AMENDMENT 01

RESPONSES TO VENDOR QUESTIONS

1. Can you please verify the due date is indeed July 24th. It does state on the solicitation form that date but there is another date that mentions July 17th?

Response: The due date for proposal submission had been Friday, July 17, 2015.

However, the due date for proposal submission has been extended to Monday, July 20, 2015 by 2:00PM EST.

2. May we receive a copy of the RFQ in Word format?

Response: Yes. The RFQ in Word is attached as Atch 1-Solicitation_CFP-15-Q-00050.

3. Can you please explain what SAM is (copy of registration) found on page 17, #2?

Response: SAM is System for Award Management and can be assessed via https://www.sam.gov. SAM is the primary supplier database for the Federal Government. All vendors/suppliers wanting to do business with the federal government must be registered in the SAM database. It is a free service. Vendors are also requested to complete the representations and certifications in SAM and submit them with your proposal.

4. Is this a follow-on requirement? If so, could you please provide the incumbent contract number for this opportunity?

Response: This is a follow-on requirement. The contract number cannot be provided at this time.

However, you may contact the Bureau’s Public Affairs office via www.consumerfinance.gov, and request that information through the Freedom of Information Act (FOIA) procedures.

5. Do you want 1 electronic copy sent and no paper copies?

Response: Yes.

6. Also, to clarify, In Section II, Letter A Item 3a, the instructions say we may not submit a zip file.

Does this mean we are prohibited from adding all of our single PDF files to a zip for easier sending/compressing? If we are in fact unable to submit in zip file format, can we combine all single documents into one PDF? Ex. One PDF for Volume I docs and one PDF for Volume II docs.

Response: Zip files are prohibited. You may combine single documents into one PDF eg.. One PDF for Volume I and one PDF for Volume II Docs.

7. Are there forms to complete to indicate that a vendor’s intent to bid?

Response: No.

8. Why are you going out to bid? Is it the standard every 5 year mandatory?

Response: No response merited.

9. Attachment 3 of the proposal, Claims Report-Vision Claims and Premium, is not sufficient to respond to the RFQ for a geo-access report, in Section II, E.1.b.

Response: Offerors are asked to please request the additional census data required to complete the geo-access report, via contacting the Contracting Officer, jeremy.jenkins@cfpb.gov and/or

Contract Specialist, denise.henderson@cfpb.gov . The additional census data will be emailed to you promptly upon your request.

10. In an effort to promote HUBZone and Disabled Veteran Owned Small Business, will you please consider vendors proposing as an insurance agent and broker (524210)? Will you also please include the HUBZone price evaluation preference?

Response: This procurement seeks a carrier to provide vision benefits services under NAICs Code 524114, Direct Health and Medical Insurance Carriers. The services required are being competed under full and open competition and is not suitable for a set-aside.

11. What are the list of items, schedule of requirements, scope of work required?

Response: Please see solicitation, Section I, Performance Work Statement

12. What are names of eligible countries to participate in this procurement?

Response: This procurement is full and open competition. There are no restrictions.

13. What is the estimated budget for this purchase and are there any addendum or Pre-Bid Meeting

Minutes?

Response: The budget will not be disclosed to the public. There are no pre-bid meeting minutes/addendum.

14. What other vision carriers are bidding on this?

Response: No response merited.

14. Will there be an extension or bidding deadline?

Response: Yes. The updated proposal due date is now Monday, July 20, 2015, by 2:00PM EST.

15 At a minimum, can the census information include date of birth, state of residence, five digit zip code, enrollment tier (ie… individual, individual plus one, and individual and family)?

Response: We can provide the age and zip code. Please see response to Question 9.

16. Can a copy of VSP's current plan Certificate be provided?

Response: Yes. The Evidence of Coverage certificate is provided as an attachment to this Amendment as Atch 2_EvidenceofCoverage.

17. Is the prospective plan design requirement outlined in Section I, 3.1 of the Performance Work Statement identical to the current CFPB plan design?

Response: Yes.

18. If plan design changes have been made within the last three contract option periods, can they be identified and include the effective date?

Response: There have been no changes in the plan design in the last three contract periods.

19. As far as the out of network coverage there are some that are higher than your current out of network currently. Are you requesting the higher out of network. For example, Trifocal 127.00

(currently 100.00), lenticular 165.00 (currently 134.00)?

Response: Please see the below table for updated coverages. The italicized areas below have been updated to reflect current plan design requirements. Vendors must be able to meet all plan design requirements specified. Please note any deviations to the Plan Design requirements may render the offeror’s proposal technically unacceptable.
