About this file
The document is a Certification Statements form for state contract bidding, specifically for the state of Ohio, which requires vendors to declare preferences for Domestic Source End Products, Ohio Preference, and/or Veteran Friendly Business Enterprise (VBE) status. The form provides a structured method for bidders to certify the origin of their products and services, with detailed sections for declaring whether items are manufactured in the United States, Ohio, or border states, and whether the company meets specific economic presence criteria.
The certification form allows bidders to claim preferences by demonstrating compliance with various state requirements, such as having significant economic presence in Ohio (including paying state taxes, being registered with the Secretary of State, and having a minimum number of employees in Ohio or border states), manufacturing products domestically, or being a certified Veteran Friendly Business Enterprise. The document warns that intentionally submitting false or misleading information can result in immediate disqualification and potential legal action, including possible debarment. Bidders must carefully complete the appropriate sections to receive any preference consideration during the evaluation process.
