor must meet DSX specifications)
a. Contractor shall provide and install the security door system matching the Lawton’s existing security door system.
b. Contractor shall install security door system in accordance with the manufactures specifications and be operational with the existing systems.
c. The security door system layout will be determined by the owner and contractor, the contractor will provide and install the security door system
d. Contractor shall connect the security door system to the Lawton Indian Hospitals existing security door system and make operational.
e. Contractor shall provide approximately 20 card readers to match the existing card reader and security system to the following:
i. Building entrance doors
ii. Supply rooms
iii. House Keeping closets
iv. IT closet
v. Soiled Utility room
vi. Mechanical room
vii. Electrical room
viii. Conference room doors
ix. Additional exit doors
x. 4 additional card readers
As-built drawing/schematic of the system in its entirety and individual units as well as a parts list as is applicable are to be supplied to facilitate future updates, troubleshooting and repair of the system.
All parts and materials must be new.
6. Fire Alarm: (Cut sheets Required) contractor shall provide and install o NFS-320(E), Intelligent Addressable Fire Alarm System, Notifier by Honeywell or equivalent. System must have capability to interface with the existing fire alarm system at the Lawton Indian Hospital.
o HS-NCM-W, HS-NCM-MF, HS-NCM-SF, HS-NCM-WMF, HS-NCM-WSF, HS- NCM-MFSF, High-Speed Network communications Modules, Notifier by Honeywell or equivalent. System must have capability to interface with the existing fire alarm system at the Lawton Indian Hospital.
o FSP-851 (A), Intelligent Plug-In Ionization Smoke Detector with FlashScan, Notifier by Honeywell or equivalent. System must have capability to interface with the existing fire alarm system at the Lawton Indian Hospital.
o NBG-12LX, Addressable Manual Pull Station, Notifier by Honeywell or equivalent.
System must have capability to interface with the existing fire alarm system at the Lawton Indian Hospital.
o SpectrAlert Advance, Selectable Output Notification Appliances, Notifier by Honeywell or equivalent. System must have capability to interface with the existing fire alarm system at the Lawton Indian Hospital.
o Connect fire alarm system to the existing Lawton Indian Hospital fire alarm system o Duct detectors shall be installed with remote enunciators to interface with Fire
Alarm System, installed by certified installer, certified and tested o Final connection and test of fire alarm system.
o All operator, installation, and technical manuals must be provided for all equipment, modules and units installed.
o Wires, cables, switches, indicators, boxes, units, and other peripheral devices are to be permanently labeled or marked in accordance with industry installation standards.
o A drawing/schematic of the system in its entirety and individual units as well as a parts list as is applicable are to be supplied to facilitate future updates, troubleshooting and repair of the system. Inventory of all devices will be included on drawings.
o All parts and materials must be new.
7. Flooring: (Color board provided by Lawton Indian Hospital) contractor shall provide and install o Floor Covering: VCT Tile o 12" x 12" x 1/8" commercial Vinyl Composite Tile (VCT/LVT), owner will make material and color selection o Restrooms to use ceramic tile.
o Cove Base:
o 4" Vinyl/Rubber Base o cove base shall be continuous roll matt finish or equivalent o Stainless Steel corner guards (2”x 48”), the guards will be anchored with screws
8. Exterior roof/walls/doors: (Cut sheets Required) contractor will provide and install o Exterior doors will be out-swing o Front entry way doors – shall be double doors with automatic door operators
(Beesam operators or equivalent), doors shall be NFPA code compliant, Full view glass, ADA compliant, commercial aluminum, bronze in color o Secondary entrances shall accordance to Life Safety Code, secondary doors shall be steel doors with welded frame for each door, the doors shall have quarter glass windows, door color shall be determined by the owner o Mechanical rooms shall have an exterior door, no window, shall be steel doors with welded frame for each door, door color shall be determined by the owner o Roof will use a mulehide roof with a 20 year minimum warranty to standardize with the other modular buildings on the campus. Roof to include reinforced walkway surface going to air handlers from ladder access points.
o Roof must include a lightning protection system.
o Scuppers, guttering, etc., must use commercial grade aluminum extruded metal equivalent too adjacent modular buildings.
o Fiber cement stucco pattern exterior siding to match the nearby modular buildings.
o Exterior trim to be made of fiber cement or SMART wood grained OSB.
o Owner will provide the specifications for the concrete cast stone skirting to match other nearby buildings. Contractor will provide and install.
o Lawton Indian Hospital will provide entrance canopy specifications..
o Exterior walls to be 2x6” with R-19 minimum wall insulation.
o Exterior walls to be sheathed with 7/16” OSB or ½” plywood, must be wrapped in
Tyvek or equivalent breathable house wrap.
o Roof to be insulated with R-38 minimum.
9. Interior walls/doors/ceilings: (Cut sheets Required) contractor will provide and install o Doors must be solid core prefinished wood, oak, dark walnut o Install metal frames 16 gauge and knock down jams for all doors o All enclosed offices, restrooms, and conference room walls and ceilings will be sound insulated and rated at STC-45 o Hardware - Grade 1 - lever/lever (handles) o Permanent Lock Cores, Yale Lever Lock AU5407LN-626 or equivalent. Locks must be ADA compliant. System must be compatible with the existing Lawton Indian Hospital lock system. (Para key way) o Restroom doors shall be supplied with Yale-Privacy Lever Lock 5402LN 626 or equivalent. (single restrooms only) o Security locks will utilize card readers/electric strikes o All interior doors with door closers will have stainless steel Kick Plates o STC rated door systems o DuraClean smooth plastic, 24”X48” (White) ceiling tiles or must use the hospitals standard ceiling tile, owner will provide specifications o Armstrong Wall Molding, main tees and cross tees, or equivalent.
o Install stainless steel Corner guards on all outside corners in the interior of the modular building o Corbin Russwin Door closers 6200 series or equivalent for the following doors:
o Restrooms o Housekeeping closet o Data/Communication closet o Sprinkler closet o Storage closets o Mechanical rooms o Floors will be constructed of 1-1/8” tongue and groove plywood o Floor underlayment will be 5/8” AC plywood underlayment o Floors to be insulated with R-30 with vapor barrier between floor joists.
o Wall studs shall be constructed of metal studs o Walls locational must meet equipment specifications (e.g. reinforced to support additional weight, etc.)
o Interior walls to be 2”x4” metal studs.
o Drywall will be 5/8” type-x drywall in all interior walls except for bathrooms.
Restrooms will require water rated cement board/backer board.
o Restrooms will use ceramic tiles for walls and floor.
o Windows must be cased with drywall and finished.
10. Glass: (Cut sheets Required) contractor will provide and install o Windows will be 36x48 bronze aluminum single hung, doubled paned, and tinted.
o Glazing - Dual/Clear o Force Protection Framing & Laminated Glass o Blinds shall be constructed wood, BALI economy 2” faux wood blinds or equivalent
11. Electrical: (Cut sheets Required) contractor will provide and install o 208V/120V three phase o Interior Sub-Panels will be recessed with clip on breakers, with aluminum busses and which are sized to code.
o All breaker boxes shall be square D door in door or equivalent, with hinged doors.
o Labeling of panel boxes shall be engraved electrical plates; multi-layered acrylic material is suitable for indoor or outdoor use. Labels must meet code.
o Electrical must be conduit or Hospital grade MC Cable.
o Interior Lights will be 2' x 4' and 2’ x 2’ lay-in led flat panels o Exterior Lights will be led at each entry and 6 additional exterior lighting that come equipped with photocells.
o Exit lights shall be self-testing Led with a battery backup. (contractor will meet the hospitals exit specifications) o Exit lights Led with emergency lights, with a battery backup. (contractor will meet the hospitals exit specifications) o All exit lights shall be on a dedicated circuit o Hospital grade Receptacles/(Child proof where required by owner) shall be duplex and 20 Amp o Dedicated receptacles shall be 20 Amp per owners locations o GFI receptacles shall be 20 Amp o Service outlets shall be exterior mounted for HVAC units o Switches shall be 20 Amp o Floor boxes shall be duplex with cover plate. The floor boxes and data boxes will be installed per the owners required locations o Data and communication boxes shall be empty J-Boxes w/ 3/4" Sch. 40 Elec. PVC conduit stubbed above ceiling o All electrical wiring must be grounded according to the National Electric Code.
o Subfloor crawlspace to include additional receptacles and lighting per owner’s requirements.
12. Plumbing: (Cut sheets Required) contractor will provide and install o Water riser must have all code required shut offs, drains, backflows, and be located in the mechanical/riser room.
o Water line must be PEX, the water system underneath the building will insulated and wrapped with heat tape for the entire system.
o Shutoff valves (ball valves) must be made of chrome o Waste line will be PVC-DWV Sch 40 Drain and will include vent with multiple drops, per code.
o Gas lines must be black steel exterior and interior o Water heaters must be electric and sized per design requirements o Toilets shall be American Standard No. 5905.100 or equivalent. These toilets must have an elongated bowl and be ADA compliant. Restrooms must be ADA compliant.
o Bemis 1955C toilet seats or equivalent.
o Install flush meters, Sloan Regal Model or equivalent.
o Urinals in restrooms shall be American Standard or equivalent and be ADA compliant.
o Mop Sink shall be a mop service basin, Zurn Z1996-24 24” or equivalent. The faucet shall be a Delta Faucet 28T9-AC 28T Two Handle 8-inch or equivalent wall mount service sink faucet. SS backsplash around mop sink.
o Water fountains (LK wall mount) shall be ADA compliant o Non-restroom sinks shall be commercial single bowl stainless steel with delta faucets classic series single handle kitchen faucet 100-WF or equivalent.
o Recessed washer boxes with a drain and water supply.
o All restroom sinks shall install 590 series Delta sensor faucets or equivalent and shall be ADA compliant.
o Drains located under sinks shall be insulated.
o Provide floor drains for all restrooms and mechanical rooms/riser room with trap primers.
o Hammer arresters in restrooms with flush valves w/access door o 4 - Wall mounted frost/free water hydrants will be provided on the four corners of the building, all outside hydrants will have a water key o Circular pumps will be designed and installed for the hot water system (continuous hot water through the building)
13. HVAC system: (Cut sheets Required) contractor shall provide and install o Lennox HVAC units (RTU) or equivalent. The RTU shall be over designed by at least 20% for future expansion). All units shall be controlled by the Lawton Indian Hospital DDC. Contractor shall make connections to the Lawton Indian Hospital’s
DDC.
o Lennox HVAC units shall be equipped with gas and electrical emergency heat.
o All HVAC systems must meet healthcare standards.
o Package units will be installed on the roof and ducted into the building.
o Contractor shall install a Prodigy Control System or equivalent to interface with the
Lawton Indian Hospital’s DDC o HVAC units shall be equipped with Indoor Air Quality Systems UVC Germicidal
Lights or equivalent o Each unit shall be equipped with hail guards o Interior mounted filters o Interior duct shall be 26 gauge sheet metal insulated o Return Duct interior 26 gauge sheet metal insulated o Registers will be adjustable dampers.
o Contractor shall perform a test and balance on HVAC system o Grilles must be metal insulated 2X2 drop-ins o Thermostat will not need DDC o Ceiling mounted exhaust fans as required by code o Air exchanges per code o Contractor shall provide start up and testing of system o All operator, installation, and technical manuals must be provided for all equipment, modules and units installed o Wires, cables, switches, indicators, boxes, units, and other peripheral devices are to be permanently labeled or marked in accordance with industry installation standards.
o As-built drawing/schematic of the system in its entirety and individual units as well as a parts list as is applicable are to be supplied to facilitate future updates, troubleshooting and repair of the system.
o HVAC diffusers will be attached with a <5’ foil flex duct, ninety degree turns will be installed on diffusers with a minimum of three screws o No flex duct on returns o Exhaust fans shall be direct drive to be vented to atmosphere with rigid duct
(contractor must use owner’s specifications) o Diffuser return grill locations to be approved by submitting a drawing with layout and design o Exhaust fans will be surface mounted with three inch discharges to atmosphere o Faucets and hose bibs will be installed by each RTU (must be frost free) o Mini split system will be installed as a backup HVAC system for the server room, the mini split system must meet the hospital’s specifications o RTUs will supply heating and air to the mechanical room, plus, a mini split system will be installed as a backup HVAC system for the mechanical room, the mini split system must meet the hospital’s specifications o All parts and materials must be new.
14. Millwork: Lawton Indian Hospital shall provide

o Stainless steel waste receptacle in restrooms.
o Wall mounted frameless mirrors in restrooms.
o Signage for offices, restrooms, closets, shall be custom engraved plates; multi-layered acrylic material is suitable for indoor, outdoor signage for the building shall comply with the Lawton Indian Hospital’s signage design.
o 3/4" Plywood phone board with fire retardant paint. (IT room) o Toilet paper dispensers for all restrooms (hospital will provide the specifications) o Paper towel dispenser for all restrooms and break rooms (hospital will provide the specifications) o Door operators for the restrooms (must be ADA compliant)
16. Fire Protection: (Cut sheets Required) contractor shall provide and install o Fire alarm boxes - empty double gang recessed boxes with 1/2" conduit above ceiling o Fire alarm control panel cabinet - semi-recessed cabinet with 1/2" conduit above ceiling o Installation of fire sprinkler system o Sprinkler system will interface with fire alarm system o Install riser with backflow preventer and fire department connection o Semi-recessed fire extinguisher metal cabinets at each entry o All fire extinguishers will be multipurpose dry chemical (ABC) o Smoke detectors will be required for all RTUs

Provide conference room 6 voice/data connections in the center of the conference room per owner’s requirements.

SURVEY DATED FEBRUARY 5, 1976 DONE BY CHARLIE W. BURKS PLS # 264.
3. THE PREVIOUS SURVEY NOTED THE DISTANCE FROM THE EXISTING RAILROAD CENTERLINE TO THE WEST PROPERTY LINE WAS 50' WE HELD THE 50' AND HONORED THE PIN FOUND AT THE SW CORNER OF
