About this file

This document comprises procurement terms and conditions established by the Ohio Bureau of Workers' Compensation (BWC) effective October 2025, applicable to a Cisco Server Hardware and Extended Service solicitation (BWC260597) for the State of Ohio. The solicitation is designated specifically for Minority Business Enterprise (MBE) dealers on the RSC024735 contract. The contract period runs from February 1, 2026, through June 30, 2026, with the procurement intended to provide Cisco server hardware and related extended service solutions to the Ohio Bureau of Workers' Compensation.

Key contractual requirements include completion of an Affirmative Action Program Verification Form for all contracts valued at $5,000 or more, submission through the Ohio Department of Development. New state suppliers must complete a Supplier Information Form and IRS Form W-9 through the Ohio Supplier Portal, while current suppliers need only submit a completed W-9 reflecting their registered tax identification. All suppliers must complete and submit the Contractor/Subcontractor Affirmation and Disclosure Form affirming compliance with prohibitions on offshore services, Russian institutional investments, and non-discrimination requirements. The State encourages procurement from MBE and EDGE vendors and reserves the right to recover any funds paid for services performed or data maintained outside the United States without proper waiver authorization. Sales and use tax blanket exemptions are available upon request.
