MEASUREMENT & PAYMENT
PART 1-GENERAL
1.01 SECTION INCLUDES

1.03 The Contract Documents set forth intent that the Contractor be responsible for determining the means, methods, and quantities of all work which may be required for the completion of the project. The bid items listed on the bid breakdown are for comparison of the bids and may be used as a method of determining the value of work performed for partial payment requests.
1.04 Work not specifically covered by the Contract Bid Schedules but necessary for the completion of the work as described in the Contract Documents will be deemed to be otherwise included in the Contractor's unit prices. No additional payment to the Contractor will be made for items not specifically listed on the Contract Bid Schedules.
1.05 The Contractor shall assume all risks in determining the quantities of each bid break down and for including as a part of the prices the appropriate percentage to cover all Contractor's overhead and profit, equipment rental and maintenance, and all other costs not specifically described on the Contract Bid Schedule but necessary to carry out the work as required by the Contract Documents. When totaled together, the totals for each item of the Contract Bid Schedules shall be the total contract price for the work of this Contract. No other payments will be made to the Contractor except as specifically authorized by change order.
1.06 Each unit or lump sum price stated on the Bid Form shall constitute full compensation as herein specified for each item of work completed in accordance with the drawings and specifications.
1.07 Under the price specified to be paid for each item, the Contractor shall furnish all materials and equipment, furnish all labor, and do all operations necessary to complete all work specified or shown. All supervision, overhead items, protection, and precautions and all other costs incidental to the construction work completed and as specified are included.
1.08 A complete, finished, working job, as intended by the general nature of these
Section 01025 Measurement and Payment 01025- 2 specifications, shall be produced whether or not any particular wording or direction is omitted or inadvertently not clearly stated.
1.09 Measurement for payment shall be by the City, except where noted elsewhere in this specification. Measurement for partial payment for lump sum items shall be on the basis of percentage of work complete and in place. Measurement for final payment shall be on the basis of the specified item, in-place, tested and accepted by the Owner or his representative, plus all incidental and supporting requirements fulfilled for the individual item and the project in total.
1.10 The prices for those items which involve excavation shall include compensation for disposal of surplus excavated material and handling water.
1.11 The prices of all pipe items shall constitute full compensation for furnishing, laying, jointing, and testing of pipe; excavation and backfill; all site restoration including cleanup.
1.12 All partial pay descriptions shall be understood as less standard retainage whether stated or not.

PART 3-MEASUREMENT AND PAYMENT
3.0 MOBILIZATION & DEMOBILIZATION – BID ITEM NO. 1
Shall be paid for lump sum and consist of preparatory work and operation for beginning work on this project. Work includes labor, equipment, materials and any permits necessary to complete the work and not covered by a specific pay item. Initial payment shall not exceed 25% of the total amount for Mobilization/Demobilization. Subsequent payments shall be based on completion percentage of project for each payment application. Lump sum amount shall not exceed 5% of the sum of the unit price pay items.
The standard retainage will be applied to these allowances. Partial payments made on this item shall in no way act to preclude or limit any of the provisions for partial payments otherwise provided for by the contract.
3.01 BONDING AND INSURANCE – BID ITEM NO. 2
A. Description: The Contractor shall provide Comprehensive General Liability insurance, builders risk including premises, workmen’s compensation and automobile insurance as required. Contractor to provide bonding as required.
B. Measurement: The quantity to be paid for under this Item shall be measured as one lump sum quantity.
C. Payment: The pay quantities for the work specified under this Item shall

3.02 MAINTENANCE OF TRAFFIC - BID ITEM NO. 3
A. Description: The work specified in this Item consists of maintaining traffic within the limits of the project for the duration of the construction period including any temporary suspensions of the work. It shall include the construction and maintenance of any necessary detour facilities; the preparation of maintenance of traffic plans prepared, signed, and sealed by a Florida Professional Engineer; the providing of necessary facilities for access to residences, businesses, etc. along the project; the furnishing, installing and maintaining of traffic control and safety devices during construction, the control of dust, and any other special requirements for safe and expeditious movement of traffic as may be called for on the plans. The term, maintenance of traffic, as used herein, shall include all of such facilities, devices and operations as are required for the safety and convenience of the public as well as for minimizing public nuisance, all as specified in this Section.
B. Measurement: The quantity of maintenance of traffic to be paid for under this Item shall be measured as one lump sum quantity. Partial payments will be pro rated throughout the duration of construction of this Project in direct proportion to the percent completion of the work, as determined by the ratio of the payment requested to the total contract price.
C. Payment: The quantities, as determined above, shall be paid for at the contract lump sum price set out in the Proposal, which price and payment constitutes full compensation for all the work.
3.03 SURVEY LAYOUT/AS-BUILTS - BID ITEM NO. 4
A. Description: This bid item describes measurement and payment for establishing all lines and grades required for all work under this contract, as well as “as-built” survey drawings for the proposed water main and forcemain installation including mains, valves, air relief valves and fittings with horizontal and vertical locations described by bearings, distances and stations. The as-built drawing shall be provided signed and sealed by a Surveyor licensed in the State of Florida. Contractor’s surveyor shall provide five (5) copies of the certified as-builts and a CAD file on a CD for all work. As-builts shall be reviewed by City of Brooksville Building Division (COBBD) and kept on record.
B. Measurement: The quantity of to be paid for under this Item shall be measured as one lump sum quantity. Partial payments will be pro rated throughout the duration of construction of this Project in direct proportion to the percent completion of the work, as determined by the ratio of the payment requested to the total contract price.

3.04 PRE/POST COLOR AUDIO VIDEO RECORDS – BID ITEM NO. 5
A. Description: This item shall include payment for all work required as specified to prepare pre-construction and post-construction records of the work area. Copy to be retained by COBBD.
B. Measurement: Measurement shall be as a single lump sum item of activity.
C. Payment: Payment for the lump sum bid price shall be 50% upon completion and acceptance of the pre-construction video, and 50% upon completion and acceptance of the post-construction video.
3.05 EROSION AND SEDIMENT CONTROL – BID ITEM NO. 6
A. Description: The Contractor shall provide erosion and sediment control in accordance with FDEP, NPDES permit and drawings. Contractor shall erect all erosion control measures as indicated on the drawings, including but not limited to silt fence, hay bales and Turbidity barriers. All turbidity barriers shall be maintained and inspected during construction by a Qualified Stormwater Management Inspector and reporting.
B. Measurement: The quantity to be paid for under this Item shall be measured as one lump sum quantity.
C. Payment: The pay quantities for the work specified under this Item shall be the lump sum price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.06 CLEARING AND GRUBBING – BID ITEM NO. 7
A. Description: The Contractor shall provide the removal and disposal of all trees, stumps, roots and other protruding structures and objects; existing pavements, demolition and removal of existing structures, removal and disposal of deleterious materials and all other incidental work necessary for the complete preparation of the site.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment

3.07 ASPHALT REPLACEMENT – BID ITEM NO. 8, 33 & 34
A. Description: The Contractor shall provide all excavation, installation and materials, compaction of subgrade, base, asphalt pavement or concrete, testing, and incidentals necessary to construct the driveway as shown on the plans.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.08 SOD RESTORATION OF DISTURBED AREAS – BID ITEM NO. 9 & 35
A. Description: The Contractor shall provide sod for all disturbed areas within the right of way, easements and project area. All costs of material, including sod, labor, equipment, fertilization, watering, and maintenance shall be included in the lump sum price. Sod placed within residential areas shall be placed and matched including type of sod.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described. Partial payments for sod shall be no more than 75% until 90 days maintenance is completed or the time of Substantial Completion.
3.09 CURB & GUTTER – BID ITEM NO. 10
A. Description: The Contractor shall provide all labor, materials, equipment and installation necessary installation of Type F curb and gutter.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.10 REMOVE EXIST. ASPHALT OR CONCRETE – BID ITEM NO. 11, 12, 36 & 37
A. Description: The Contractor shall provide all labor, materials, and equipment necessary remove and property dispose of existing concrete and asphalt paved areas.
B. Measurement: The quantities to be paid for under this item will be per unit

3.11 SIDEWALK INSTALLATION – BID ITEM NO. 13 & 38
A. Description: The Contractor shall provide all excavation, installation and materials, compaction of subgrade, base, asphalt pavement or concrete, testing, and incidentals necessary to construct the driveway as shown on the plans.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.12 WATER MAIN INSTALLATION – BID ITEM NO. 17, 18, 39, 40, 41 & 42
A. Description: This bid item describes measurement and payment for furnishing and installing polyvinyl chloride (PVC) installed in open cut trenches. The pay quantities for the work specified under this section shall be the number of linear feet of the types and sizes of pipe specified in the applicable pay items actually constructed and accepted.
B. Measurement: Measurement will be horizontally above the centerline of the pipe.
C. Payment: Payment will be per linear foot according to the diameter, thickness class, and type of joint, as set forth on the proposal. The unit bid price includes furnishing and installing all pipe by open cut, and materials above or below ground along the pipeline alignment;
polyethylene encasement, joints and jointing materials, joint restraints, making pipe connections, pipe coatings, maintenance of flow, bypass pumping as required, thrust blocking, clearing; sheeting, shoring and bracing; dewatering, excavation; bedding and backfill; compaction and testing; protection and adjusting of existing above ground and underground utilities and service connections; excavation and construction beneath existing ditch blocks including protection or repair of ditch blocks; disposal of spoil; tracer wire, identification tape, hydrostatic testing; flushing, pigging, cleaning, erosion control; protection of culverts and drainage facilities, surface restoration, concrete curb replacement, concrete sidewalk replacement, temporary pavement replacement and all other related and necessary materials, work and equipment required to construct a complete and operable pipeline. For unit price for open cut under paved areas, price includes removal and disposal of asphalt and/or concrete pavement, appropriate compaction, compaction testing, Section 01025 Measurement and Payment 01025- 7 pressure testing, disinfection and testing, resurfacing in kind, and all incidentals necessary to complete the work except where such are shown to be paid for under a separate item.
3.13 DIRECTIONAL DRILL WATER MAIN INSTALLATION – BID ITEM NO. 43
A. Description: This bid item describes measurement and payment for furnishing and installing polyvinyl chloride (PVC) installed by horizontal directional drilling (HDD).
B. Measurement: Measurement will be horizontally above the centerline of the pipe and includes length of any valves or fittings.
C. Payment: Payment for the work specified under the Section shall be per linear foot for the types and sizes of pipe specified in the applicable pay items, actually constructed and accepted. The bid price includes pre and post recorded video inspections of existing storm culverts being crossed by the HDD, furnishing all materials, including pipe, tracer wire, plus all the work, tools, equipment, wire line tracking system including electronic locator wire riser assembly complete materials, and incidentals required to construct the pipe by HDD, as shown on the plans and/or as described in the specifications, complete, in place, and accepted. The work includes, but is not limited to assembling, welding, supporting stringing, pulling, contact grouting, testing, pigging, flushing, cleaning, test pits, excavation, backfill, sheeting, shoring, bracing, dewatering, protection of culverts and drainage facilities, erosion and sedimentation control, sod, locating existing utilities in advance of performing any drilling, providing a drilling plan in accordance with the contract documents, providing bore logs to the Owner and all incidentals necessary to complete the work except where such items are shown to be paid for under a separate item.
3.14 REMOVE EXIST. WATER MAIN – BID ITEM NO. 19, 20, 44, 45, 46, 47, & 48
A. Description: This bid item includes the excavation, removal and disposal of water main removed as indicated in the construction documents.
B. Measurement: Measurement will be horizontally above the centerline of the existing pipe.
C. Payment: Payment for the work specified under the Section shall be per linear foot for the types and sizes of pipe specified in the applicable pay items actually removed and disposed of.
3.15 THERMOPLASTIC STRIPING – BID ITEM NO. 14, 15 & 16
A. Description: The Contractor shall provide all thermoplastic striping installation and material necessary for installation.
B. Measurement: Measurement will be horizontally along the striping installed.

3.16 REMOVE EXISTING FIRE HYRANT ASSEMBLY - BID ITEM NO. 22 & 51
A. Description: The bid item describes measurement and payment for removing and disposing of fire hydrant assemblies as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.17 FIRE HYDRANT ASSEMBLY – BID ITEM NO. 21 & 52
A. Description: The bid item describes measurement and payment for furnishing and installing fire hydrant assemblies as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: The quantities for the work paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.18 WATER SERVICE RELOCATION – BID ITEM NO. 23 &53
A. Description: The bid item describes measurement and payment for furnishing and installing appurtenances necessary for the relocation of existing water services as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: The quantities for the work paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.19 PRESSURE TEST AND CHLORINATION – BID ITEM NO. 24 & 55
A. Description: This bid item describes measurement and payment for pressure testing, flushing, chlorinating and testing of the relocated water lines. The pay quantities for the work specified under this section shall be the number of linear feet of the types and sizes of pipe specified in the applicable pay items actually constructed and accepted.

A. Description: This bid item describes measurement and payment for the installation of gate valves with boxes as shown on the Contract Drawings or where directed by the Project Representative

3.21 RELOCATE BACKFLOW PREVENTER – BID ITEM NO. 26
A. Description: This bid item describes measurement and payment for the installation of a backflow preventer including all necessary fittings as shown on the Contract Drawings or where directed by the Project Representative. The pay quantities for the work specified under this section shall be the BFP’s and sizes specified in the applicable pay items actually constructed and accepted.
B. Measurement: The quantities for the work paid for under this item will be per unit price furnished, installed, completed and accepted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.22 GATE VAVLE ASSEMBLIES – BID ITEM NO. 49 & 50
A. Description: This bid item describes measurement and payment for the installation of gate valves with boxes as shown on the Contract Drawings or where directed by the Project Representative. The pay quantities for the work specified under this section shall be the number of valves and sizes specified in the applicable pay items actually constructed and accepted.
B. Measurement: The quantities for the work paid for under this item will be per unit price furnished, installed, completed and accepted.

3.23 CONNECT TO EXISTING (WET TAP) – BID ITEM NO. 54
A. Description: This bid item describes measurement and payment for furnishing and installing tapping saddle or sleeves with gate valves and performing wet tap as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: Measurement will be on an individual basis for each wet tap and valve and box assembly acceptably installed.
C. Payment: Payment will be according to the size or type of tapping assembly as set forth on the Proposal. The unit bid price includes furnishing and installing all incidental items including boxes, restraints etc.

3.24 SAMPLE POINT - BID ITEM NO. 56
A. Description: This bid item describes measurement and payment for furnishing and installing sample points on the water main as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: Measurement will be on an individual basis for each sample point acceptably installed.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.25 TEMPORARY WATER CONNECTION - BID ITEM NO. 57
A. Description: This bid item describes measurement and payment for furnishing and installing all appurtenances required with a temporary water connection as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: Measurement will be on an individual basis for each temporary water connection acceptably installed.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.26 8” PVC GRAVITY SEWER (6’-8’ IN DEPTH) – BID ITEM NO. 27
A. Description: This bid item describes measurement and payment for furnishing and installing polyvinyl chloride (PVC) installed in open cut trenches. The pay quantities for the work specified under this section shall be the number of linear feet of the types and sizes of pipe specified in the applicable pay items actually constructed and accepted.

3.27 ADJUST MANHOLE LID – BID ITEM NO. 28 & 58
A. Description: This bid item describes measurement and payment for furnishing all materials necessary to adjust existing manhole lids as shown on the Contract Drawings or where directed by the Project Representative.
B. Measurement: Measurement will be on an individual basis for each manhole lid acceptably adjusted.
C. Payment: The pay quantities for the work specified under this Item shall be per unit price set out in the Proposal, which price and payment constitutes full compensation for all the work described.
3.28 STANDARD MANHOLE (8’-10’ IN DEPTH) – BID ITEM NO. 29
A. Description: This bid item describes measurement and payment for furnishing and installing standard manholes in open cut trenches. excavation and construction beneath existing ditch blocks including protection or repair of ditch blocks; disposal of spoil; testing; cleaning, erosion control; protection of culverts and drainage facilities, surface restoration, work and equipment required to construct a complete and operable pipeline. For unit price for open cut under paved areas, price includes removal and disposal of asphalt and/or concrete pavement, appropriate compaction, compaction testing, pressure testing, disinfection and testing, resurfacing in kind, and all incidentals necessary to complete the work except where such are shown to be paid for under a separate item. All testing and reports to be submitted to COBBD prior to payment.
3.29 TV/PRESSURE TEST – BID ITEM NO. 30
A. Description: This bid item describes measurement and payment for furnishing all necessary equipment to pressure test and TV the gravity sewer system.
B. Measurement: The quantities to be paid for under this item will be per unit price furnished, installed, completed and accepted.

PROJET MEETINGS
PART 1 – PRE-CONSTRUCTION CONFERENCE
1.01 Before starting work, a Pre-Construction Conference will be held to review and determine methods of scheduling and progress reporting of this project, establish procedures for handling Shop Drawings and other submissions and for processing applications for payment, and to establish a working understanding between the parties as to the City of Brooksville Building Division (COBBD), Engineer, the Contractor, and the Superintendent.
PART 2 – SCHEDULES, REPORTS AND RECORDS
2.01 The Contractor shall submit to the Engineer and COBBD such schedule of quantities and costs, progress schedules, payrolls, reports, estimates, records, and other data where applicable as are required by the Contract Documents for the work to be performed.
2.02 The Contractor shall also submit a schedule of payments that anticipates what they will earn during the course of the work to the Engineer and COBBD.
PART 3 – OBSERVATIONS
3.01 Periodic observations will be held throughout the work at the discretion of the Engineer and COBBD to verify progress and compliance to Contract Documents, Pay Requests, and general quality control.
3.02 Pre-Final Observations are held for the purpose of substantiating completion of the work and preparing a punch list of any deficiencies or corrections to be made. Pre-finals should be made with a representative of the Contractor, Engineer and the COBBD.
3.03 Final Observations will be held prior to acceptance in order to verify that all corrections and/or deficiencies have been performed or resolved and such inspections shall be mandatory prior to approval of final pay request. Finals shall be made with a representative of the Contractor, Engineer, and the COBBD.

SUBMITTALS
PART 1 - GENERAL
1.01 This section outlines in general the items that the Contractor must prepare or assemble for submittal during the progress of the work. Costs for the work under this section shall be included in the appropriate items of the Contractor's bid prices. There is no attempt herein to state in detail all of the procedures and requirements for each submittal. The Contractor's attention is directed to the individual Specification sections in these Contract Documents which may contain additional and special submittal requirements. The City reserves the right to direct and modify the procedures and requirements for submittals as necessary to accomplish the specific purpose of each submittal. Should the Contractor be in doubt as to the procedure, purpose, or extent of any submittal, he should direct his inquiry to the Engineer.
1.02 For submittals required with the bid, see Instructions to Bidders, Special Conditions, Proposal, and General Conditions.
PART 2 - ADMINISTRATIVE SUBMITTALS
2.01 The Contractor shall provide all of the submittals required by the General Conditions, Special Conditions, and as may be specifically required in other parts of the Documents.
2.02 The Contractor is reminded of his obligation as required by law to make required submittals promptly to the applicable federal, state, or local agency. Failure to comply with this requirement may result in the withholding of progress payments and make the Contractor liable for other prescribed action and sanctions.
PART 3 - TECHNICAL SUBMITTALS
3.01 GENERAL
A. Requirements in this section are in addition to any specific requirements for submittals specified in other divisions and sections of these Contract Documents.
B. Submittals shall be addressed to:

C. Submitted data shall be fully sufficient in detail for determination of compliance with the Contract Documents.
D. Review or acceptance of substitutions, schedules, shop drawings, lists of materials, and procedures submitted or requested by the Contractor shall not add to the Contract amount, and all additional costs which may result therefrom shall be solely the obligation of the Contractor.

E. The City is not precluded, by virtue of review, acceptance, or approval, from obtaining a credit for construction savings resulting from allowed concessions in the work or materials therefore.
F. It shall not be the responsibility of the City to provide engineering or other services to protect the Contractor from additional costs accruing from such approvals.
G. No equipment or material for which listings, drawings, or descriptive material is required shall be fabricated, purchased, or installed until the Engineer has reviewed same and returned copies with stamp and signature indicating action taken.
H. Submittals will be acted upon by the Engineer as promptly as possible, and returned to the Contractor not later than the time allowed for review in Subsection 3.02, SHOP DRAWING SUBMITTAL PROCEDURE. Delays caused by the need for resubmittals shall not constitute reason for an extension of the Contract time.
3.02 SHOP DRAWING SUBMITTAL PROCEDURE
A. The Contractor should refer to the General Conditions for related requirements.
B. The Contractor shall submit to the Engineer for his review five (5) copies of shop drawings, electrical diagrams, and catalog cuts for fabricated items and manufactured items (including mechanical and electrical equipment) furnished under this Contract. In-lieu of five (5) hard copies, the Contractor may submit electronic (.pdf) copies to the Engineer, who will review, stamp and return electronic copies via e-mail. Shop drawings shall be submitted in sufficient time to allow the Engineer not less than 15 regular working days for examining the shop drawings. One copy of same with Engineer’s comments to be submitted to COBBD for review and approval.
C. These shop drawings shall be accurate, distinct, and complete, and shall contain all required information, including satisfactory identification of items, units, and assemblies in relation to the Contract Drawings and Specifications.
D. Shop drawings shall be submitted only by the Contractor, who shall indicate by a signed stamp on the shop drawings, or other means, that the Contractor has checked and approved the shop drawings, and that the work shown is in accordance with Contract requirements and has been checked for dimensions and relationship with work of all other trades involved. The practice of submitting incomplete or unchecked shop drawings for the Engineer to correct or finish will not be acceptable. Shop drawings which, in the opinion of the Engineer, clearly indicate that they have not been checked by the Contractor will be considered as not complying with the intent of the Contract Documents and will be returned to the Contractor for resubmission in the proper form.
E. When the shop drawings have been reviewed by the Engineer, three sets of submittals will be returned to the Contractor appropriately stamped. In the case of electronic submittals, an electronic copy will be returned. If major changes or corrections are necessary, the shop drawing may be rejected and four (4) sets, or electronic copy will be returned to the Contractor with such changes or corrections indicated, and the Contractor shall correct and resubmit the shop drawings in the same manner and quantity as specified for the original

submittal, unless otherwise directed by the Engineer. If changes are made by the Contractor (in addition to those requested by the Engineer) on the resubmitted shop drawings, such changes shall be clearly explained in the transmittal letter accompanying the resubmitted shop drawings.
The Engineer review stamp will contain the following language:
Review is for general compliance with contract documents. No responsibility is assumed for correctness of dimensions or details.

MAKE CORRECTIONS NOTED - Implied acceptance of the item(s) provided the noted changes are made. Resubmittal is not required assuming all noted corrections are made.
REVISE AND RESUBMIT - Submittal item(s) have a sufficient number of corrections and are sufficiently in non-conformance with the Specifications so that resubmittal is required before the item(s) can be accepted.
F. The review of such shop drawings and catalog cuts by the Engineer shall not relieve the Contractor from responsibility for correctness of dimensions, fabrication details, and space requirements, or for deviations from the Contract Drawings or Specifications, unless the Contractor has called attention to such deviations in writing by a letter accompanying the shop drawings and the Engineer accepts the change or deviation in writing at the time of submission; nor shall review by the Engineer relieve the Contractor from the responsibility for errors in the shop drawings.
G. The Contractor agrees that shop drawing submittals processed by the Engineer do not become Contract Documents and are not Change Orders; that the purpose of the shop drawing review is to establish a reporting procedure and is intended for the Contractor's convenience in organizing his work and to permit the Engineer to monitor the Contractor's progress and understanding of the design.

3.03 RECORD DRAWINGS
The Engineer will prepare a set of Record Drawings for the project which will include the changes made in materials, equipment, locations, and dimensions of the work. Each month, or as otherwise agreed, the Contractor shall submit to the Engineer a current listing and description (written and graphic) of each change incorporated into the work since the preceding submittal. Copy of same to be submitted to COBBD for review and record.
3.04 SAMPLES AND TEST SPECIMENS
A. Where required in the Specifications, and as determined necessary by the Engineer, test specimens or samples of materials, appliances, and fittings to be used or offered for use in connection with the work shall be submitted to the Engineer at the Contractor's expense, with information as to their sources, with all cartage charges prepaid, and in such quantities and sizes as may be required for proper examination and tests to establish the quality or equality thereof, as applicable.
B. All samples and test specimens shall be submitted in ample time to enable the Engineer to make any examinations necessary, without delay to the work. The Contractor will be held responsible for any loss of time due to his neglect or failure to deliver the required samples to the Engineer, as specified.
C. The Contractor shall submit additional samples as required by the Engineer to ensure equality with the original approved sample and/or for determination of Specification compliance.
D. Laboratory tests and examinations that the City elects to have made by an independent testing laboratory will be made at no cost to the Contractor, except that, if a sample of any material or equipment proposed for use by the Contractor fails to meet the Specifications, the cost of testing subsequent samples shall be borne by the Contractor.
E. All tests required by the Specifications to be performed by an independent laboratory shall be made by an approved laboratory. Certified test results of all specified tests shall be submitted in duplicate to the Engineer. The samples furnished and the cost for the laboratory services shall be at the expense of the Contractor and included in the prices bid for the associated work.
F. Sample items (fixtures, hardware, etc.) may be incorporated into the work upon approval, and when no longer needed by the Engineer for reference.
3.05 CERTIFICATES OF COMPLIANCE
A. A Certificate of Compliance shall be furnished for materials specified to a recognized standard or code prior to the use of any such materials in the work. The Engineer may permit the use of certain materials or assemblies prior to sampling and testing if accompanied by a Certificate of Compliance. The certificate shall be signed by the manufacturer of the material or the manufacturer of assembled materials and shall state that the materials involved comply in all respects with the requirements of the Specifications. A Certificate of Compliance shall be furnished with each lot of material delivered to the work and the lot so certified shall be clearly identified in the certificate.

B. All materials used on the basis of a Certificate of Compliance may be sampled and tested at any time. The fact that material is used on the basis of a Certificate of Compliance shall not relieve the Contractor of responsibility for incorporating material in the work which conforms to the requirements of the Contract Documents and any such material not conforming to such requirements will be subject to rejection whether in place or not.
C. The Engineer reserves the right to refuse permission for use of material on the basis of a Certificate of Compliance.
D. The form of the Certificate of Compliance and its disposition shall be as directed by the Engineer. Copy of all certificates of compliance shall be provided to the COBBD monthly or as otherwise agreed.
3.06 PRE-CONSTRUCTION VIDEO
A. Contractor shall video the construction site, including all proposed areas to be disturbed including, but not limited to force main route, connection to existing structures, laydown or storage areas, parking lot, sidewalks, curbs, driveway and roadway crossings, existing adjacent structures, utilities and landscaping. Provide two (2) copies of audio-video in DVD format. This item is not included in the contract unit prices and is considered incidental to this project. Copy to be provided to COBBD for record.
PART 4 - PAYMENT

PART 1 - GENERAL
1.01 TECHNICAL SPECIFICATIONS FOR SITE & UTILITY CONSTRUCTION
A. All site work on this project shall be governed by the most stringent applicable provisions of the specifications, guidelines, standards and criteria included in the references listed below. In the event any item of work is not addressed on the plans or in these referenced manuals, then the applicable provisions of the Florida Department of Transportation Standard Specifications for Road and Bridge Construction, current edition and supplements thereto shall apply, except as otherwise specifically approved by the Engineer.
The following are incorporated herein by reference:

1.02 INTENT OF CONTRACT DOCUMENTS
The intent of this contract is to provide for the construction and completion of every detail of the work described in the contract to the lines, elevations and grades shown on the plans.
Should any detail or details be omitted from these plans and/or specifications be incorrect, it shall be the responsibility of the Contractor to bring this omission or inaccuracy to the attention of the Engineer and to furnish and install said details to properly complete the work so that the project will serve its intended function.
1.03 FLORIDA TRENCH SAFETY ACT
Contractor shall be solely responsible for complying with the Florida Trench Safety Act and Occupational Safety and Health Administration excavation safety standards, 29 CFR

1926.650 (subpart P) as amended. All costs associated with complying with these requirements shall be included in the appropriate items of the Bid.
1.04 CONSTRUCTION BARRICADES
The Contractor shall provide, erect, and maintain effective barricades, danger signals, and signs on all intersecting streets or highways and in other locations where required for the protection of the work and the safety of the public. Barricades or obstructions which encroach on, or are adjacent to public rights of way shall be provided with lights which shall be kept burning in conformity with state, county and local laws and regulations. The Contractor shall be responsible for all damages resulting from a neglect or failure to meet these requirements.
1.05 PROJECT RECORD DOCUMENTS
During the entire construction operation, the Contractor shall maintain records of all deviations from the Drawings and Specifications and shall prepare therefrom “record” drawings showing correctly and accurately all changes and deviations from the work made during construction to reflect the work as it was actually constructed. These drawings shall conform to recognized standards of drafting, shall be neat and legible. Specific minimum requirements are noted on the drawings.
1.06 DAMAGE TO EXISTING STRUCTURES AND UTILITIES
The Contractor shall be responsible for and make good all damage to pavement, buildings, fences, mail boxes, telephone or other cables, water pipes, sanitary pipes, survey markers or other structures which may be encountered, whether or not shown on the Drawings.
The Contractor shall be responsible to restore all disturbed areas within the road right-of-way back to its original condition including ditch blocks, MES and storm culverts and sod all disturbed areas.
Information shown on the Drawings as to the location of existing utilities has been prepared from the most reliable data available to the Design Engineer. This information is not guaranteed, however, and it shall be the Contractor's responsibility to determine the location, character and depth of any existing utilities. Contractor shall notify owners of adjacent property and of underground facilities and utility owners when prosecution of the Work may affect them, and shall cooperate with them in the protection and replacement of their property.
1.07 ADJUSTMENTS OF UTILITY CASTINGS, COVERS AND BOXES
All existing utility castings, including valve boxes, junction boxes, manholes, hand holes, pull boxes, inlets and similar structures in the areas of trench restoration and pavement shall be adjusted by the Contractor to bring them flush with the surface of the…
