Subject: Prequalification for B212 / LS212 D&D NAICS 562910 Remediation Services
Lawrence Livermore National Security, LLC (hereinafter called “LLNS”) is seeking sources for the performance of demolition and decontamination in support of LLNS’ requirements.
The objective of the project is to support decommissioning and demolition (D&D) of the B212 and LS212 sites located in Livermore, California. The objective of this effort is to ensure the complete removal of hazardous materials, subsurface structures demolition , utilities, and hazardous materials abatement and to prepare the site for its future use as a flat unimproved area.
The project requires addressing projects with legacy issues, development of characterization plans, execution of data-gap sampling, and the formulation of demolition and waste management solutions.
LLNS is seeking qualified sources to support its requirements for the execution of waste characterization, waste removal, and final waste disposition activities at the site, experience with projects of similar scope and magnitude worked at Federal or similar institutions and can meet LLNS prequalification requirements.
To qualify, contractors must meet the requirements shown below. If your firm meets the requirements below, submit the requested information by the due date, 3/13/2025.
While not required to be submitted at this time, LLNS may ask the contractor to provide financial documents in the future.
1) Contractor License
• Provide a copy of your Contractor License and date of expiration.
2) Insurance
• Name of insurance carrier and local broker.
• A certificate of insurance showing that the Contractor can meet the insurance requirements listed below.

3) Safety Records
• Completed “Subcontractor Safety Prequalification Questionnaire”
• Provide a copy of the contractor’s corporate safety plan OR IIPP (Illness/Injury
Prevention Plan)
• Provide a signed statement from the contractor's workers compensation carrier listing the contractor's California Experience Modification Rates (EMR) for the last three years. If your EMR rates are above 1.0 in the last three years, please provide a written explanation of the incident(s) and of the safety measures implemented to reduce the rate.
• Provide a written explanation of the corrective measures that are being implemented by your company to improve its safety if any of the following occurs:
i. Occurrence of any fatalities in the last three years,
ii. Receipt of any OSHA citations within the past five years,
iii. Days Away or OSHA Recordable Incident Rates for the past three years are above the national average found on Table 1 of the Bureau of Labor Statistics website (http://www.bls.gov/iif/oshsum.htm).
• Provide the contractor's NAICS code for OSHA reporting purposes.

i. Include form 300 if there are accidents/injuries reported on 300A forms
ii. Mitigation plan to prevent future occurrence
4) Supplier Quality Program Information Worksheet
• Completed “Supplier Quality Program Information Worksheet”
• Attach a copy of your quality management systems certifications
5) Experience
• Provide (3) Three or more project examples of decommissioning and demolition (D&D) projects requiring characterization plans, execution of data-gap sampling, and the formulation of demolition and waste management solutions.
i. Value of the work performed by the contractor's own forces on all projects listed above.
ii. Name, phone number and email address of the owner’s representative for each project listed above.
• Provide examples or explanations of prior radiation protection plans provided.
6) Capability
• Provide a Capability Statement about the type of work your company specializes in
• Select the relevant NAICS codes below to indicate what type of work your company specializes in WITHOUT needing to subcontract the work to a lower-tier subcontractor.

• The small business size standard 1,000 employees or less.
• Refer to Subpart 14 Size Standards of the Federal Acquisition Regulation (FAR) for qualification.
• Complete the attached Representations and Certifications form
8) Staffing
• Provide a statement identifying the number of US Citizens employed, or the ability to staff projects adequately with US Citizens.
• Provide evidence of enrollment as a Federal Contractor in the E-Verify System by providing a copy of the Contractor's ‘Company Information’ page printed directly from the E-Verify System.
i. The copy of the ‘Company Information’ page from E-Verify System must show employer category information.
ii. Do not provide the form you used to enroll in E-Verify, Do provide proof that your company has successfully enrolled as described above.
Email the completed attachments, and information requested in a portable document format (PDF) to Nicholas Michlig michlig1@llnll.gov no later than March 13, 2024, 4:00PM PST. Each requirement should be sent in separate files, except for OSHA Logs that can be combined into one file for all seven years.
LLNS will conduct an evaluation of your response to determine if all of the information requested is provided. Failure to provide any information may result in disqualification. LLNS will evaluate the remaining responses to determine if, in the LLNS’ opinion, the contractor is deemed qualified for successful performance of the type of work required.
